In order to widen the diagnostic capability of single-plane transesophageal echocardiography, which has been so far confined to transverse imaging planes, we obtained four transgastric longitudinal echocardiographic views which have not been previously described. These views can image structures such as superior and inferior vena cava, the right ventricular inflow and outflow tract, the mitral apparatus and the ascending aorta, which are poorly visualized by transesophageal transverse single-plane echocardiography. Among 400 consecutive patients these scans gave relevant additional diagnostic information in 62 cases (15.5%) and provided the correct diagnosis in 37 (9.2%). There were no complications related to the longer gastric manipulation of the probe and the quality of the images was high. We conclude that longitudinal echotomographic scanning of the heart is not exclusively confined to the use of biplane or omniplane probes, but longitudinal views can be consistently obtained with a single-plane instrument.
Occurrence since 1986 and extension as an extended common source epidemic in United Kingdom of a new neurological bovine condition: bovine spongiform encephalopathy (BSE) initiated numerous epidemiological research actions in this country in order to discover the origin of this new disease, predict its evolution and implement control measures. In the same time epidemiological surveillance systems have been set up in several European countries leading to the identification of sporadic BSE cases in Ireland, Switzerland and France. In spite of the numerous informations published, a number of topics are not precisely known: identity and origin of BSE agent, influence of contaminated bovine genetics over disease development in contaminated bovine, infection incidence in herds where disease has been identified. As BSE is a member of the group of transmissible spongiform encephalopathies, concerns about foodborne transmission from cattle to human and inoculation of medicinal products containing bovine material and used for therapy or prevention in human beings or animals, has been expressed. The demonstration of iatrogenic transmission between human beings of Creutzfeld-Jakob disease has considerably influenced the implementation of measures to prevent the hypothetical risk for man to be infected by medicinal products or medical devises containing material of bovine origin.
Acute respiratory failure (ARF) occurred in two patients who had normal preoperative findings and intraoperative courses, early after posterior cranial fossa (PCF) surgery. In both instances, the postoperative pulmonary failure appeared to be due to an unknown pulmonary cancer, revealed by autopsy. The diagnosis of an asymptomatic pulmonary malignancy might be considered in the presence of ARF of unclear etiology in patients who undergo PCF surgery for removal of a brain tumor.
Once more, the authors are confronted with the difficult interpretation of senological images. When reviewing the radiologic features of a series including the symptomatic, the asymptomatic and the male breasts, the goal is to relate the patterns to clinical and histologic findings in order to improve the perception of the radiologist himself in transferring and integrating the information. The final conclusions, after reviewing the images, are: a) for the correct interpretation of senological images, the sign must be correlated with clinical findings and with data from other modalities; b) the radiologist's attitude must be always investigative and never exhaustive, due to the variability of all factors, which sometimes make a judgement of "radiological tranquility" in the breast impossible; c) the radiologist must, nowadays, improve his perception of senological images, thus becoming the one who knows how to correlate clinics to the images. By increasing his perception, his interpretation capabilities will certainly increase too.
We describe the case of a woman in whom a Ki-1 lymphoma confined to the meninges developed nearly 4 years after complete remission of a Hodgkin lymphoma. The neurological symptoms appeared in two stages. The first, marked by multi-radicular impairment, cleared almost completely within a few months. The second, a year after the beginning of the first, manifested with a syndrome of meningeal irritation, which quickly led to death. We discuss the clinical course and the possible relationship between Hodgkin lymphoma and Ki-1 lymphoma.
Five cows were inoculated with 3.10(9) CFU Mycoplasma mycoides subsp. mycoides. Five other cows and five goats were placed in close contact with inoculated cows. Clinical observations, serological survey and microbial excretion by culture for mycoplasma from tracheobronchial washes liquid were carried out during 4 months. Mycoplasmas excretion is detected before antibodies responses on all 10 infected cows. The cows never presented clinical respiratory signs and showed no or light lesions but one animal having typical lesions of acute form of contagious bovine pleuropneumonia. Goats do not seem to be susceptible and play no part in the spreading of the infection.
During an experimental reproduction of CBPP, 5 inoculated cows and 5 contacts cows were bled twice a week and antibodies research was performed using complement fixation test (CFT), passive haemagglutination test (PHAT) and slide agglutination test (SAT). In the same period, trachobronchial washings were performed weekly to detect and to count Mycoplasma mycoides subsp. mycoides SC. For four months these tests were used to compare antibodies kinetics with kinetics of mycoplasmas excretion. With titer over 40 as threshold of positivity, PHAT detects antibodies earlier than CFT and SAT at the beginning of infection, but fails to detect chronic carriers. In contacts cows no failure has been observed with CFT and SAT except during prodromic phase of infection. These results are valid for natural infections. However, for inoculated animals, most serious failures to detect infected cows occur.
Four groups of cattle were experimentally immunised by four mycoplasma species of "mycoides-like" group, Mycoplasma (M) capricolum, M. mycoides subsp. mycoides (LC), M. mycoides subsp. capri and M. species group 7 of LEACH (PG50). They were then bled weekly during 2 months to establish antibodies kinetics against homologous and heterologous antigens. The standard method of complement fixation test (CFT) used in Europe and a new ELISA test for diagnosis of contagious bovine pleuropneumonia were performed in comparison with passive haemagglutination test (PHA) against antigens used for experimental immunisation. Cross reactions obtained are rather equal to the degree of similitude between these mycoplasma species. With CFT-cross reactions are transitory and occur only while homologous titers are very high, particularly with "PG50" and the two caprine mycoides strains. ELISA results using a threshold of positivity of optical density of 0.20, were similar to that obtained with CFT except ELISA specificity is not so different from CFT one. This experimental model could explain some natural situations.

We have used pulsed Doppler to ascertain effective atrial contribution to ventricular filling in sequential pacemaker rhythm. The technique is particularly valuable when electrocardiographic evidence of atrial capture is uncertain.
Nineteen patients with distal juxtapapillary choledochoduodenal fistula diagnosed by endoscopic retrograde cholangiopancreatography are reported. Eighty-four percent of the patients had undergone previous operations on the biliary tract. Fistula appears to be mostly secondary to common bile duct exploration with rigid probes or to a technical mistake in performing sphincteroplasty. Spontaneous fistula secondary to the passage of a stone into the duodenal lumen may also occur. Common duct stones were responsible for the symptoms in most cases (70%). Endoscopic sphincterotomy showed good results in cholecystectomized patients with stones. Surgery directed to the fistula or biliary by-pass are not recommended. Uncomplicated fistula probably does not require treatment.

The localized form of pigmented villonodular synovitis (PVNS) is a rare pathological entity characterized by a limited involvement of the synovium. The knee is most commonly affected. In the knee joint, the disorder generally presents as a nodular, pedunculated lesion protruding into the articular cavity. The lesion is usually revealed by mechanical symptoms such as locking. We report on two cases of localized PVNS that involved the patellar fat pad, an extremely rare area of involvement. Mechanical symptoms were not present. In one case, the lesion reached considerable dimensions without interfering with joint function. Arthroscopic treatment was easily performed. No recurrence was observed at 1-year follow up.
A study was conducted to determine the metabolic fate of deltamethrin in lactating cows after a 'pour-on' application. Two cows were treated with 0.1 g deltamethrin and two with 1 g of the compound. Urine, faeces, milk and blood were collected over an 8-day period and analyzed for deltamethrin. This preliminary experiment has shown that it is necessary to undertake further experiment of longer duration (1 month). Deltamethrin was rapidly absorbed and slowly excreted. In milk, residues levels were very low: less than 1% of the treatment dose, and maximum levels were reached after 2 days (0.009 micrograms/ml for 0.1 g deltamethrin and 0.053 micrograms/ml for 1 g deltamethrin). For cows 1 and 2 (0.1 g deltamethrin), no residue level was detected at the detection limit (0.001 micrograms/ml) after 8 days. A total of 0.3-0.6% of excreted deltamethrin was present in urine, and no residue was found after 8 days. The major route for elimination was via faeces (about 95% of the total eliminated compound). Maxima were reached after 2 days and were still present after 8 days. Results shown in this study substantiate previously published work.
Endovaginal sonography plays a major role in the study of pelvic endometriosis and uterine adenomyosis. Other procedures as CT or MRI are reserved for particular cases. Diagnostic problems and main radiologic findings are analyzed.
In breast physiopathology, in spite of the crop of information and clinical studies, a number of questions are still unsolved. The adequacy of research sources and the transmission of information to clinical practice through consistent and validated efficacy evidences are required to ensure the treatment quality. In this article the different specialist approaches and the main themes debated in the interdisciplinary approach to breast cancer are considered: the genetical risk factors, the role of diagnostic imaging, the mapping and assessment of sentinel lymph node, the role of nodal radiation therapy following mastectomy.
It is the purpose of this study to evaluate the radiographic results of the conservative treatment of fractures of the thoracolumbar passage in relation to the initial and long-term injury parameters measured by using three different methods: the sagittal index (SI), by Farcy et al., the McRae Index, the Knight et al. Index. The predictive value for each single method was calculated, to determine which of them could be reliable for the purposes of defining primary stability of the injured spine segment. The three methods of measurement were used for initial evaluation and at follow-up (mean 31 months; range 9-45) in 60 non-neurologic thoracolumbar fractures treated conservatively. The fractures were classified based on Magerl et al. Initial instability was evaluated on the basis of the progression of the kyphotic deformity (by at least 5 Cobb degrees as compared to the pre-treatment condition). The Pearson chi 2 test was used for a statistical analysis of the data. There was progression in kyphosis of the fracture in 36 cases (60%). The sagittal index (SI) measured on initial X-rays revealed 27 (76%) potentially stable fractures, while the McRae Index considered 19 to be at risk (55%), the Knight Index, 15 (43%). The higher predictive value (73%) of the sagittal index shows the greater reliability of this method of measurement, as compared to the McRae Index (predictive value = 60%) and the Knight Index (53%) in recognizing the initial phases to be those where the potential instability of the fracture is observed. Even when the SI is applied, the probability of an error in evaluation occurring is 27%. This fact once again stresses the need for more accurate radiological examinations (CT scan, MRI) that clarify the morphological aspects of the lesion injury in an exact manner.
