Hereditary angioedema and thyroid autoimmunity.

Sera from 91 patients with hereditary angioedema were screened for thyroid antibodies. The results for the 77 patients more than 17 years old were compared with previously published data for the prevalence of thyroid disease in a large community (Whickham). Of the female patients with hereditary angioedema, the prevalence of thyroglobulin antibodies (TGA) was 14.0%, higher than the expected 3% (p less than 0.001). The prevalence of thyroid microsomal antibodies (TMA) was 20%, also higher than the expected 7.6% (p less than 0.01). The age distributions of the females in both groups differed: in the group with hereditary angioedema there was a greater proportion of younger patients which should have resulted in a lower prevalence of thyroid antibodies. Adjusting for related patients with hereditary angioedema, there was still an increased prevalence of TGA (p less than 0.01) and TMA (p less than 0.01).

Paracetamol toxicity: is enzyme induction important?

A female patient receiving carbamazepine developed severe liver damage after an overdose of paracetamol. Antidotal therapy was not administered because her plasma paracetamol concentration was below that normally indicating the need for such therapy. Subsequently, carbamazepine was readministered and resulted in marked induction of the metabolism of a single dose of paracetamol. Antidotal therapy may be indicated if the plasma paracetamol concentrations are 70% or more of those shown in the conventional treatment line, if the patients are also receiving enzyme-inducing drugs.

Action of hormonal contraceptives on the coagulation system and some of its inhibitors.

Changes in the level of inhibitors of the coagulation system, primarily of thrombin neutralizing factors have been studied during the prolonged use of four products of contraceptive preparations containing various amounts of oestrogens and progestogens, two fixed dose pills (Bisecurin, Ovidon), a low dose combination pill (Rigevidon) and a biphasic preparation (Anteovin). Thrombelastographic values referred to hypercoagulability while the results of other examinations indicating activation of the coagulation system did not show definite changes in comparison with the control. The activity and quantity of antithrombin III decreased but never below 80%. Except Anteovin all contraceptives significantly enhanced alpha 1-antitrypsin while alpha 2-macroglobulin levels, remained nearly the same as the control values. Attention is called to that the increased alpha 1-antitrypsin level may be a biochemical risk factor. The results showed that the increased coagulability and disposition to thromboembolic disorders caused by hormonal contraceptives may be attributed not only to the decrease of thrombin inhibitors but also to increased alpha 1-antitrypsin levels which may cause increased inhibition of the fibrinolytic system.

IgM paraprotein in the subretinal fluid of a patient with recurrent retinal detachment and Waldenström's macroglobulinaemia.

IgM paraprotein was detected by immunoelectrophoresis and polyacrylamide-gel electrophoresis in the subretinal fluid (SRF) of a patient suffering from recurrent retinal detachment and Waldenström's macroglobulinaemia. Paraproteins in large quantities may inhibit fibrin formation which in turn may be an important agent in retinal reattachment and the starting point of scar formation in the subretinal space. By this case report the authors wanted to support the assumption that changes in the protein composition of SRF might explain the unsuccessful outcome in certain cases of retinal detachment when reattachment could not be achieved though the diagnosis was correct and the surgery performed correctly.

The role of laparoscopy in the diagnosis of ectopic pregnancy: a plea for conservative management.

This report evaluates the role of laparoscopy in patients admitted with the diagnosis of "Rule Out Ectopic". There were 186 patients admitted to the Brookdale Hospital Medical Center in the 6-year period 1974-1979 inclusive with a diagnosis of "Rule Out Ectopic" or "Possible Unruptured Ectopic". These patients were all laparoscoped. There were 157 satisfactory and 27 unsatisfactory laparoscopies. There were 71 ectopic pregnancies: 42 ruptured, 25 unruptured. Thirty-six cysts were diagnosed. The value of early laparoscopy is clearly established. Particular emphasis is placed on the diagnosis of the unruptured ectopic so that appropriate conservative management of the tube can be instituted in order to preserve potential reproductive capacity.

Haemostasis in diabetics.

Haemostasis was studied in 34 diabetic patients with and without detectable vascular complications (micro- and macroangiopathy). Information on platelet functions was obtained by beta-thromboglobulin determination, and of heparin-thrombin coagulation time, platelet aggregation in vivo, and on the condition of the vessel walls by estimation of factor VIII-protein (VIIIR:Ag). The results were suggestive of an increased platelet activity, the most marked abnormalities having been found in cases of angiopathy. Attention is drawn to the therapeutic possibilities offered by studies of the pathogenetic role of the abnormalities of haemostasis in diabetes.

Antithrombin III levels in term and preterm infants measured by rate nephelometry.

The plasma antithrombin III level was measured in term and premature newborns within the first 24 h of life; in addition to the authors', own immunological method utilizing rate nephelometry the assays of Laurell and Kabi Coatest were used. Antithrombin III concentration showed a linear correlation with the degree of maturity, in accordance with previous data. The lowest values were found in premature babies affected by idiopathic respiratory distress syndrome who later developed pulmonary haemorrhage. At all gestational ages the antithrombin III functional index was lower than 1.0, indicating antithrombin III consumption. Since both concentration and activity of antithrombin III are low in prematures, they are at an increased risk for coagulopathies, hence substitution therapy is indicated. Measurement of antithrombin III by rate nephelometry can also be used for following changes in inhibitor concentration.

Neurodevelopmental functioning of good and poor readers in urban schools.

This study investigated specific aspects of neurodevelopmental functioning in good and poor readers entering second grade and followed their neurodevelopmental course and reading progress over a three-year period. Poor readers at age seven performed significantly worse than good readers on assessment of gross and fine motor coordination and corticosensory functioning. Furthermore, the presence of soft signs at age seven was significantly predictive of reading achievement at age ten. Group differences in neurodevelopmental functioning disappeared by age ten, and the presence of soft signs at ages eight and nine no longer was predictive of reading achievement. These findings have implications for pediatricians, educators, and researchers exploring brain-behavior relationships.
