A prospective study has been conducted to determine the clinical importance of lung scintigraphy in the work-up of patients thought to have bronchial carcinoma (i.e., accuracy of the scintigraphic diagnosis, data concerning spread of the primary tumor [=T] as well as the possibility of metastases to the lung hilus and mediastinum [=N]). Ninety-four patients, all of whom were hospitalized for suspected bronchial carcinoma, were examined by 133Xenon i.v., 133Xenon gas inhalation,, and 99mTc-MAA i.v. (macroaggregated albumin or microspheres of human albumin). The suspicion was confirmed in 77 patients. Central and peripheral bronchial carcinomas were kept separate in the evaluation. A normal lung scintigram practically rules out a central bronchial carcinoma. Where the scintigraphic finding prompts suspicion of central bronchial carcinoma, this suspicion must be confirmed by other examinations and preferably those which cytologically and histologically directly demonstrate the tumor. The diagnostic reliability of lung scintigraphy is drastically reduced in patients with bilateral ventilation disturbances. Because of its 27% rate of false negative findings, bronchoscopy is a less suitable means of ruling out bronchial carcinoma than scintigraphy. The peripheral bronchial carcinoma displays no characteristic scintigraphic signs; thus, a normal scintigram does not rule out peripheral bronchial carcinoma. Bronchoscopy was negative in all these patients. Spread of the primary tumor was exactly predicted with bronchoscopy in 11% and with scintigraphy in 42% of cases. When the scintigraphic criteria for hilar or mediastinal involvement are ful filled, it is practically certain that the patient is no longer radically operable. This assertion is valid when mediastinoscopy is normal. Lung scintigraphy possesses the same accuracy in the diagnosis of mediastinal involvement both with peripheral lung tumors and with centrally located tumors.
The goal of this prospective study is to determine the reliability of the scintigraphic diagnosis of mediastinal involvement in patients with bronchial carcinoma, as well as to establish the value of each individual scintigraphic criterion. Ninety-four patients, all of whom were hospitalized because of suspicion of bronchial carcinoma were examined using 133-Xenon i.v., 133-Xenon gas inhalation, and 99m-Tc-MAA i.v. (macroaggregated albumin or microspheres of human albumin). The suspicion was confirmed in 77 patients. -The most reliable scintigraphic criteria are: 1. a localized delayed appearance of the activity in the afflicted lung area during the first 20 sec of the 133-Xenon i.v. series, and 2. the presence of a "dead-space" ventilation. Activity remaining in the area of the subclavian vein or the superior vena cava up to 10 min after the injection of 133-Xenon i.v. must be very carefully interpreted. Sixty-five patients underwent both mediastinoscopy and scintigraphy: in 44 patients (= 68%) the findings in both examinations were the same. Compared to the final diagnosis (mediastinoscopy, operation) there were 20 (= 30%) false negative but only one (= 2%) false positive scintigraphic diagnoses made. -When the scintigraphic criteria of hilar or mediastinal involvement are fulfilled, then the patient is practically certain to be no longer radically operable. This assertion is valid even when the mediastinoscopy is normal.

Although acute renal artery occlusion is luckily a rare event, many cases are probably overlooked. They are caused either by emboli, accidents or iatrogenic vascular injury. The conventional wisdom holds that the kidney is irreversibly damaged by arterial obstruction lasting several hours, but clinical experience shows that there are exceptions to this rule. How this is to be explained from the pathophysiologic viewpoint is discussed in this article. Recognition of acute renal artery occlusion is easiest and fastest in iatrogenic cases. In trauma due to accidents, useful pointers are hematuria, site and intensity of the injuring force and concomitant lesions. Emboli do not give rise to specific symptoms and are most to be suspected in cases prone to embolism. It is helpful to subdivide renal artery occlusions into two groups, one with and the other without mural lesionsof the artery. The outlook is a priori considerably better in cases with intact vascular wall. Here, i.e. in the presence of fresh or recent thrombo-embolic occlusion, surgery is most likely to prove successful, but conservative management alone is known to afford good results too. A differentiated approach to treatment is outlined. Severe vascular trauma may confront the surgeon with virtually insoluble problems, but the conditions under which surgical intervention is nonetheless recommended are defined. Generally the presence of a normal conttalateral kidney will lessen the need for surgical action. Surgery is however mandatory in the none too rare case of bilateral renal artery occlusion or when a solitary kidney is affected. In these cases in-situ perfusion combined with organ cooling, explantation, reconstructive surgery extra vivum and heterotopic re-implantation (for instance into the pelvis) is a most promising procedure.
A system has been developed for a specific biopharmaceutical purpose: testing for ophthalmic preparations, the influence of formulation on drug transport through the cornea. The apparatus is a lucite cell, divided in two compartments by a clamped rabbit cornea. Physiological conditions are ensured by a supply of oxygenated perfusion medium. They are monitored by electrical conductivity and corneal thickness measurements. Reliability of the system was tested in a set of experiments.
