[Genitourinary bilharzioma. Isolated cases of vesical bilharzioma and epididymal bilharzioma].

The authors report on two isolated cases of bilharzioma: one vesical revealed by an hematuria and one epididymal constituting an intrascrotal mass. Clinical signs did not oriented toward these diagnosis and biology was helpless. Serodiagnosis by hemagglutination was negative, but immunofluorescence was more reliable. On the contrary both tumors were detected by echotomography of which the efficiency is determinant for the diagnosis and must be underlined. Only anatomopathology was able to confirm the diagnosis of tumor. As a matter of course, surgical exploration and excision are necessary in these types of tumors with negative biology. In both cases, effects were isolated. In one patient, infestation was old and already treated; in the second one, infestation was probably light. The authors recall also bilharzioma physiopathology. Frequency of vesical bilharzioma is about 20% of genito-urinary schistosomiasis while epididymal bilharzioma reaches only 0.29 to 4.8%. Direct line between bilharzioma and cancer is likely but controversial, schistosomiasis beeing a true irritative agent.

[Shock during necrotic amoebic colitis (author's transl)].

Two varieties of conditions of shock may be isolated during necrotic amoebic colitis (13 cases). Eight patients present a simple hypovolemic shock secondary to wastage by diarrhea and perilesional oedema with globular, protein, alcaline and potassic deficiency. Its prognosis is relatively good, after vascular infilling and corrections of metabolic disorders. Five other patients present real toxi-infectious shock resulting from widespread tissue necrosis with auto-intoxication associated with septicemic complications. Its prognosis is frankly bad. The treatment is far more difficult. The use of cardiovascular analeptics, such as dopamine, after an infilling failure, is not always sufficient to re-establish the situation. The exeresis of necrotic tissues is an indispensable condition to remove the cause of the shock before septicaemic generalisation.

[Genital tuberculosis in African female patients (author's transl)].

Genital tuberculosis in 76 Senegalese women and 82 Nigerian women, have been compiled from statistical tables covering a period of 10 years. The authors present the features of genital tuberculosis in african females. In most cases, diagnosis is made through peroperative findings or anatomo-pathological control of exeresis specimens; still, biopsy of endometrium, systematically performed on each patient consulting for sterility, gave opportunity to detect an important number of cases of genital tuberculosis in Nigeria. The frequency of this disease is low among other unspecific genital infections. Its medical treatment is effective on tuberculous infection but not on sterility which is an unavoidable sequella.

[Osteo-articular tuberculosis in African (author's transl)].

General, clinical and therapeutic features of osteo-articular tuberculosis in African, excluding vertebral localizations, are compiled from 81 records: The are: -- a frequency lower than in expatriated Africans and this indicates their special physical resistance when they live in their natural environment; -- frequently an easy diagnosis because of infected advanced foci with associated lesions; -- a surgical indication (curettage, resection, arthrodesis) as frequent as in vertebral localizations.

[Pott's disease in African (author's transl)].

A study of anatomical clinical, and therapeutic features of Pott's disease in Africans from 131 cases treated by the same team and compiled from the principal series published during the last ten years in Black Africa. Anatomical features are related to the frequency of the disease (70 p. 100 of tuberculous osteo-arthritis), the elective lumbodorsal localization involving several vertebrae (gpenerally 3 or 4) and being sometimes multifocal. Clinical features are in part explained by the delay with which patients report to the hospital for the first time so that 80 to 90 p. 100 of the patients suffer from severe lesions (which are in their destructive phase in 30 p. 100 of the cases), with associated-complications (gibbosity, abcess, 50 p. 100; fistulae, 2 to 6 p. 100; paraplegie 23 to 25 p. 100; multiplefoci, 4 to 13 p. 100). Diagnostic difficulty mainly raised by "False Pott's diseases" (P. Bourrel), is considered. Therapeutics is studied; importance and place of the surgical treatment is emphasized; its various approaches are discussed. The authors report their surgical interventions on 89 patients out of 181 admitted: 64 direct approaches for simple Pott (with 70 p. 100 of good results and 20 p. 100 of satisfactory results), 25 operations for complicated cases of Pott (18 approaches for antero-lateral decompression and 5 laminectomies, with 22 satisfactory results).

[Surgical treatment of pulmonary tuberculosis in tropical environment (indications and technics) (author's transl)].

A study of 85 clinical records of patients with pulmonary tuberculosis who were operated in hospitals of South Vietnam and West Africa. Some common features may be noted: --an ongoing clinical evolution before the indication for a surgical intervention (more than 5 years in half of the cases); -- positive bacilloscopies in 70 p. 100 of the cases; -- bilateral lesions in one case out of three; -- unilateral lesions restricted to one lobe in one case out of two; -- surgery limited to a thoracoplasty for 48 p. 100 of the patients; for the remaining 52 p. 100, half underwent lobotomy, half pneumonectomy.

[Shock in abruptio placentae (author's transl)].

In sixty abruptions of the placenta observed during eighteen months, a severe condition of shock was observed in thirty per cent of patients. This shock state is seen during the abruption or after delivery. It's essentially a question of hemorrhagic shock, the importance of hemorrhagy being often underestimated, if the drop of blood pressure and blood losses are only estimated. The measurement of central venous pressure and the research of metabolic acidosis are better indexes of shock. This shock is associated with hypofibrinogenaemia and with other symptoms of disseminated intra-vascular coagulation. Fibrinolysis is rarely observed (3 times). Presence of fibrin degradation products is frequent. Renal complications are observed only in 1/6 of cases, but are frequently severe (one case of renal cortical necrosis). Heparinotherapy does not confirm the hope be suscited and seems to be a frequent source of ehmorrhagic complications in African women. Perfusion of fibrinogen, if useful, can be another source of complications. Rapid transfusion is the more effective treatment of abruption placenta and probably the less dangerous with strict control of the central venous pressure.

[Surgical approach to the so-called infectious perforations of the small intestine in Africa south of the Sahara (author's transl)].

Non traumatic perforations are frequent in Africa South of the Sahara and most often due to salmonellosis. From 50 cases recorded in Dakar, the authors study the main problems raised by diagnosis (specially in non reactive peritonitis requiring iterative controls), surgical tactic and procedure, and intensive care. In 86 p. 100 of the case they report on, the authors performed a suture completed by passive drainage. However, and in spite of the risk of leaking, resection had to be done either immediately or because suture failure. As patients were presented generaly in very bad condition, intensive cares were complex but essential. Death rate ranged between 25 p. 100 and 50 p. 100.

[Necrotizing amoebic colitis (a propos of 12 cases observed in Dakar)].

Twelve cases of necrotizing amoebic colitis recorded in Dakar Hospital Principal are studied. The authors draw basis rules for an early surgical indication giving opportunity to detect not too extensive lesions and implement intensive care which may prove effective. Prognosis is still unfavourable with a death rate of two thirds.

[Urogenital tuberculosis in African (author's transl)].

A report of nine recent cases of urogenital tuberculosis in Africans with a review of literature. This possible but rare complication must be discovered, although it is a difficult process mainly in rural areas, since tuberculosis may bring about severe urogenital tract damage. Schistosomiasis and unspecific epidimymitis may hide it. Acido-alcoolo-resistant bacillus, detected in urin, must be cultivated on Loewenstein medium to remove contaminants and to confirm tuberculous pathogenesis.

[Parasitosis and pregnancy overseas].

The influence of the parasitic diseases on pregnancy and the influence of pregnancy on the parasitic diseases are both well known concepts. The authors lay special interest on malaria, amebiasis and hookworm disease by giving a description of their effects and by mentioning the risks they may entail; then, they give practical directions intended for their detection and the treatment of their effects. It is, once again, emphasized on the utmost importance of performing antenatal examinations.
