Hysterectomy and psychiatric disorder: II. Demographic psychiatric and physical factors in relation to psychiatric outcome.

One hundred and fifty-six patients with menorrhagia of benign origin were interviewed before hysterectomy, and again six months (n = 147) and 18 months (n = 148) after the operation. Psychiatric outcome, measured in terms of PSE cases detected at 18 months, was strongly associated with pre-operative mental state, neuroticism, previous psychiatric history, and family psychiatric history. No association was found with organic pathology in the uterus, bilateral oophorectomy or demographic variables.

Evaluation of a modified self-report measure of social adjustment.

A self-report Social Adjustment Scale (SAS-M) for use in British populations was devised by modifying the original North American version, its usefulness was evaluated among 331 women drawn from two local populations. In a group of mothers of one year old babies (n = 130), high levels of agreement were found between the subjects' self-ratings on the SAS-M and (i) a psychiatrist's ratings of their social adjustment made at interview; (ii) ratings of the subjects' social adjustment made by their husbands on the SAS-M; (iii) measures of concurrent mental state. In a group of women undergoing elective sterilization (n = 201), the SAS-M was found to be sensitive to changes in mental state over time. Possible application of the SAS-M in psychiatric research, particularly when an interview is not feasible, are discussed.

Analgesic effects of branding in treatment of headaches.

The effect of branding--that is, the labelling and marketing--of a well-known proprietary analgesic used to treat headaches was studied in a sample of women given a branded or unbranded form with either an inert or an active formulation. The sample was also divided according to whether the subjects were regular users of the brand or users of other brands. The findings showed that branded tablets were overall significantly more effective than unbranded tablets in relieving headaches. Differential effects were observed: the effects of branding were more noticeable one hour after the tablets were taken compared with 30 minutes; in the women given the placebo; and in the users of the brand compared with the users of other brands. It is hypothesised that these effects are due to increased confidence in obtaining relief with a well-known brand, and that branding has an analgesic effect that interacts with the analgesic effects of placebos and active ingredients.

Working overseas: a bargain from which all sides gain.

The advantages of obtaining experience outside one's own health care system are not always fully appreciated. Some employers and senior officers tend to be discouraging. Paul Cooper, district administrator, East Roding HD, and consultant adviser on health planning to the Crown Agents, follows up Pauline Drummond's recent article on working outside the NHS, with a look at the advantages to the individual and the system of a spell of work overseas.

Prometaphase chromosomes in five patients with the Brachmann-de Lange syndrome.

We analyzed the prometaphase chromosomes of 5 patients (including one pair of sibs) with the Brachmann-de Lange syndrome (BDLS), and did not find a significant chromosome abnormality in any of them. It appears that two distinct entities can be distinguished on clinical and chromosomal bases; the BDLS and the dup(3q) syndrome. We still recommend chromosome studies in any patients with BDLS and BDLS-like manifestations.

Informal advice- and information-seeking between physicians.

Presently little is known or understood regarding the informal advice- and information-seeking behavior which occurs between physicians. In this paper the authors present an assessment of the quantity, quality, and patterns of direct peer communication in a county's physician population. All physicians within a country were mailed a sociometric questionnaire comprised of two questions. The data from the questionnaire revealed that colleague interaction occurs on a regular and frequent basis and is of considerable value to the physician seeking advice and information. An analysis of the communication patterns distinguished six of the country's physicians as opinion leaders. These findings illustrate the strength of informal communication among physicians and their opinion leaders. This phenomenon may be a key element in facilitating the physician's translation of medical advances into improved patient care with the least delay possible.

Follicular cervicitis--colposcopic appearances and association with Chlamydia trachomatis.

Follicular cervicitis was recognised in 15 (44 per cent) of 34 women who were examined colposcopically and who were sexual partners of men with non-gonococcal urethritis. Valid results of culture for Chlamydia trachomatis were obtained in 26 cases: the organism was isolated from the cervix of five of 11 women in whom follicular cervicitis had been diagnosed, but from only one of 15 whose cervices did not have this change. A similar correlation was not found for infection with Mycoplasma hominis or Ureaplasma urealyticum.
