Associations between ERCC2 polymorphisms and gliomas.

Xeroderma pigmentosum complementation group D/excision repair cross-complementing in rodents 2 (ERCC2) encodes a protein that is part of the nucleotide excision repair pathway and the transcription factor IIH transcription complex. Mutations in this gene have been shown to cause three distinct clinical diseases including xeroderma pigmentosum, Cockayne syndrome, and trichothiodystrophy. Several ERCC2 polymorphisms, the effects of which on gene function are not known, have been described. To investigate whether constitutive sequence variations might be associated with adult onset gliomas, blood specimens from a case-control study (187 cases and 169 controls) were genotyped for seven previously described polymorphisms (R156R, I199M, H201Y, D312N, A575A, D711D, and K751Q). A novel R616C polymorphism was also identified. Cases were significantly more likely than controls to be homozygous for the silent AA variant at codon 156 (odds ratio, 2.3; 95% confidence interval, 1.3-4.2). Although this was observed for patients in each of three histological subgroups of cases, (glioblastoma multiforme, astrocytoma, and oligoastrocytoma) compared with controls, the association was strongest for patients with oligoastrocytoma (odds ratio, 3.2; 95% confidence interval, 1.1-9.5). In contrast, cases were somewhat less likely than controls to carry variants at D312N, D711D, and K751Q, but not significantly so overall or for any subgroup after adjustment for age and gender. Individuals with variant nucleotides at D312N, D711D, and K751Q were significantly more likely to carry a variant at another of those three codons and less likely to carry a variant nucleotide at R156R, regardless of case or control status. Although the pattern of association observed here is consistent with a role of ERCC2 variants in the prevention or causation of glioma, these results are also consistent with the possibility that another gene linked to ERCC2 may be involved. This seems especially so because the strongest association was observed with a silent nucleotide variation.

Helper T-lymphocyte precursor frequency predicts the occurrence of graft-versus-host disease and disease relapse after allogeneic bone marrow transplantation from HLA-identical siblings.

BACKGROUND AND OBJECTIVES: Donor helper T-lymphocytes may be involved in graft-versus-host disease (GVHD) and a graft-versus-leukemia effect after bone marrow transplantation (BMT). We assayed donor helper T-lymphocyte precursor frequencies (HTLP(f)) to see whether they could predict the severity of GVHD and disease relapse after transplantation, thereby facilitating donor selection, pre-transplant counselling and modification of GVHD prophylaxis after BMT. DESIGN AND METHODS: Thirty-six consecutive adult BMT recipients and their HLA-identical sibling donors were recruited. HTLP((f)) was measured as a function of interleukin-2 secretion by alloreactive donor T-cells using a limiting dilution assay. Patients were followed prospectively to assess the severity of GVHD and the status of the primary disease after BMT. RESULTS: Eight donors had HTLP((f)) less than or equal to 10(-6); no recipients of these grafts developed severe GVHD after transplantation. Twenty-eight donors had HTLP(f) greater than 10(-6) and 18 recipients of these grafts developed severe GVHD (> or = grade 2) (chi(2) test, p<0.01). Seven donors had HTLP(f) greater than 10(-5) and no recipient had disease relapse. Twenty-nine donors had HTLP(f) less than or equal to 10(-5), 11 recipients of these grafts developed disease relapse (chi(2) test, p=0.08). INTERPRETATION AND CONCLUSIONS: BMT recipients from HLA-identical sibling donors with low (<10(-6)) and high (>10(-5)) HTLP(f) may have a low risk of acute GVHD and disease relapse after transplantation.

Studies on the oxidative stress in alcohol abusers in China.

OBJECTIVE: In order to study the relationship between alcohol abuse and oxidative stress, and to identify oxidative damage of alcohol abuse in human bodies. METHODS: 80 cases of male alcoholics (AL) aged 40 years old and 80 cases of male healthy volunteers (HV) of the same age without drinking history were investigated by measuring concentrations of vitamin C (VC), vitamin E (VE) and beta-carotene (beta-CAR) in plasma as well as activities of superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GSH-Px) in RBC with spectrophotometric assays. RESULTS: Compared with the average values (AV) of the above biochemical parameters in the HV group, the average values of VC, VE and beta-CAR in plasma and the activities of SOD, CAT and GSH-Px in RBC in the AL group were significantly decreased (P = 0.0000). The findings in linear regression and correlation analysis for 80 alcoholics showed that with the prolonged drinking duration and increased daily drinking quantity, the values of VC, VE and beta-CAR in plasma as well as SOD, CAT and GSH-Px in RBC in the alcoholics were gradually decreased (P = 0.000), representing a respectively significant linear negative correlation. The analysis of stepwise regression and correlation revealed that the drinking duration had the closest correlation with the values of VE in plasma as well as CAT and GSH-Px in RBC, while the daily drinking quantity had the closest correlation with the values of VC, VE and beta-CAR in plasma as well as SOD and GSH-Px in RBC. CONCLUSION: The findings of the present study suggested that the oxidative stress in the alcoholics became pathologically intensified, leading to potential oxidative damages in their bodies. Therefore, alcoholics should abstain from alcohol drinking, and should take as supplements suitable dosage of antioxidants per day such as VC, VE, beta-CAR and others to moderate potential oxidative damages in their bodies.

Abnormal metabolism of nitric oxide, oxidative stress and lipoperoxidative stress in patients with acute viral myocarditis.

OBJECTIVE: To investigate metabolic status of nitric oxide (NO) as well as oxidative and lipoperoxidative stress, pathological chain reactions of a series of free radicals, and oxidative and lipoperoxidative damages in patients with acute viral myocarditis (AVM). METHODS: Using a random paired control design, plasma levels of NO, lipoperoxides (LPO), vitamin C (VC), vitamin E (VE) and beta-carotene (beta-CAR), erythrocytic level of LPO as well as activities of erythrocytic superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GSH-Px) were investigated by spectrophotometric assays in 60 AVM patients and 60 healthy adult volunteers (HAV). RESULTS: Compared with the HAV group, plasma NO, and LPO in plasma and in erythrocyte of the AVM group significantly increased (P = 0.0001), while VC, VE, beta-CAR, SOD, CAT and GSH-Px of the AVM group significantly decreased (P = 0.0001). Compared with the AVM group before treatment, plasma NO, and LPO in plasma and in erythrocyte of the AVM group post treatment significantly decreased (P < 0.05), whereas plasma VC, VE and beta-CAR as well as erythrocytic SOD, CAT and GSH-Px of the AVM group post treatment significantly increased (P < 0.05). CONCLUSION: The findings in this study suggested that in the AVM patients the metabolism of NO was disturbed, and the pathological chain reactions of a series of free radicals were severely aggravated, thus produced the oxidative damage and lipoperoxidative damages. Therefore, we recommend that antioxidants at suitable dosage, such as VC, VE, beta-CAR and others, should be given to AVM patients daily to alleviate potential oxidative and lipoperoxidative damages in their bodies.

A naturally occurring point mutation in the beta3 integrin MIDAS-like domain affects differently alphavbeta3 and alphaIIIbbeta3 receptor function.

We have investigated the effect of a new Leu196Pro mutation, identified in the MIDAS-like domain of the beta3 integrin subunit in a patient with type II Glanzmann thrombasthenia, on beta3 integrin receptor function. Expression of the mutant beta3Pro196 subunit in CHO cells, either associated with recombinant human alphaIIb or alphav, resulted in normal biosynthesis of beta3 and heterodimerization with alphav or alphaIIb, but selectively interfered with alphaIIbbeta3 maturation and transport to the cell surface. Functional analysis of the beta3 mutant receptors revealed strong inhibition of alphavbeta3-mediated cell spreading on immobilized fibrinogen, focal contact formation, p125FAK phosphorylation and fibrin clot retraction, as opposed to normal alphaIIbbeta3-mediated cell interaction with immobilized fibrinogen, focal contact translocation and signaling. In contrast, antibody- or DTT-activated mutant aIIbbeta3 was unable to bind soluble fibrinogen or the ligand mimetic PAC-1 monoclonal antibody, but underwent a conformational change following RGD peptide binding as demonstrated by AP5-LIBS epitope expression. These results suggest that (1) the highly conserved TL196T motif in the beta3 integrin subunit is located in a domain structurally important for the exposure of a functional binding site for soluble fibrinogen; and (2) that the MIDAS-like contact site in beta3 is not involved in alphaIIbbeta3-mediated cell adhesion to immobilized fibrinogen, while it is essential for alphavbeta3-mediated interaction with this ligand.

[Alveolar macrophage and airway inflammation in patients with chronic obstructive pulmonary disease].

OBJECTIVE: To investigate the effects of alveolar macrophage(AM) on airway inflammation of chronic obstructive pulmonary disease(COPD). METHODS: Broncho alveolar fluid(BALF) and bronchial mucosa biopsies were performed as usual. The levels of macrophage inflammatory-1 alpha (MIP-1 alpha), gelatinase B (MMP-9), interleukin(IL)-8 in BALF and the culture supernatants of AMs were measured with ELISA. The levels of endothelin (ET) were measured with radioimmunoassay method. The AMs expressing lymphocytic functional associated antigen-1 (LFA-1) in BALF, the AMs in mucosa and the thickness of mucosa were measured with immunocytochemistry and immunohistochemistry technique. RESULTS: The levels of MIP-1 alpha, MMP-9, IL-8 and ET in the culture supernatants of AMs in patients with COPD were (1 446.9 +/- 126.7) microg/L, (16.98 +/- 5.15) microg /L, (43.98 +/- 5.54) ng/L, (168.10 +/- 21.22) microg/L respectively; while those were (1 123.5 +/- 90.6) microg/L, (5.24 +/- 0.93) microg/L, (25.99 +/- 6.05) ng/L, (124.68 +/- 9.96) microg/L in a control group. The number of macrophages in mucosa in patients with COPD was (1.71 +/- 0.48) cells/Hp, while that was (0.59 +/- 0.18) cells/Hp in the control group (0.63 +/- 0.25) x 10(4). Statistical significance was found between the two groups. The number of AMs and polymorphonuclear leucocyte in BALF were inversely correlated with forced expiratory volume in one second as percentage of predicted value(FEV(1)% Pre) (r = -0.511, P < 0.01; r = -0.562, P < 0.01). The levels of MMP-9 and ET in the culture supernatants of AMs were inversely correlated with FEV(1)% Pre(r = -0.678, P < 0.01; r = -0.871, P < 0.01). Both the level of MIP-1 alpha and the positive number of AM expressing LFA-1 in BALF were positively correlated with the number of AMs in BALF(r = 0.572, P < 0.01; r = 0.625, P < 0.01). CONCLUSION: AM may be involved in the process of airway inflammation and subsequent airway obstruction.

[Renal protective effects of vitamin E in diabetic rats].

OBJECTIVE: To investigate the renal protective effect of vitamin E on streptozotocin induced diabetic rats, as well as the possible mechanism involved in. METHODS: Vitamin E 20 mg x kg(-1) x d(-1) was given to diabetic rats for 8 weeks. Urinary albumin excretion (UAE) was measured by radioimmunoassay. The changes of creatinine clearance rate (Ccr), nitric oxide(NO), nitric oxide synthase (NOS) and endothelin(ET) in the plasma and renal cortical tissue, membrane protein kinase C(PKC) in the renal glomeruli were observed. RESULTS: At two weeks, diabetic rats had supernormal Ccr[(6.47 +/- 1.51) ml x min(-1) x kg(-1)], UAE[(15.60 +/- 1.64) microg/24h], NO[(37.30 +/- 3.77) micromol/L], NOS[(34.89 +/- 3.83) U/L] and PKC[(86.85 +/- 11.37) pmol x min(-1) x mg protein(-1)] of diabetic rats were increase but ET was decreased. At 8 weeks, PKC[(84.18 +/- 12.14 ) pmol x min(-1) x mg protein(-1)] and ET of diabetic rats was increased significantly, but NO[(22.75 +/- 2.89) micromol/L] and NOS[(21.34 +/- 1.92) U/L] of diabetic rats was lowered. After treated with vitamin E, Ccr[(4.46 +/- 0.49) ml x min(-1) x kg(-1)], UAE[(16.31 +/- 1.12) microg/24h], PKC[(65.19 +/- 8.83) pmol x min(-1) x mg protein(-1)] at 8 weeks were decreased significantly. Both NO[(33.13 +/- 3.77) micromol/L] and NOS[(30.16 +/- 2.89) U/L] in plasma and kidney tissue were lowered as compared with untreated diabetic group in two weeks. Vitamin E also delayed the NO and NOS lowering in 8 weeks. CONCLUSION: Vitamin E could ameliorate early diabetic renal hemodynamic abnormality via inhibiting PKC, in which inhibition of NO production might be involved. It is important to reduced the PKC activity so as to prevent or delay the development of diabetic nephropathy.

[Airway alveolar macrophages and LFA-1 expression in chronic bronchitis].

OBJECTIVE: To assess the relationship between the expressions of lymphocyte function associated antigen-1 (LFA-1) in macrophages in both BALF and bronchial mucosa and the severity of airway inflammation in patients of chronic bronchitis. METHODS: Bronchial alveolar lavage and bronchial mucosa biopsy were performed in 18 patients with stable stage of chronic bronchitis (CB). LFA-1 positive alveolar macrophage (AM) was measured by immunocytochemistry stain. The percentages and number of AM in BALF and bronchial mucosa were also calculated and the thickness of mucosa measured. RESULTS: (1) The numbers of AM and neutrophils of BALF in CB were significantly higher than those in healthy group (P < 0.01, respectively). So did the percentage of neutrophils of BALF (all P < 0.05). (2) The number [(4.36 +/- 2.11) x 10(4)/ml] and percentage [(24.6 +/- 5.8)%] of positive AM expressing LFA-1 in CB were significantly higher than those [(0.63 +/- 0.25) x 10(4)/ml, (10.6 +/- 2.7)%] in healthy group (all P < 0.01, respectively). The number of AM in BALF was positively correlated with that of positive AM expressing LFA-1 (r = 0.624 7, P < 0.01). The number of CD(68)(+) cells [(21.6 +/- 4.6)/HP] in mucosa and the thickness of mucosa [(0.20 +/- 0.09) micrometer] in CB were significantly greater than those [(10.0 +/- 3.4)/HP, (0.08 +/- 0.04) micrometer] in healthy group (P < 0.01, respectively). They were inversely corrlated with the value of FEV(1)%pre (r = -0.5875, P < 0.01; r = -0.655, P < 0.05). CONCLUSIONS: Increased expression of LFA-1 may be one of important causes leading to the accumulation and migration of AM in the airway of patient with CB. AM infiltration and the thickening of bronchial mucosa are closely related to the decline of FEV(1).

[The change of concentration of endothelin derived from alveolar macrophages and in induced sputum in patients with chronic bronchitis].

OBJECTIVE: To evaluate the changes of levels of endothelin (ET) in induced sputum, the culture supernatants of alveolar macrophages (AMs) and the culture supernatants of alveolar macrophages (AMs) when cultivated with aminophylline and lipopolysaccharide (LPS) in patients with chronic bronchitis and COPD. Then to investigate the role of ET derived from AMs in chronic bronchitis and COPD. METHODS: Fourteen patients with chronic bronchitis and thirteen patients with COPD were studied. Fourteen healthy were enrolled as control. Bronchoscopy and bronchial alveolar lavage were performed routinely, BAL cells were count with hemacytometer and differentater with wright stain. Sputum induction with 4% saline was performed on twenty-six subjects. The levels of ET in BALF and in the culture supernatants of AMs were measured by radioimmunoassay methods. RESULTS: (1) The number of all cells, neutrophils and AMs in BALF of patients with chronic bronchitis and COPD were significantly higher than those of healthy control group (P < 0.01 respectively). (2) The levels of ET in the culture supernatants of AMs and induced sputum were higher in heathy than those in patients with chronic bronchitis and COPD (P < 0.01 respectively). But there were no differences between patients with chronic bronchitis and COPD (P > 0.05 respectively). (3) Positive correlation was found between the levels of ET in the culture supernatants of AMs and those in induced sputum (r = 0.741, P < 0.01), between the number of AMs and the levels of ET in induced sputum (r = 0.597, P < 0.01). (4) Negative correlation was found between FEV(1)% of predicted value and the levels of ET in the culture supernatants of AMs in patients with COPD (r = -0.828, P < 0.01), between FEV(1)% of predited value and the levels of ET in induced sputum in patients with COPD (r = -0.748, P < 0.05). (5) In the culture with aminophylline, the level of ET derived from AMs remained stable (P > 0.05). But in the culture with LPS, the level of ET was significantly higher than that in the culture with DMEM (P < 0.01). CONCLUSIONS: (1) The characteristics of non-specific airway inflammation in patients with chronic bronchitis and COPD is the increased numbers of neutrophils and AMs. (2) AMs are perhaps important sources of ET in the lung. LPS can stimulate AMs to secrete more ET. ET derived from AMs takes part in development of COPD.

[Airway inflammation and macrophage inflammatory protein-1alpha, gelatinase B level in patients with COPD].

OBJECTIVE: To investigate the levels of macrophage inflammatory protein-1alpha (MIP-1alpha) and gelatinase-B (MMP-9) in bronchial alveolar lavage fluids (BALF) and supernatants of the cultured alveolar macrophages (AM) in patients with chronic bronchitis (CB) and chronic obstructive pulmonary disease (COPD). METHODS: Bronchial alveolar lavage (BAL) was performed bronchoscopically in CB group, COPD group, and control group. Total cells were counted using hemacytometer. Differential cell counts were made with Wright's stained cell smear. The levels of MIP-1alpha, MMP-9 in BALF and in supernatants of cultured AMs were measured by ELISA. RESULTS: The numbers of AMs and neutrophils in BALF in patients with CB and COPD groups were significantly higher than those in control group (P < 0.05). The levels of MIP-1alpha and MMP-9 in BALF and the supernatants of cultured AMs in patients with CB and COPD groups were higher than those in control group (P < 0.05). The levels of MIP-1alpha and MMP-9 in BALF were positively correlated with those in the supernatants of cultured AMs (r = 0.253, P < 0.05; r = 0.529, P < 0.01). The number of AMs in BALF was positively correlated with the level of MIP-1alpha and MMP-9 in BALF (r = 0.558, P < 0.01; r = 0.405, P < 0.01). Both AMs counts and the levels of MIP-1alpha, MMP-9 in BALF were inversely correlated with FEV(1.0)%pred (r = -0.322, P < 0.05; r = -0.319, P < 0.05; r = -0.616, P < 0.01). The levels of MIP-1alpha and MMP-9 in supernatants of the culture AMs were increased significantly after LPS stimulation (P < 0.05). CONCLUSION: AMs, which may be the most important cellular source of MIP-1alpha and MMP-9 in COPD, accelerate MIP-1alpha and MMP-9 accumulation in the lung, which exaggerates inflammation process in the airway.

[Study on expression of caspase-3 and bcl-2 proteins and their relations with cell apoptosis and proliferation in non-Hodgkin's lymphoma].

OBJECTIVE: To explore the possible roles of caspase-3 and bcl-2 proteins in the development and progression of non-Hodgkin's lymphoma (NHL) and their relationship. METHODS: TdT-mediated dUTP nick end labeling (TUNEL) and immunohistochemistry were used to study cell apoptosis and expression of proliferating cell nuclear antigen (PCNA), caspase-3 and bcl-2 in 119 NHL cases. RESULTS: The caspase-3 and bcl-2 positive rates were 86.6% (103/119) and 53.8% (64/119) respectively. Contrasting expression of these two proteins were found in reactive and malignant lymphoid tissues: In reactive lymphoid follicles, caspases-3 exhibited strong immunoactivity in cells located in germinal centers but not in mantle zone lymphocytes, whereas bcl-2 exhibited strong immunoactivity in mantle zone lymphocytes but not in cells located in germinal centers, and in neoplastic follicles, caspase-3 was negatively or weakly expressed, whereas bcl-2 was often positively expressed. In B cell lymphomas, the high grade group had a higher caspase-3 immunointensity and a lower bcl-2 immunointensity than the low grade group (P < 0.01). Apoptotic index correlated positively with expression of caspase-3 (r = 0.512, P < 0.01) and inversely with expression of bcl-2 (r = -0.436, P < 0.01). In addition, apoptotic index and proliferative index were positively related (r = 0.710, P < 0.01). CONCLUSIONS: caspase-3 may participate in the regulation mechanism of lymphoma cell apoptosis. The contrasting expression of caspase-3 and bcl-2 is often seen in reactive and malignant lymphoid tissues, and it may indicate a close relationship between these two proteins in the regulation of lymphocyte proliferation kinetics.

A cost-effectiveness analysis of biliary anastomosis with or without T-tube after orthotopic liver transplantation.

Biliary reconstruction continues to be a major source of morbidity following orthotopic liver transplantation. We wished to determine if choledochocholedochostomy without a T-tube was associated with fewer biliary complications and was less costly than choledochocholedochostomy with a T-tube. A retrospective cohort study of patients who underwent liver transplantation was performed. Patients were stratified into two groups: group I had bile duct reconstruction with T-tube and group II did not have a T-tube. The results were interpreted on an intention-to-treat analysis. We identified 147 adult patients who underwent initial liver transplantation. There were 76 patients in group I and 71 patients in group II. There were no statistical differences between the two groups regarding underlying cause of liver disease, patient age, gender or United Network for Organ Sharing status. As the decision to use a T-tube was made at the time of surgery, the two groups may not be strictly comparable. The mean hospital stay was longer in group I (31.1 +/- 27.9d) than in group II (18.8 +/- 15.5d) (p = 0.001). Biliary complications were statistically more frequent in patients from group I patients (25/76, 32.9%) than in patients from group II (11/71, 15.5%) (p = 0.01). There was a trend for the costs associated with diagnostic and therapeutic procedures for the management of biliary complications to be greater for group I than for group II, although this was not statistically significant (p = 0.235). Our study suggests choledochocholedochostomy without T-tube reconstruction is the preferred strategy for biliary reconstruction in orthotopic liver transplantation. It is not only associated with fewer biliary complications, but also less costly than using choledochocholedochostomy over a T-tube. Randomized prospective studies are needed to confirm our results.

[A multicenter randomized controlled clinical trial on lipids regulating effects of domestic simvastatin].

OBJECTIVE: This clinical trial was designed to assess the lipids regulating effects of domestic simvastatin (DS, produced by Chengdu Huayu Pharmaceutical Co.) in patients with hyperlipidemia. METHODS: 160 hyperlipidemic patients were randomly divided into 3 groups (A, B and C). Groups A and B were subjected to single-blind trial; group C was for open trial. Group A took DS 10 mg q.n., group B Zocor 10 mg q.n. and group C DS 10 mg q.n. respectively for 8 weeks. All the patients were followed up at the 4th week and 8th week. 155 patients finished the trial with 59 cases in group A, 47 cases in group B and 39 cases in group C. RESULTS: At the 4th week, serum total cholesterol (TC) in group A, B and C decreased by 16.88%, 19.23% and 14.10%; serum triglycerides (TG) decreased by 19.27%, 15.66% and 17.96%; HDL-C increased by 7.69%, 7.46% and 6.69%; and LDL-C decreased by 23.02%, 27.84% and 24.43%, respectively; there was no significant difference among the three groups (P > 0.05). At the 8th week, serum TC in groups A, B and C decreased by 25.03%, 26.53% and 25.22%. TG decreased by 23.85%, 24.74% and 24.75%; HDL-C increased by 9.23%, 8.95% and 8.39%; and LDL-C decreased by 33.72%, 35.50% and 30.99%, respectively; still, no significant difference among the three groups was observed (P > 0.05). The incidence rates of side effects in the three groups were similar. The clinical effects were more significant at the 8th week than at the 4th week for Zocor and DS. CONCLUSION: These data suggest that DS is as effective and safe as Zocor in clinical use for lipids regulating serum.

Isobutyramide activates transcription of human fetal gamma- and murine embryonic epsilon(y)-globin genes.

OBJECTIVE: To examine the effect of isobutyramide synthesized in our laboratory on human and murine globin gene expression and to test cell toxicity of the drug. METHODS: MEL cells were transfected with the recombinant construct muLCRAgammapsibetadeltabeta and the stable transformants were cultured in the medium with different concentrations of isobutyramide. The experimental mice and rabbit were injected with different doses of isobutyramide. The globin mRNAs were analyzed by RNase protection assay. The hematological toxicity and electrolyte toxicity of the drug were tested. RESULTS: An inducible and dose-dependent expression of the human gamma-, beta- and mouse alpha-globin gene was observed in the transfected MEL cells. The induction of the human gamma-globin gene is significant stronger than that of the beta-globin gene. With 2.5 approximately 5 mmol/L isobutyramide, the induction of the human gamma-globin gene is even more effective than that of mouse alpha-globin gene. After a 15-day injection under the doses of 500 approximately 900 mg x kg(-1) x d(-1), the level of the mouse embryonic epsilon(y)-globin mRNA could be significantly induced up to 3 approximately 4 fold of that of uninjected controls. The changes of hemoglobin(Hb), RBC, hematocrit(HCT), WBC, derived from mice injected with different doses of isobutyramide at the interval of 24 hours for 2 approximately 4 weeks, were generally within the normal range. In rabbits injected with isobutyramide in the same regiment for 2 weeks, the concentration of blood K+, Na+, Cl- and CO2 were all within normal range and serum ionic osmotic pressure remained stable as well. CONCLUSION: Our results suggested that isobutyramide is a weak inducer of cell differentiation, but it can selectively activate transcription of human gamma-globin gene at a certain degree, and it can act on early stages of erythroid progenitor differentiation in adult mice and activate transcription of embryonic epsilon(y)-globin gene and have no hematological toxicity. Our results have further proved the potential value of isobutyramide in treatment of beta-thalassemia and sickle cell disease.
