Solitary brain metastases from thyroid carcinoma: study of 6 cases.

AIMS AND BACKGROUND: Although systemic metastases from thyroid cancer are relatively frequent, they rarely affect the nervous system and only exceptionally originate from well-differentiated carcinoma. METHODS: The authors describe 6 cases of solitary brain metastases from thyroid carcinoma, well-differentiated in 3 cases and anaplastic in the other 3 cases. RESULTS: Four patients were females and 2 males (M:F ratio, 1:2); average age was 51 yrs (range 48-56). On average, the interval between diagnosis of the thyroid tumor and appearance of the metastasis was 2.8 years (range 1-12 years) and was shorter in the anaplastic forms (1.2 versus 4.4 years). Average survival was 15 months (range 6-48 months); in particular, average survival was longer in patients with well-differentiated carcinoma (9 vs 21 months) as also in those who did not present other metastatic sites (6 vs 24 months). Death was due to systemic progression of the disease in 4 cases and to brain-relapse in the other 2 cases. CONCLUSIONS: Surgery, radioactive iodine (where uptake is demonstrable), and radiotherapy are the main therapies available for metastases from thyroid carcinoma. However, survival of patients appears to be modified by the type of treatment performed.

[Encephaloceles. with special reference to the diagnostic difficulties].

We have studied 15 occipital, 3 frontal and one parietal cases of encephaloceles from the point of view of the etiopathogenetic, clinic and especially diagnostic problem. The diagnosis of M.E. is based on: 1) the x-ray demonstration of the bone-gap by particular projections and by tomography; 2) in the frontal M.E. the presence of face deformity, with hipertelorism and widening of nose bridge and the finding of ectopic tissue in the nose or in the oral cavity in presence of palatoschisis. Cerebral angiography and pneumoencephalography, made by tomography, are useful to show the herniated brain, hydrocephalus and other associated anomalies. Finally we think computerized tomography (C.T.) is very important for the diagnosis, since it shows the herniation, the bone-gap and the kind of herniated tissue.

Medulloblastoma in adulthood.

Reporting 32 cases of medulloblastoma in patients aged 17 to 65 years and reviewing the major case-series in the literature, we analyze the clinicobiological and diagnostic features and treatment of this tumor in adulthood. We consider some factors that seem to have a favorable bearing on the prognosis: female sex, length of clinical history, tumor size, hemispheric site, and total removal followed by radiotherapy. We then discuss the differences in tumor behavior between adulthood and childhood that emerged from the comparative analysis.

A review of papillomas of the third ventricle. One case report.

The Authors present one case of choroid plexus papilloma localized in the third ventricle. This is in fact the only case in a series of about 4,000 patients operated on for brain tumors at the Neurosurgical Institute of the University of Rome. They consider 56 cases reported in the literature. Out of these, 43 are described and summarized in a table according to the clinical, radiological and surgical features. Twentyfive were the patients operated on, the average mortality in the cases which underwent indirect or not specified operation was 80%; that of patients submitted to a direct approach or to subtotal excision was 50%; and finally 3 patients out 13 (about 23%) who had total resection, died after surgery.

Intraspinal hemangiopericytomas. Report of two cases and review of the literature.

A description of two cases of intraspinal hemangiopericytoma is given, which join the 30 reported to date. The clinical features of this tumor rarely found in the CNS are compared with those of the same oncotype found at other sites. Surgical treatment is necessary, accompanied by radiotherapy. Spinal tumor embolization preoperatively is of benefit.

[Selective posterior radiculotomy in the treatment of spasticity: immediate and long-term results].

Immediate and late results of a series of 44 patients suffering from spastic syndromes, operated on from January 1973 to December 1976 by selective posterior rootlet section, have been analyzed and reported. The operation consisted of a selective section of the dorsomedial part of the rootlets constituting each posterior root and was performed usually from L1 to S1 bilaterally. Thirty patients improved markedly after the operation and there were no immediate or late complications. The procedure caused no sensory disorders. The indications and contraindications to the operation are discussed.

Hematologic and cytogenetic analyses of 99 cases of de novo myelodysplastic syndromes.

BACKGROUND. The myelodysplastic syndromes (MDS) are a heterogeneous group of clonal hematopoietic stem cell disorders characterized by peripheral pancytopenia, abnormal bone marrow differentiation and a high risk of leukemic transformation. The role of karyotypic abnormalities in the pathogenesis of MDS is still unclear. In this paper we present the hematologic and cytogenetic findings in 99 patients with de novo MDS. RESULTS AND CONCLUSIONS. Fifty-three cases had chromosomal abnormalities nor complex karyotypes were associated with the morphologic subsets described by the FAB criteria. Nevertheless, the karyotypic profile is prognostically important, as indicated by the fact that patients with complex karyotype abnormalities have very short survival.

Gliomas of the conus medullaris.

AIMS AND BACKGROUND: Gliomas of the conus medullaris often show characteristic clinical, radiological, and intraoperative features which differ from gliomas involving other parts of the spinal cord. METHODS: Eight patients with histologically verified gliomas of the conus medullaris were diagnosed and studied. RESULTS: There were five men and three women ranging in age from 21 to 59 years. Predominant initial symptoms were back pain (4 cases) and leg weakness (4 cases). The most common findings on admission were flaccid paraparesis with impaired sensation and bladder dysfunction. Postoperative magnetic resonance (MR) images with more than 95% removal of a tumour were defined as "subtotal removal" (noted in 4 of 8 cases), and less than 95% as "partial removal" (4 of 8 cases). All patients had postoperative radiotherapy. During the follow-up period ranging from 3 to 10 years, there was no tumour recurrence or regrowth on MR images. CONCLUSIONS: The postoperative radiotherapy in gliomas of the conus medullaris where total resection is not possible seems provide a beneficial effect on preventing tumour regrowth.
