[Winter sports injuries of the hand. 3 problem injuries].

Growing redistribution of injuries has been recordable from winter sport activities, in recent years, with the upper extremity being more often affected than in the past. This prompted the authors to undertake follow-up checks for the purpose of verifying therapeutic success of patients treated earlier for three frequent injuries which had quite often been underestimated or even overlooked, rupture of the ulnar ligament of the metacarpophalangeal joint of the thumb, fracture of the distal radius, and fracture of the scaphoid. Included in the follow-up check were 37 patients with rupture of the ulnar ligament, 34 with fracture of the distal radius, and 11 with scaphoid fracture. All of them had been treated by the authors, in 1982 and 1983. Periods between 4 and 26 months had passed from surgical treatment or beginning of conservative therapy at the time of the follow-up check.

[Post-traumatic arthrosis of the upper ankle joint in chronic insufficiency of the fibular ligament].

X-ray studies of the ankle joints of 209 patients with operatively certified ankle joint instability were examined retrospectively in order to estimate the importance of lateral instability in causing degenerative osteoarthritis of the ankle joint. The patients were distributed into two groups: group A (126 patients with chronic fibular instability; 98 were up to 29 years of age and 28 between 30 and 40 years) and group B (83 patients with a fresh fibular rupture: 70 up to 29 years and 13 between 30 and 40 years). For more subtle differentiation of the grades of osteoarthritis, a new score system was developed. The results were tested statistically. In the patients with chronic instability in the younger group signs of degenerative arthritis could be seen twice as frequently as in the recently injured patients; in particular, higher grades of arthritis were found five times more frequently than in the recently injured. All patients with chronic instability in the older group had signs of degenerative arthritis. Higher grades of degenerative arthritis occurred ten times more frequently than in the recently injured. Regarding the recently injured patients, there was no significant difference between the younger and the older group. It is concluded that chronic fibular instability is an important factor in the development of degenerative arthritis, which shows the importance of early stabilization in order to prevent the development of degenerative arthritis.

[Treatment of fresh and aged cruciate ligament ruptures with combined autologous and alloplastic reinforced tendon transplant (polypropylene ligament)].

After discussing basic problems in repair and reconstruction of acute as well as secondary anterior cruciate ligament-lesions authors present a modified Kennedy-MacIntosh technique of acl-reconstruction. Temporary compensation of loss in tensile strength of the autograft (quadriceps-patellar-tendon) is achieved by an 8 mm Polypropylene band with excellent biocompatibility and mechanical characteristics. "Biological fixation" of the distal part of the tendon-graft avoids "stress shielding" and physiological applied stress may lead to remodeling and structuring in the sense of functional adaptation. Acute acl-ruptures are sutured or reinserted and treated with fibrine-tissue-glue. Furthermore a semitendinosus-autograft combined with a 6 mm Polypropylene band is added. Details of the operations-procedure are presented as well as steps in postoperative treatment. Early results and complications are finally discussed.

[Drowning, swimming pool death and other emergencies related to swimming].

In the Federal Republic of Germany, some 700 people drown every year. Of particular importance is the so-called swimming bath black-out that occurs while swimming under water, which must always be avoidable when the causes are known and appropriate precautionary measures taken. Of life-saving importance in a case of drowning is the rapid and proper use of first aid. The severe pulmonary injury induced by freshwater or salt water aspiration urgently requires the treatment of the victim in an intensive care unit of a hospital, even after successful resuscitation.

[Cartilage adhesion on the knee joint. Clinical follow-up].

During the years 1979 to 1982, 34 cartilage adhesion with fibrin were performed in the knee joint. 29 patients could be re-examined. 21 patients additionally underwent a control arthroscopy. Analyzation of the results showed that the prognostical outlook after a replantation of old cartilage fragments into a degeneratively affected fracture bed must be considered as poor. Better results were achieved when the cartilage lesion could be treated in a fresh state.

[Experiences with Weber's subcapital rotation osteotomy in recurrent shoulder dislocation].

In 23 patients suffering from recurrent posttraumatic dislocation of the shoulder joint the derotation of the humeral head was performed as described by Weber. A description of the operative technique is given. As recommended by Weber an angled 1/2-tubular plate with 7 holes was used for stable osteosynthesis 17 times. After movement treatment removal of the osteo-synthesis-material was possible 6 to 8 months later. Because of high absence of recidivity with repair of full mobility and full strength the derotation of the humeral head is a treatment of choice in cases of recurrent posttraumatic dislocation of the shoulder joint.

[Injuries of the posterior end of the medial meniscus and the posterior oblique ligament. Classification and therapy].

Meniscectomy undoubtedly leads to arthrotic degeneration in the knee joint. This can be proven by X-ray technique. It is especially the resection of the medial meniscus which increases the anteromedial instability of the knee joint. It is important therefore to preserve a meniscus whenever this is possible. The resection or partial resection of a medial meniscus showing a lesion in loco classico is still indicated. If, however, a meniscus is only marginally detached from its fixation to the lig. collaterale mediale posterior a carefully carried out reinsertion of the medial meniscus has proven to be appropriate. It is on one hand the vascular supply of the base of the meniscus which indicates a reinsertion, on the other hand an anteromedial instability of the knee joint can be compensated by a reinsertion.

[Gluing of fresh cartilaginous lesions of the talus using concentrated fibrinogen adhesive (author's transl)].

Lesions of the talar cartilage are extremely difficult to diagnose clinically or roentgenologically. Often they are masked by accompanying lesions of the bone or the capsular ligaments, so that arthrotomy will be necessary to obtain information on the condition of the cartilage. Reconstruction of the articular surface is a most important prerequisite für restoring the ability of the cartilage to cope with physiological load. Hence, the aim of the treatment is the replantation of abraded cartilaginous fragments. The use of concentrated fibrinogen as a biological tissue adhesive has been gaining in importance as an alternative to the methods of refixation practised so far. The actual process of gluing is technically simple. 7 patients in whom gluing of the cartilage at the talus had been performed, remained free from complaints for a follow-up period of three years.
