Acute myelofibrosis: a report of three cases.

This report describes three cases of an atypical variant of agnogenic myeloid metaplasia characterized by pancytopenia, high peripheral blast cell count, lack of significant splenomegaly or tear-drop poikilocytosis, and diffuse marrow fibrosis with an abnormal proliferation of megakaryocytes. It is suggested that these cases represent an acute variant of agnogenic myeloid metaplasia that is distinct from either the terminal blastic transformation of myelofibrosis or acute granulocytic leukemia.

Increased serum folate-binding capacity: a familial trait.

This report describes increased folate-binding capacity (FABC) in the serum of 7 of 10 family members. Genetically controlled elevation of FABC in normal subjects needs to be distinguished from the acquired increase in bindings capacity which has been described in a number of disorders.

Comparison of the Wintrobe erythrocyte sedimentation rate with the zeta sedimentation ratio.

Erythrocyte sedimentation rate (ESR) determinations performed by the Wintrobe method were correlated with a newer, rapid method, the zeta sedimentation ratio (ZSR), in 189 adult patients. Good correlation between the two methods was achieved over a wide range of values, with a correlation coefficient of 0.88 in female and 0.92 in male subjects. The precision of each method was evaluated by performing ten replicate determinations for 11 patients. The average variance for the ZSR was 2.91%, compared with 7.04% for the Wintrobe ESR; a difference significant at p < 0.01. Good correlation with standard methods, speed of performance and superior precision make the ZSR an attractive alternate procedure for the determination of the sedimentation rate.

Treatment of acute leukemia during pregnancy.

Cytarabine and thioguanine therapy for acute myelocytic leukemia, initiated during the 26th week of pregnancy, led to complete remission of the leukemia in a 22-year-old woman, and allowed for delivery of a normal infant at term. No chromosomal abnormalities were detected in the infant. Cytarabine and thioguanine in combination are effective agents in the treatment of acute leukemia in adults. Their use appears warranted in pregnant patients after the first trimester.

Acute granulocytic leukemia in Hodgkin's disease.

Two cases of acute myeloblastic leukemia were observed in Hodgkin's disease patients two and five years following intensive radiation therapy. Sixteen additional cases of acute granulocytic leukemia occurring in radiation-treated Hodgkin's disease patients have been reported in the literature and are reviewed. It is suggested that the development of acute leukemia was related to irradiation in these patients, and that additional such cases could be expected with the use of intensive radiation and chemotherapy, a risk probably justified in view of the improved control of Hodgkin's disease achieved by these programs.

Aspirin therapy in microangiopathic hemolytic anemia and renal failure.

An 18-year-old man presented initially with findings compatible with disseminated intravascular coagulation. Following a brief partial response to intravenous heparin therapy rapid deterioration of renal function was associated with evidence of a hypercoagulable state characterized primarily by excessive platelet aggregation and sequestration. Initiation of aspirin therapy led to a prompt rise in the platelet count and permanent improvement in renal function.
