[Meniscal injury in the plastic reconstruction of the anterior cruciate ligament. Meniscal suture or abstention].

The purpose of this study was to observe the change of the meniscal lesions, which are conservable by suture or simply by leaving-in-place within the frame of operated anterior instability. Forty-six knees which had been operated by the free transplant of bone-patellar tendon-bone, presented the associated lesions of conservable meniscus. These lesions were treated by suture of the meniscus in 15 cases and left in place in 31 cases. The programme of rehabilitation was the same in the two groups. All the patients have been followed-up for 26 months in average (12-40 months). In the group of meniscal suture, only one secondary meniscectomy was carried out. 8 patients presented moderate pain on the corresponding femorotibial joint line and 5/13 of the sportive patients recovered to the same level of activity. In the group of leaving-in-place, no meniscectomy has been carried out. Only 4 patients complained of pain and 23/27 recovered to the same activity level. The arthrographic and arthroscopic observations have been carried out in 23 cases. They confirmed the cicatrization of the sutured meniscus but also showed the possibility of total or partial cicatrization of the lesions left in place (12 on 13). The authors conclude that the peripheral meniscal lesions associated with the operation of the chronic anterior instability do not always require suture. The abstention on the meniscal lesion is possible if it is limited on the posterior segment: Then the functional results seem better.

[Arthroscopic internal meniscectomy in patients over 55 years of age. Results over more than 4 years].

Fifty-one meniscectomies by arthroscopy were realised on patients of more than 55 years of age between 1981 and 1986. Twenty-four patients were reviewed after at least 4 years of follow up, 12 of them after a 7.5 years follow up. 21 results were noted as functionally very good or good, 3 were poor. The radiological analysis of femoro-tibial compartments in comparison with the non operative side showed a clear pejorative difference in one case, moderate in 5 without incriminating the morphological type. If the quality of result does'nt correlate with age, it was'nt the same for the type of lesion seen during arthroscopy, the best results after arthroscopic meniscectomies were seen in traumatic lesions occurring on normal menisci or on degenerative ones. The authors concluded that medial meniscectomy in patients older than 55 years is justified in precise indications: a known traumatism or also acute medial pain, or speedy with normal or subnormal X-ray and after a positive arthrography.

Temperature-jump method for studying the fast transport of Na+ by (221) C10-cryptand across lipid membranes.

The kinetics of Na+ transport by (221)C10-cryptand through thin lipid membranes were determined by performing temperature-jump relaxation experiments on large unilamellar vesicles (L.U.V.) loaded with a fluorescent pH indicator. Applying temperature jumps of 4 to 7 degrees C to liposomes having phosphate as internal buffer and Tris as external buffer resulted in transmembrane delta pH's of about 0.104 to 0.182. After a temperature-jump, a decay in the delta pH was observed which corresponded to a Na+/H+ exchange occurring through membranes in the simultaneous presence of the cryptand and a proton carrier. The transport of Na+ ions by (221)C10 was found to be a fast kinetic process. Its initial rate increased with both the temperature and the cryptand concentrations. In addition, the temperature-induced changes in the apparent rate constants of the translocation of Na+ by (221)C10 were carrier concentration-dependent, and the apparent activation energy required to activate the transport decreased significantly with increasing cryptand concentrations. The results are discussed in terms of the structural, physico-chemical and electrical characteristics of carriers and complexes.

[Arthroscopy of the shoulder with the patient in beach-chair position].

The authors describe a technique of arthroscopy in the "beach chair" position, that they have exclusively used since 1989. The classical setting in lateral decubitus takes a long time and can be the cause of complications especially neurological due to the traction. In the chair position, the vision of the intraarticular structures is identical to the lateral position, and the absence of traction avoid the neurological complication. Another advantage is the quick and easy setting; the locoregional anesthesia is easy to use, it is possible to practice an open surgery without changing the setting. All these advantages lead the authors to propose this position for all arthroscopy of the shoulder whether diagnostic or therapeutic.

[Massive delayed-action verapamil poisoning].

An 18-year old woman developed cardiogenic shock after ingestion of 16.8 g of a sustained release form of verapamil. Severe left ventricular diastolic dysfunction was demonstrated. For the first time muscular involvement was observed with myalgia and elevation of the MM isoenzyme of creatinine kinase. The efficacy isoproterenol and the need for prolonged treatment are emphasised.

Arthroscopic treatment of osteochondral lesions of the talar dome.

Nine osteochondral lesions of the talar dome were treated arthroscopically. Two patients (a 16-year-old boy) had two recent osteochondral fractures, anterior and superior lateral, with a free fragment detached in the joint. The lesions were treated with simple ablation of the loose body. Seven old lesions were found in four men and three women, aged 18-32 years, with an osteocartilaginous, partially loose body with necrosis of the underlying bone. The lesion was posterior medial in these seven cases. Technically, arthroscopy is usually performed using an anterior approach. However, it is sometimes necessary to place the foot in the talipes equinus position to achieve articular distraction for diagnosis and treatment of posterior lesions. Treatment consisted of removal of the loose body, with curettage of the necrotic bone. The nine patients were clinically and radiologically reviewed with a follow-up period of 10-24 months. For the two osteochondral fractures, clinical results following the removal of free loose bodies in the joint were spectacular, with complete pain relief and osseous rehabilitation almost radiologically complete after 2 years. In the seven patients with an old necrotic lesion, the result was very good or good in six cases, and poor in one case. Radiologically, osseous rehabilitation occurred progressively, but remained incomplete at 2 years. This technique provides multiple advantages: minimal morbidity, 48-h hospitalization, and rapid functional recuperation without immobilization. Our results confirm the recent data in the literature (1-4).

[Functional treatment of recent fractures of the humeral shaft using the Sarmiento method].

Functional treatment of humeral shaft recent fractures by A. Sarmiento's method was described in 1977. It is based on fixation with a plaster or plastic sleeve individually molded. This sleeve keeps free the adjacent joints, allowing an early functional rehabilitation and a fast consolidation. A 50 cases serial study from 1983 to 1987, confirms the advantages of a method which allies reasonable means, safety and complete reliability to obtain a good and rapid consolidation (one case of non-union due to a technical failure), and rapidly recovered function. We had frequently resulting axial defects: 16 patients have mal-union with more than 10 degrees of angular deformity but without any functional consequences. The advantage of this method compared to usual orthopaedic, or surgical treatments leads us to propose it to most of our patients.

[Cardiac involvement in Behçet's disease. 12 cases].

Fifteen cardiac manifestations observed in 12 of a series of 196 patients with Behçet's disease are reported. In this retrospective study conducted in an internal medicine department, 5 cases of pericarditis and 4 cases of myocardial infarction were encountered. The other cardiac manifestations were observed only once. They included ventricular aneurysm, endomyocardial fibrosis of the right heart, aortic insufficiency, mitral valve insufficiency, mitral valve prolapse, and right heart failure consecutive to pulmonary arterial hypertension. There was no relationship between the severity of cardiac lesions and that of the extracardiac manifestations of the disease. Comparison of these data with those found in the literature showed that pericarditis is the most frequent pathology, but it usually regresses rapidly. Lesions of the coronary arteries, with or without myocardial infarction, consist of stenosis, occlusion or pseudoaneurysm requiring surgical treatment. Myocardial lesions (with the possibility of pseudoaneurysm) and endomyocardial right heart fibrosis are exceptional but fairly characteristic of Behçet's disease which they should suggest. Hughes-Stovin's syndrome may be complicated by pulmonary arterial hypertension, although death is generally caused by massive haemoptysis.

The influence of molsidomine on infarct size: an acute post-infarction pilot study with 303 patients.

In a multicenter, randomized and double-blind study, the efficiency of molsidomine on infarct size has been examined in 303 patients suffering from a first myocardial infarction and compared with a placebo. According to previous enzyme studies, and in order to detect a 20% reduction infarct size with conventional levels of risk, alpha = 0.05 and beta = 0.20, the recommended sample size was 264 patients. Thirty-three patients initially selected were excluded for protocol violation and, among the 270 patients definitively included, 133 were allocated to molsidomine and 137 to placebo, without any difference concerning age, delay of treatment, infarct location, and initial blood pressure. Test drugs were both initiated within the 6 first hours and administered orally at decreasing doses for 10 days: 16 mg on the first day, 12 mg on the second day, and 6 mg daily from the third to the tenth days. There was not a significant difference between the molsidomine and placebo groups regarding the enzyme evaluation of infarct size, neither for CK dosage (101.72 +/- 74.76 gram equivalents vs. 92.71 +/- 65.91 gram equivalents, NS) nor for its MB fraction (67.34 +/- 50.07 gram equivalents vs. 63.50 +/- 43.01 gram equivalents, NS). Moreover, changes in the Q- or R-wave sum during the 10 days of follow-up were strictly identical. However, in-hospital mortality was lower in the molsidomine group than in the placebo group (4.5% vs. 8.0%), but this reduction was not statistically significant. During the study, there were few side effects, mainly headaches, without withdrawal of the treatment.
