[Risk factors for cystic kidney disease in hemodialysis patients].

Acquired bilateral cystic transformation of kidneys in patients on chronic dialysis has been increasingly noted by morphological and clinical studies. Our study aims to identify a group of high-risk patients for incidence and extent of such disease. We studied with ultrasonography 87 patients on maintenance hemodialysis (60 men and 27 women), median age 58 years (range 18 to 81), mean duration of hemodialysis 75 months (range 2-256). The presence of cysts was divided into five grades from 0 no cysts to 4 more than 15 cysts detected for each kidney. 74 patients 85% of our dialytic population had acquired cystic disease; cysts varied in size from 0.5 to 4.5 cm in diameter. The cysts were found in 97% of patients dialyzed for more than 8 years and in 73% of patients on hemodialysis for less than 3 years. The incidence of cystic disease was highly and significantly correlated with the duration of dialysis, the grade correlated with duration of chronic renal failure and hemodialysis. The extent of disease appears to be increased in males. Because of high incidence of disease in long term dialysis patients ultrasonography monitoring is recommended for patients on dialysis for more than 3 years with more attention for males.

Keeping fit when injured.

Staying physically fit while injured can improve the patient's outlook, both physiologically and psychologically, and help him or her return to regular activities completely, safely, and with decreased risk of re-injury. Although treating and protecting the injured area is of primary concern, it is important to then look beyond the injury, toward keeping the rest of the body as fit as possible. Whether through alternative sports, cross training, or water exercise, the patient can preserve the integrity of the injured joint, keep the noninjured muscles active, and maintain ever-important cardiovascular fitness. The exercise prescription for the injured athlete should emphasize our most vital muscle, the heart.

[Male varicocele. Analysis of the incidence of varicocele in a population of 18,800 young men].

The authors evaluate the incidence of varicocele after having examined 18,800 healthy white men members of the Italian Army Corps born in 1966 and visited in 1985. They found a varicocele in 775 patients (4.12%), in 5 was present bilaterally, and in 770 in the left side. In 45 patients the surgical treatment was already done at the time of the examination and there were 3 recurrences. The patients with varicocele were divided according to the classification of Dubin-Amelar and were: grade I n. 175 (0.93%); grade II n. 515 (2.74%); grade III n. 35 (50.18%). In 566 (73.03%) the diagnosis was done at the moment of the examination, 152 (19.62%) discovered the varicocele at the self examination; in 57 (7.37%) the diagnosis was already done by the family doctor. Infertility is sometime associated to the varicocele and with the unrelieved local discomfort are the main indications for surgery. It should be also emphasized the need of education on the infertility problems during the high school that is a time of the life in which there is the highest incidence of varicocele.

Pulmonary alveolar microlithiasis. A case report with a discussion of differential diagnosis.

The rare disease of pulmonary alveolar microlithiasis is characterized by innumerable microscopic stones, mainly of tribasic calcium phosphate, within the pulmonary alveoli. In a 13-year-old boy an earlier radiological examination showed diffuse lung opacity. The main differential diagnoses taken into consideration were sarcoidosis, idiopathic hemosiderosis, diffuse interstitial fibrosis and alveolar proteinosis. The correct diagnosis was made after a lung biopsy which showed intra-alveolar microliths. The radiological findings are in agreement with the reports in the literature. The specific X-ray features of pulmonary alveolar microlithiasis are calcified opacities, nodules, hilifugal trabeculations and pleural striae. These radiological signs are sufficiently diagnostic to restrict lung biopsy to particularly difficult cases. The disease is resistant to all therapeutic measures.

[Cytological diagnosis of the breast. Aspiration technics].

Over the past twenty years the use of fine needle aspiration techniques in breast cancer has spread worldwide, becoming a routine examination in the early phase of the study of palpable breast nodules. In this paper, the Authors review the state-of-the-art of fine needle aspiration, illustrating the most common methods and the various modifications made by operators. A new type of metal handle is also described which facilitates the collection of cells for cytological tests. Previously reported studies show that operators must be trained and supplied with suitable instruments in order to obtain valid results and to minimise the number of false positives and false negatives. There are very few complications to fine needle aspiration and none are severe, with the exception of pneumothorax which however has a frequency of under 3%. The majority of Authors agree that there should be more collaboration between the clinician and the anatomopathologist in order to increase the reliability of the technique. Fears of spreading cancer along the path of the needle have been dispelled following clinical experience over time. The survival of patients with breast cancer undergoing fine needle aspiration in the diagnostic phase does not differ from that in other patients.

[Correlation between indicators of the fertile period of the menstrual cycle and ultrasonically-determined ovulation].

The authors intend to verify the reliability of some indices set up for the assessment of the fertile period of the menstrual cycle in women. The patterns of the following indices were studied and related to the day of ovulation, as detected by pelvic ultrasonography, in 42 cycles in healty women: the urinary surge of luteinising hormone tested by radioimmunoassay (R.I.A.), urinary surges of estrone-3-glucuronide (E1-3-G) and pregnanediol-3 alpha-glucuronide (Pd-3-alpha-G) tested by chemiluminescence immunoassay, cervical mucus at the vulva and basal body temperature (B.B.T.) recorded by self-observation. The patterns of these parameters have shown a various temporal relationship and reliability for detection of the fertile days of the menstrual cycle.
