The relationship between gastric carcinoma and gastric juice lactate (L + D) and lactate dehydrogenase.

L-Lactate, D-lactate, and lactate dehydrogenase (LDH) levels were measured in the resting gastric juice of 73 patients and mean levels were found to be significantly higher in patients with gastric carcinoma than in those with benign gastric ulcer or those with normal stomachs. Raised levels of gastric juice L-lactate (greater than 1.3 mmol/l), D-lactate (greater than 0.15 mmol/l), and LDH (greater than 55 IU/1) were seen in 14 of 27 patients with carcinomas, two of 15 with benign ulcers, and six of 31 controls. Both L/D-lactate and LDH levels correlated with tumor size and depth of invasion. While six problem cases with large gastric lesions were correctly diagnosed using these indices, there was only poor sensitivity and moderate specificity, as early cancers had normal L/D-lactate and LDH levels, and these indices were raised in a significant number of controls. Thus while it is of interest that L/D-lactate and LDH are elevated in the gastric juice of patients with gastric carcinoma there appears to be little optimism for using these indices in the early diagnosis of gastric neoplasia.

Restriction endonuclease mapping of globin genes in beta-thalassaemia.

We have used the restriction endonuclease mapping technique to analyse the globin genes in local cases of beta-thalassaemia major. No gross deletions or rearrangements of the beta-globin gene locus were detected. In one individual the point mutation within the beta-globin gene which gives rise to a beta(0)-thalassaemia allele was detected by use of a restriction enzyme which cuts the gene at this site. Analysis of restriction fragment length polymorphisms at the beta-globin locus showed that antenatal diagnosis of the disorder could be accomplished in an indigenous family with thalassaemia. The application of the gene-mapping technique to the diagnosis of sickle cell anaemia and alpha-thalassaemia is also discussed.

Pressure induced rhabdomyolysis complicating an undiscovered fall.

An 82-year-old man developed severe pressure-induced rhabdomyolysis after falling and remaining undiscovered on the floor. This unusually severe case is reported to illustrate the features of a condition which in milder degree may be common in old people and give rise to diagnostic confusion.

The use of adenosine deaminase assays in the diagnosis of tuberculosis.

We describe a rapid method for the estimation of adenosine deaminase (ADA) activity in cerebrospinal, pleural, peritoneal, and pericardial fluids. Tuberculous effusions show significantly higher levels of ADA activity compared with effusions due to other underlying lesions such as neoplasms, bacterial and viral infections and simple transudates. Similarly, tuberculous meningitis results in raised ADA activity in the cerebrospinal fluid (CSF) when compared with the CSF from patients with other neurological disorders and from normal subjects. The results in 359 cases studied indicated the value of ADA assays in the diagnosis of tuberculous diseases. Levels above 30 IU/I in effusions and above 6 IU/I in the CSF indicate probable tuberculosis.

Keriorrhoea--the passage of oil per rectum--after ingestion of marine wax esters.

Lepidocybium flavobrunneum, a marine fish of the snake mackerel family, is sometimes sold under the category of 'butterfish'. Ingestion of this fish was observed to be followed by the passage of oil per rectum. The fish was found to contain 23% by weight of lipid and analysis of this, and of the oil passed per rectum, showed that it consisted of wax esters. The high proportion of wax esters in the flesh of the fish, their resistance to digestive enzymes, and their low melting point, result in pooling in the rectum of significant volumes of oil, the lubricant quality of which causes frequent calls to stool. The symptomatology is therefore quite characteristic, but with the quantities normally ingested the condition appears to be harmless, despite reports of toxic effects in experimental animals.

Density dependence of expiratory flow and bronchodilator response in asthma.

Change in the magnitude of density dependence of the maximal expiratory flow (D/MEF) following inhalation of isoproterenol was used as a test for predicting the long term response to isoproterenol vs atropine in 24 adult patients with longstanding asthma. Eleven subjects showed a decrease in D/MEF manifested by increase in volume of isoflow (VisoV) and/or decrease in Vmax50 Helox/air following isoproterenol inhalation (group 1). Thirteen subjects manifested an opposite response (group 2). Atropine sulphate (0.08 mg/kg) and isoproterenol hydrochloride (2.5 mg) were then administered by inhalation, each four times a day for seven days in a randomized double-blind cross over fashion to all subjects. One of group 1 but ten of group 2 subjects had a greater subjective and objective improvement with atropine than with isoproterenol (P less than .005). An increase in D/MEF following isoproterenol can be used as a test to predict a better response to atropine than to isoproterenol over a one week period. Such a response occurs in almost half of the adult chronic asthmatic patients. The results are consistent with a preferential dilatation of the large airways by atropine.

Ophthalmic manifestations of Aarskog (facial-digital-genital) syndrome.

Four boys with facial-digital-genital or Aarskog syndrome were whort with triangular faces, characteristic deformities of the hands and feet, and anomalies of the external genitalia. The syndrome appears to be inherited in an X-chromosomes-limked recessive manner. Previous reports emphasized the presence of hypertelorism but careful measurements of the interorbital dimensions revealed primary telecanthus in addition to hypertelorism. The palpebral fissures had a marked antimongoloid obliquity and in half the reported cases, there was unilateral or bilateral congenital blepharoptosis. Strabismus, hyperopic astigmatism, and large corneas may be additional features.
