Oesophagus carcinoma amino acids control.

Serum amino acids level examination of the oesophagus carcinoma patients admitted to I Surgical Clinic of Teaching Hospital in Olomouc from 1/1/1990 to 31/7/1991 were carried out by the authors. The standard values were found with the group of patients with resectionable tumor or with a tumor in non-advanced stage on the one hand and on the other hand there were some lower values with the patients with a tumor in an advanced stage. In this way two groups of the sick were examined--the first one with benign oesophagus stenosis accompanied by nutritional disorder, the second with resectionable lung carcinoma without nutritional disorder. Serum amino acids level examination is therefore considered to be rather an indicator of the important factor of a possible surgical treatment of a tumor.

[Volvulus of the cecum].

The authors present a group of seven patients treated at the First Surgical Clinic of the Faculty Hospital Olomouc on account of volvulus of the caecum during the period between 1963 and 1992. Based on their own experience which is consistent with results reported in the literature, the authors discuss the problem of diagnosis and optimal treatment of volvulus of the caecum.

[Alport's syndrome].

After a historical introduction the authors analyze contemporary views on the aetiology, pathogenesis and genetics, they characterize the clinical picture and histopathological findings in Alport's syndrome. They emphasize that Alport's syndrome is more frequent than generally reported and that it deserves therefore appropriate attention in the differential diagnosis of renal diseases. They draw attention to the results of most recent genetic research which makes it possible to detect gene carriers of Alport's syndrome as well as early prenatal and postnatal diagnosis of this most frequently progressing hereditary nephritis.

Kidney transplantation and malignant diseases.

Basing on three types of questionnaires, the authors summarized the incidence of all malignant diseases in kidney transplant patients in CSFR. The total number of registered tumours was 34. Previously to the transplantation, two patients with malignancies were found, another one was transplanted for a kidney tumour and 31 patients manifested malignant diseases in various time intervals following the transplantation. This number is five times greater than the incidence of tumours in the standard population. At classical immunosuppression, the mean time interval between transplantation and tumour formation is 48 months; at cyclosporine application, it decreases to 19 months. The most frequent sites of tumour formation are the skin and the kidney. The early diagnosis of tumours and the surgical treatment will increase the survival period up to 58 months. The systematic oncologic follow-up and preventive controls of transplant patients represent one of the possibilities of the prolongation of the patient's life with the functional graft.
