Serum hyaluronate in rheumatoid arthritis: study by affino-immunoenzymatic assay.

Serum level of hyaluronate (HA) was assayed by an affino-immunoenzymatic method using hyaluronectin, in 25 patients with rheumatoid arthritis (RA), 24 patients with ankylosing spondylitis (AS) and 103 controls. The mean serum HA level was significantly higher (p less than 0.001) in RA (171 +/- 39 micrograms/l) than in controls (22.4 +/- 1.6 micrograms/l) and AS patients (30.3 +/- 3.0 micrograms/l). The mean HA level in RA on corticosteroids (98.2 +/- 25.9 micrograms/l) was significantly lower (p less than 0.05) than that of RA without steroids (220.1 +/- 60.5 micrograms/l). The serum HA levels were not correlated with clinical or serological indicators of inflammatory activity. This increase in serum HA probably reflected an excessive production by rheumatoid synovium and not a lowered catabolism, all patients having normal hepatic function. The origin of the increased synthesis is still unknown. These results suggest that serum hyaluronate is not a universal indicator of inflammation but is perhaps a characteristic of RA.

[Importance of analogs of LHRH in the treatment of bone metastasis of cancer of the prostate in 1988].

LHRH analogous drugs cause a marked decrease of the testicular secretion, similar to that noted after pulpectomy or estrogen therapy. The experience acquired enables to consider LHRH analogous drugs as alternative in the first intention treatment of the bone metastases prostatic cancer if pulpectomy is refused or if estrogen therapy indicated because of its cardiovascular risks. It is not yet whether the systematic and long-term adjunction of an anti-androgen to LHRH analogous drugs, really increases its efficacy. In the late stage of bone metastases, when the effects of pulpectomy estrogen therapy have ended, LHRH analogous drugs only result in when the tumor process is still hormone-dependent.

[The epidemiology of multiple myeloma in France].

From mortality and population data provided by INSERM and INSEE, we have determined the mortality rate of multiple myeloma--uncorrected, specific and standardized rates--which have enabled to conduct an epidemiological, demographic, geographic and chronological study. The demographic study done in 1981-1982, confirms that the incidence of multiple myeloma is linked to ageing and that it is higher in men. The geographic study does not find any preferential regional distribution. The chronological survey done between 1958 and 1982, shows an increased mortality rate of about 500 p. cent and this increase is seen exclusively in patients over 55. Such progression analyzed according to the model age, period, cohort, is either compatible with a cohort effect, reflection of a true incidence increase, or with a period effect depending upon age, reflection of an artificial increase linked to modifications of hygiene conditions. This last factor seems to predominate. Anyway, decreased mortality rates have been observed for the most recent generations, which should, in the future, mean a stagnation or even a decrease in the incidence of multiple myeloma.

[Osteoarticular manifestations of pustulosis palmaris et plantaris].

In 6 patients with clinically and histologically proven pustulosis palmaris et plantaris the disease was complicated by osteo-articular manifestations. Five patients presented with articular symptoms and one with aseptic subacute osteomyelitis. The distribution of these bone and joint disorders was different from that of Sonozaki's "pustulotic arthro-osteitis": in contrast with the latter, the anterior chest was inconstantly involved whereas the spine, sacro-iliac joints and peripheral articulations were more frequently affected. Although the B27 antigen is usually absent, it seems possible to classify the rheumatic disorders of pustulosis palmaris et plantaris among seronegative spondylo-arthropathies.

Taq I polymorphism of HLA class I genes in an ankylosing spondylitis family.

A 3.5-kb HLA class I fragment is polymorphic in an ankylosing spondylitis (AS) family. All B27 AS patients show the 3.5-kb band, which is also present in B12 AS patients in this family. When hybridized with an HLA-B-specific probe, the polymorphic band is revealed in B12 patients, but not in B27 patients.

[Genetic study of a family of subjects with ankylosing spondylitis].

In a family of four patients with ankylosing spondylarthritis, the study of HLA typing has permitted to establish the dissociated transmission of the B27 antigen and the spondylarthritis: as a matter of fact, if the father and his two sons have the disease, and carry the B27, one of the daughters is also definitely affected with spondylarthritis according to New York criteria, and does not carry the B27. This young woman, also, does not present in her haplotype the genes of susceptibility to psoriasis, B13, B17, CW6, DR7, nor the antigens giving cross reactions with B27, type CREG, B7, B22, B40. So, this young woman seems to have inherited from her father a genetic predisposition toward the disease without transmission of antigen B27 and it is supposed that if this gene is linked to the HLA system by an unbalanced binding, it was transmitted after recombination in her father.

[Bone lymphomas].

Non-hodgkin's lymphomas have benefitted from recent progresses in pathology and from new classifications having a prognostic value. In reference to a case of primary osseous lymphoma, we mention their definition and main characteristics. We stress the good sensitivity of the osseous scintigraphy to detect the osseous localizations of lymphomas. These secondary osseous lymphomas have a poor prognosis. The difficulties of the pathological diagnosis are frequent, but should decrease with the wider use of immunohistochemical marking of the bone or node biopsies.
