[Clinical and prognostic usefulness of Ki67 determination in breast carcinoma].

We studied the correlation between tumor size, nodal status, menopausal status, nuclear oestrogen receptors, disease free survival and Ki67 (proliferation rate) measured by mean of immunohistochemical analysis with monoclonal antibody. Median value was 10%. From December 1986 to October 1989 we studied 236 consecutive patients (pts). Median follow up was 34 months (range 7-51 months). Median age was 56 years (r. 26-82). We observed: 1) lower levels in smaller tumors; 2) poor correlation with nodes involvement; 3) lower levels when nuclear oestrogen receptors were positive. Preliminary data seem to suggest longer disease free survival when Ki67 is low.

[General movements in neonatology].

General movements (GMs) can be observed in fetus (as young as nine weeks postmenstrual age) and in infants. Without neurological dysfunction, GMs continue in a similar pattern (writhing movements) until the end of the eight week postterm which is then followed by a gradually emerging new pattern (fidgety movements). At an age of about 15 weeks postterm, infants develop gradually other patterns of spontaneous motor activity (manipulation, voluntary movements, reaching, touching, grasping, axial rolling, antigravity movements, etc.). The changes in spontaneous motility and GMs developmental trajectories were good predictors of the neurological outcome. We report the results of our studies in fetal distress and in perinatal abnormalities for the evaluation of predictive value of GMs assessment.

[Conjoined twins: a case report].

Detection during pregnancy of twin kinsmen generates a series of issues. The delivery must be programmed and prepared. The unexpected birth of twin kinsmen represents an emergency. In this woman, the normal antenatal US evaluations were not performed because of the social conditions of the mother. The outcome of the twins is determined by the malformations complexity. In this case the abnormalities caused the neonatal death.

[Levels of tumor necrosis factor and interleukin 1 in uremia and in hemodialysis].

The present study aimed to assess the serum levels of tumor necrosis factor (TNFalpha) and Interleukin-1 (IL-1) in uremic patients and those patients undergoing extracorporeal replacement therapy in relation to the duration of dialysis and the type of membrane. Serum cytokine values were assayed using an immunoradiometric technique (monoclonal antibodies) in 28 uremic patients and 7 healthy controls. TNFalpha levels were normal in control subjects but increased in uremic patients. Patients undergoing hemodialytic treatment showed increases which were statistically significant in comparison to basal levels, with a trend which was directly correlated to the duration of dialysis. No significant variations were induced by the different types of dialysis membranes used. In virtually all patients IL-1 values could not be assayed using this method.

[The renin-angiotensin-aldosterone system in liver cirrhosis].

The role of the RAA system in the genesis of ascites in liver cirrhosis patients is not yet perfectly clear. The present study was conducted on 176 cirrhosis patients in order to investigate RAA system function, to assess the changes taking place in the various stages of the disease and to correlate such changes with the various kidney function parameters. The patients were divided into 3 groups as follows: Group I: patients without ascites on admission and with no history of the condition; Group 2: patients with ascites of recent onset and/or response to diuretic treatment; Group 3: patients with ascites not responsive to diuretic treatment. In Group 1, 19 patients (38%) reveal a significant reduction in renin activity together with portal hypertension and increased hydrosaline retention. In Group 2 renin activity was reduced in 4 patients (6%), aldosterone activity in 3 (4%). Progressive deterioration in liver function parameters and progressive activation of the RAA system combined with reduced sodiuria content were found in over 50% of these patients. The presence or absence of portal hypertension in this group was not related to significant changes in diuresis or sodiuria. In Group 3 renin was activated in 54 patients (89%), aldosterone in 58 (95%) and there was also a distinct reduction in sodiuria (96% of patients) and chloruria (100%). A substantial increase was also noted in the incidence of low blood sodium (53%) while portal hypertension was found in 97% of patients. On the basis of those data it may be hypothesised that high pressure inside the liver creates the stimulus for primary sodium retention. The decrease in effective blood volume after vasodilation, accentuated by low blood albumin and splanchnic venous stagnation may the stimulate the sympathetic nervous system and RAA system. Hyperaldosteronism only becomes the dominant factor in renal imbalance when the cirrhosis reaches the resistant ascites phase.

Cross-linked sodium carboxymethylcellulose as a carrier for dissolution rate improvement of drugs.

The dissolution rate is often the limiting step in gastrointestinal absorption of water insoluble drugs from solid oral dosage forms. The aim of this work was to use a swellable polymer chosen among superdisintegrants, for improving the dissolution rate of a sparingly soluble drug, loaded on its surface. Nifedipine, which has a very low water solubility, was chosen as a model drug, while cross-linked sodium carboxymethylcellulose (Ac-Di-Sol) was chosen as the swellable polymer. The Nifedipine/Ac-Di-Sol systems were prepared using two different techniques: evaporation and spraying; in some preparations polyethylene glycol (PEG 1500), or sucrose palmitate (Sucrodet), or dioctyl sodium sulfosuccinate (Aerosol OT) were added. The results of the dissolution tests showed that the dissolution rate of Nifedipine from the systems prepared increases, particularly in the case of the preparation composed of Ac-Di-Sol plus surfactant agents.

[Volvulus of the sigmoid. Case report].

Reference is made to a series of 7 cases of volvulus of the sigmoid colon in a brief account of the aetiopathogenic factors and anatomopathological aspects most commonly assigned to this form. Its typical clinical picture and radiological diagnosis are described and the controversial matter of the best method of treatment is discussed. Comparison of the literature results suggest that elective resection is to be preferred, or non-invasive decompression followed by devolvulation and simple colopexy of the wall.
