Immunolocalization of the costameres in human skeletal muscle fibers: confocal scanning laser microscope investigations.

BACKGROUND: The costameres in skeletal muscle fibers were first described by Pardo et al. (1983a) and have been defined as transverse circumferential elements of the cytoskeleton associated to the sarcolemma. Specific immunostaining for vinculin shows that the costameres overlie I bands. However, an exact correlation between the costameres and the Z line is uncertain, although approximately 10 proteins so far have been localized in the costameres. To define the exact localization of costameres in human skeletal muscle fibers, we carried out an immunofluorescence study using confocal scanning laser microscopy on the fascia lata muscle of adult males. METHODS: Samples were fixed in 3% paraformaldehyde; frozen sections were treated with antivinculin, antitalin, antidesmin, and anti-alpha-actinin, then immunostained with TRITC. For double localization, the TRITC-streptavidin, as a marker for vinculin and FITC-streptavidin a marker for desmin, were used. RESULTS: The distance between two subsequent transverse lines of actininf indicated that muscle fibers were well stretched. Processing, with different software functions of the images obtained using CLSM, shows that vinculin and talin are only present in the sarcolemmal lattice. Immunostaining for vinculin and double immunostaining for vinculin and desmin demonstrate that costameres superimpose underlying I bands without interruption at the Z line. Immunostaining for talin showed that the protein is located in correspondence with the I band and M line. CONCLUSIONS: We believe that costameres are "proteic machinery." The findings of the present study suggest that it is possible to determine the width and the period of each proteic component. In addition, we indicate that costameres are present in correspondence with M line.

Sleep spindles in the initial stages of the vegetative state.

The aim of this study was to look for the presence of spindling in the different stages of remission of the vegetative state to underline all possible correlations with lesional sites, severity of coma and final outcome. The nocturnal polygraphic recordings from 30 patients were examined: 20 (15M, 5F, mean age 31.7 years, range 16-41) had originally suffered a traumatic brain injury, 10 (4M, 6F, mean age 40.5 years, range 24-48) had hypoxic brain lesions. Evidence of spindling, always reduced in density and duration, was found in 44% of these patients, prevalently in the traumatic patients (53.3% versus 30% of hypoxic patients). No subjects in the full stage 0, 0-1 of the vegetative state (apallic syndrome) presented spindling. No significant correlation was found between spindling and the following parameters: gender, the time between the onset of coma and the polygraphic recording, or the site of the lesion.

Post-traumatic mutism.

We report on the infrequent verbalisation disorder called "post-traumatic mutism" observed in 12 subjects with severe traumatic head injuries. These patients during the recovery of their conditions showed for a certain period a speechlessness even though some of them could communicate through gestures and writing. The nature and the possible mechanism of the disorder are discussed.

Imaging of inner cerebral trauma.

The concept of "Inner cerebral trauma" (ICT) has been preliminary defined as a characteristic topographic pattern of deep brain lesions produced by physical forces occurring within the cranial cavity in closed head injury of the acceleration/deceleration type. The lesions, based on neuropathological examinations, are characteristically localized in the "centro-axial" regions of the brain. The extent of ICT is often underestimated by CT. Due to assess the value of MR imaging, 83 patients with ICT were examined on a 1.5 T unit in different stages after trauma. The pattern of lesions, as shown with MR imaging, correlated well with neuropathological studies, suggesting a multifocal pathogenesis of severe traumatic brain injury.

The presence of anti-Borrelia burgdorferi antibodies in a group of multiple sclerosis patients in eastern Sicily. Preliminary data.

The authors evaluate the presence of anti-Borrelia burgdorferi antibodies in a group of polysclerotic patients of Eastern Sicily, in order to verify or dismiss a correlation between Borrelia infection and demyelinizing syndrome. 460 samples of serum were examined, of which 60 belonged to patients affected by definite multiple sclerosis; the other 400 were taken from a group of apparently healthy subjects. From the analysis of the data obtained it can be seen that of the 60 polysclerotic subjects examined, 12 equal to 20% were found to be positive for the presence of IgG anti-Borrelia burgdorferi, while in the control group 30 subjects were found to be positive, equal 7.5%.

Early removal of periarticular ossifications in patients with head injury.

Heterotopic ossifications are a well known but serious complication in patients suffering severe head injury. Their prevalence varies between 10% and 20%. The current concept of surgery recommends removal after "maturation" of the new developed bone because of a suspected higher rate of recurrence. The observations of early operations in 6 patients and 9 joints led us to the conclusion that the disadvantages of long lasting joint stiffness and the according complications outweigh the danger of recurrence. In our patients recurrence was observed in one subject and even there recurrence was most probably the result of wound infection and concomitant osteomyelitis.

Indication, efficiency and complications of intrathecal pump supported baclofen treatment in spinal spasticity.

In 19 patients, who suffered from severe spinal spasticity of different etiologies and did not respond sufficiently to oral antispastic therapy, intrathecal Baclofen test boli were administered. In 11 patients a DAD (Drug Administration Device) [SynchroMedR Model 8611 H, Medtronic Inc. Minneapolis, USA] was implanted. Catheter dislocation or torsion was the most common complication to be observed in these 11 patients. Long term intrathecal Baclofen application was effective in all patients, as reducing spasticity, flexor spasms and spasm induced pain. In some cases the motor performance ameliorated.

Long-term intrathecal baclofen treatment in supraspinal spasticity.

Baclofen, a derivate of gamma-amino butyric acid (GABA), is known to be a useful drug in spasticity treatment. To achieve a good therapeutic response higher oral dosages have to be administered related with central side effects. Intrathecal application of Baclofen in microgram range dosages is proved to be effective in spinal spasticity. The efficiency of intrathecal Baclofen in patients suffering from supraspinal spasticity is discussed controversially. We report on 9 patients with long-term intrathecal Baclofen treatment, all of them responding well presenting a marked reduced muscle tone. In most cases an improvement of motor performance and in two cases improved bladder function was observed. The therapeutical dosages administered to patients with supraspinal spasticity exceed those administered to patients with spinal spasticity by approximately 100% without provoking central side effects. Despite the risks connected with this method it has to be considered as treatment of choice in cases of severe supraspinal spasticity.

Amoebae free-living. Report of the first Italian case of myelitis.

The AA. refer on the first Italian case of amoebae "Limax" primitive myelitis in a 25-year-old man. Microscopical observation showed amoebae free-living, but their cultivation and identification were not possible. The patient recovered because of the propriety of diagnosis and therapy with amphotericin B.

[The effect of dihydroergocristine on cognitive functions and sleep in elderly subjects].

The aim of this study was to assess the effect induced by the dopamine agonist dihydroergocristine (DHEC, CAS 17479-19-5), whose memory-improving activity is well-known, on sleep pattern and cognitive function, and the possible relationship between them. Ten elderly volunteers were included in the study. Selected subjects had to be neither demented nor depressed, according to neuropsychodiagnostic criteria (SCAG < 30, Hachinski dementia score < or = 15, Hachinski ischemic score < 6, HRSD < or = 22). All subjects underwent a nightly polysomnographic evaluation during placebo, after single and long-term once-daily 6 mg DHEC administration. Cognitive function and attention were also assessed by the Randt memory test and WAIS digit-symbol subtest. DHEC caused a marked and significant increase in the acquisition subitem, and memory improvement was documented during DHEC treatment. A significant direct relationship between the effect of DHEC on REM sleep and memory test was also evidenced. Our results confirmed the role of REM sleep in the restoration of cognitive function.

[Patients with stroke treated with TRH-T. Evaluation of electric brain activity using traditional and computerized methods].

This study was aimed at evaluating the EEG effects of acutely and chronically administered protirelin tartrate in patients with acute and chronic cerebrovascular disease by traditional and computerized analysis of cerebral electric activity. Our data showed: a) TRH-T did not induce EEG abnormalities; b) acute and chronic administration of TRH-T increased alpha frequency band and decreased delta and theta activities.

[Electrophysiological findings after TRH-T administration to healthy volunteers].

The central effect of acutely and chronically administered TRH-T were evaluated in ten volunteers by analysis of spontaneous and evoked cerebral electric activity. None of the subjects showed significant BAER changes, of EEG abnormalities. In some subjects acute and chronic administration of TRH-T increased alfa and beta 1 activities.

The effect of nocturnal physiological sleep on various movement disorders.

Thirty-one subjects affected by different movement disorders underwent polygraphic and videotape monitoring during nocturnal sleep, to assess movement patterns during the night. It was possible to distinguish two categories of disorders according to their pattern of movements. In the largest group (Meige's syndrome, blepharospasm, amyotrophic choreoacanthocytosis, Tourette syndrome, tonic foot, hemiballism) abnormal movements were still present during sleep, but decreased in frequency and amplitude in all stages. The second group presented three syndromes (nocturnal paroxysmal dystonia, nocturnal myoclonus, restless legs syndrome), in which light non-rapid-eye-movement sleep induced a strong activation of abnormal movements, whereas rapid-eye-movement sleep suppressed them.
