[Phreno-celiac disease and merycism in adults].

In this paper are described two new examples of a peculiar semiological association of which the authors have already reported a case, and in which occured simultaneously a phreno-coeliac disease and merycism since childhood. Strong epigastric pains accompained the merycism. In these two cases, the compression of the coeliac trunk by the median arcuate ligament of the diaphragm was important; in one case existed besides a compressive stenosis of the superior mesenteric artery. After the surgical liberation of the coeliac artery and the peri-coeliac neurectomy in one case, all symptoms relieved. This good result persists after twelve months. Because of the rarity of the phreno-coeliac disease and of this type of merycism, the authors think that this association is not accidental, but the result of a new nosological association.

[Severe hemolytic jaundice and Wilson's disease].

The onset of spontaneous hemolytic jaundice in a young subject should lead to the search for Wilson's disease when clinical examination reveals cirrhosis. This hemolysis may evolve in the form of severe jaundice to a stage where the cirrhosis remains usually latent or well tolerated. The intervention of a toxic, allergic of infective factor liable to produce a hepatic lesion which frees a dose of copper sufficient to trigger off hemolysis, is discussed. The mechanism of the latter, that of the coagulation disorders observed, liver cell failure and widespread intravascular coagulation, are analysed in this paper and compared with data in the literature. The dramatic character of the case indicates that it is necessary to treat as a routine with penicillamine all homozygous forms of Wilson's disease.

[Endomyocardial biopsy by venous route. Our initial results].

The authors report their experience of right ventricular endomyocardial biopsy by the technic of Caves and Schulz. The instrument which consists of a spiral metallic catheter covered with synthetic material, includes at one end a hand lever and at the other end a biopsy forceps 3 mm in diameter. It is introduced through the skin into the right internal jugular vein then pushed under direct vision with an image intensifier upto the tip of the right ventricle. The angle of the distal part of the bioptome may be freely modified before use. Passage across the tricuspid orifice from the right atrium is usually easy. 30 good quality samples were thus taken from 10 patients with various forms of heart disease. There were no complications. The simplicity, rapidity and benign nature of this technic are emphasised together with its use in the diagnosis of myocardiopathies.

[Emergency surgical treatment of septal perforations in the acute phase of myocardial infarct. Apropos of 2 cases operated on successfully in the 48th hour and 5th day after development after the necrosis].

The authors report two personal cases of septal perforation during the acute phase of myocardial infarction which under medical treatment would have been rapidly fatal, and which were treated successfully by surgery on the second and fifth days respectively after the infarction, i.e. very early. After briefly recalling the main diagnostic and etiological factors of this serious complication, and the spontaneous prognosis which is usually catastrophic, they emphasise that in presence of poorly controlled heart failure and cardiogenic shock, uncontrollable by intensive medical treatment, only early surgical repair during the first few hours or days of the course of the coronary accident will permit one in certain cases to avoid a fatal issue as proved by their two cases and a review of recent publications. They emphasis the place of circulatory assistance by intraaortic balloon in the preparation of patients for operation and the methods of mycardial revascularisatin during the operation.

[Post infarction septal perforation. Is the early operation benefical?].

A case is reported of a ruptured septum in a man of 74 which was operated on after 48 hours; the authors recall the development of ideas on the surgical treatment, and also the advantages which may be expected from counterpressure from an intra-aortic balloon. If this is not available, it seems to them that the operation must be carried out even more swiftly, within the first few hours, if damage to the myocardium and the consequences of prolonged shock, are to be avoided.

[Pheochromocytoma and Recklinghausen's cutaneous neurofibromatosis. Localizing value of abdominal reflexes in a case].

The authors present a case of pheochromocytoma observed in a 49 year old man suffering from the cutaneous form of von Recklinghausen's disease. In this patient, they noted the constant absence of the inferior abdominal cutaneous reflex on the right side, contrasting with the briskness of the other abdominal cutaneous relexes. From this they concluded that the chromaffin tumor was very probably situated in the right adrenal region. This hypothesis was confirmed by retro-pneumoperitoneum, selective arteriography and finally surgical intervention. After having reviewed the essential medical data from the literature concerning the conditions associated with pheochromocytoma and especially phacomatosis, and having looked at the problem of hypertension related to neurofibromatosis, they make a critical study of the behaviour of the abdominal reflexes in case of pheochromocytoma and emphasize the part that it is possible to draw from this in view of its' topographic diagnosis. The present case is the fourth in which one of the authors has noted the abolition of the inferior abdominal reflex on the side corresponding to the site of the pheochromocytoma.

[Familial forms of the mid-end systolic click and murmur syndrome with deviations of left ventricular kinetics].

The aetiology and pathogenesis of the "mid/end-diastolic click and murmur" syndrome, with prolapse of the mitral valves, is obscure in most cases. However, the fact that some cases have had a familial distribution is evidence in favour of a dysgenetic origin. Seven new cases of this type are reported. The authors suggest in this paper that the incidence of the familial form of the syndrome is greater than the literature seems to suggest, and that the syndrome is likely to be due to a malformation. They also emphasise the important part which echocardiography plays in its diagnosis and investigation. Finally, the finding on cine-angiocardiography of specific abnormalities of left ventricular function similar to those found in other studies is indicative of a primary myocardial disorder, associated with subsequent structural alterations of the valvular mechanism.

[Echocardiographic abnormalities in the mitral valve prolapse syndrome].

Eight patients with prolapsed mitral valve syndrome, with a mid - or - end - diastolic click or murmur, underwent echocardiographic examination using ultrasound. Examination of the displacement of the valves by the "time motion" method showed all cases to have an abnormal recoil; in 6 cases this occurred in mid or late systole, and in two from the beginning of systole. The recorded amplitude of the pathological displacement, which gives the systolic tracings a domed appearance, and the consistancy with which it could be recorded (irrespective of the incidence of the ultrasonic waves), seem to afford a method for evaluating the degree of prolapse which, in most cases, involves both the valves. The authors compare their results with those which have already appeared in the literature, and emphasise the very real contribution which echocardiography can make in the diagnosis and assessment of the prolapsed mitral valve syndrome.

[Intravenous nitroglycerin as vasodilator therapy in acute myocardial infarction (author's transl)].

Thirteen patients with left ventricular failure complicating acute myocardial infarction were treated with intravenous nitroglycerin. The hemodynamic criterium for admission to this study was a pulmonary capillary wedge pressure greater than 15 mm Hg. All patients received intravenous nitroglycerin at infusion rates sufficient to lower pulmonary capillary wedge pressure by an average of 15 mm Hg, corresponding to the optimal left ventricular filling pressure in acute myocardial infarction. During this study, heart rate did not vary significantly; pulmonary capillary wedge pressure decreased from 24 +/- 0,7 to 15 +/- 0,8 mm Hg (p less than 0,001), mean arterial pressure from 95 +/- 5 to 80 +/- 4 mm Hg (p less than 0,005), mean right atrium pressure from 8,7 +/- 1 to 4,6 +/- 0,8 mm Hg (p less than 0,001), systemic vascular resistance index from 36,4 +/- 2 to 29,6 +/- 2 mm Hg/mm-1/l/m2 (p less than 0,005). Cardiac index increased from 2,5 +/- 0,2 to 2,7 +/- 0,1 l/mn/m2 (p less than 0,025) and stroke work index did not vary significantly. These data indicate that nitroglycerin provides a significant improvement of cardiac performance, more through reduction of preload than through reduction of afterload, the later being however significant. This study points out that intravenous nitroglycerin is a most efficient drug in patients with left ventricular failure complicating acute myocardial infarction.

[Value of global ejection fractions based on the presence or absence of left ventricular dyskinesia. A comparison between contrast angiography and angioscintigraphy].

The total ejection fraction of the left ventricle is studied in 37 patients, most of them with coronary disease, according to two methods: contrast angiography in a single plane and angioscintigraphy (first passage and balanced), in order to compare the results according to the presence or the absence of dyskinesias. The two methods are performed in succession according to the usual views. The population is divided in two sub-groups according to the presence (17 patients) or the absence (20 patients) of segmental abnormalities. Segmental abnormalities are divided into 11 antero-apical aneurysms and 6 infero-posterior aneurysms. The correlations of the ejection fractions calculated according to the two methods and the two views, are usually satisfactory in the two sub-groups and especially as the parietal kinetics is homogeneous. In case of segmental abnormalities of the contraction, there are variations in the total ejection fractions depending on the technique and the views that are used, but the differences are not significant.

[Diagnostic elements of a right papyraceous ventricle. The value of nuclear magnetic resonance].

We are reporting the case of a young man presenting repeated syncopes linked to episodes of ventricular tachycardia. The rhythm disorders were due to the existence of a papyraceous right ventricle with concomitant involvement of the left ventricle disclosed during the etiological work-up. We are reporting, at this time, the usual diagnostic elements of this disease, but mostly original data provided by the nuclear magnetic resonance, as the clinical examination and the EKG are often less revealing except for late potentials always found in the arrhythmic form. The right, ventricular dysplasia and the papyraceous right ventricle represent two very close entities. Particular characteristics enable to differentiate them and also the differential diagnosis with other diseases are discussed by stressing the advantage of magnetic resonance imaging, a new method, non invasive, which lends itself well to the analysis of cardiac morphology.

Acute post-infarction left ventricle rupture. Five operations with three long-term survivals.

Cardiac rupture is cause of death in myocardial infarction. Surprisingly only seventeen successful attempts at operative treatment have been published, with a rather good long term survival. The authors report five cases of cardiac rupture operated upon with two deaths and three long term survivals. Frequency, clinical features and surgical possibilities are discussed with particular insistance on a rather aggressive surgical attitude when considering this complication.

[Cystadenoma of the pancreas. Study of the angiogenesis and the associated diseases (author's transl)].

We describe a case of pancreatic cystadenoma (PC) concerning a 66 year-old woman. This PC was a highly vascular cystic tumor, with typical signs: pulsatile tumor, systolic epigastric souffle and demonstrative phonangiography; the selective angiography revealed a hypervascular mass. Microscopically, tiny cysts lined by small, cuboid and columnar cells containing no glycogen, and numerous capillaries. With this case and 210 cases from the literature, two problems have been studied in this paper: the problem of the vascularity of the PC and the problem of the associated diseases. The vascularity allows to distinguish two types of PC: PC with hypervascular pattern and hypo- or avascular PC. The study of the associated diseases shows the high incidence of the tumors (23%), benign and malignant (13%) and of the cysts in other organs (5%). Comparison with polycystic disease of the kidney and the Von Hippel-Lindau disease and hypothesis concerning angio-genesis and cystogenesis in the PC are discussed.
