[Cystic teratoma of the diaphragm. Apropos of a case, review of the literature].

Benign tumors of the diaphragm are rare lesions. The cystic teratoma of this muscle is an internal axial tumor, a benign dysembryoma, and only 4 cases could be found in the published literature. The cystic teratoma reported was discovered fortuitously during investigation of hypertension. It was located in the diaphragm above the psoas arch.

[Giant fibroma of the pleura. Apropos of 2 cases].

Two cases are reported of giant pleural fibroma (2.9 and 4.2 kg), of slow growth (both had been present for 17 years), developing in the parietal pleura in a 56-year-old woman and in the triangular ligament of a 35-year-old man respectively, and treated by surgical excision. Findings in these cases and data on those reported in the literature indicate the principal pathologic and clinical characteristics of these very rare benign tumors of the pleura: onset in the absent of any history of exposure to dust (asbestos); usually fortuitous discovery; suggestive radiological appearances (calcification in some cases) that are not pathognomonic however; differentiation from fibrous mesothelioma by the microscopic or particularly gross pathologic appearance; treatment exclusively surgical and of variable difficulty according to the size of and especially the structures related to the tumor, with the need for careful preoperative screening.

[Adventitial cyst of the aorta].

The authors report a case of adventitial cyst of the abdominal aorta. This would seem to be the first time that a cyst has been reported in the literature with this localization. A 54 year old female patient was treated surgically for a suspected aneurysm of the abdominal aorta. Her antecedents included syphilis treated with bismuth. The condition began with a lumbago with no clearly defined etiology. An abdominal X-ray without preparation visualized a calcified abdominal tumor in a retroperitoneal position, apparently attached to the aorta. Arteriography showed that the tumor was excluded from the circulation. Surgical exploration of the aorta demonstrated a cystic tumor, which was resected. A review of the literature concerning the pathology of arterial cysts shows no previous cases of aortic cyst. On the other hand, references to identical anomalies of the iliac, femoral, popliteal and radial arteries provide grounds for discussion of the etiology of this aortic disease. The macroscopic data and histological studies suggest a phenomenon of cystic degeneration rather than a constitutional anomaly of the artery, or, in this particular case, an anomaly connected with syphilitic lesions.

[Esophagopharyngoplasty by transplantation of a right ileocolic graft in the treatment of sequelae of esophageal and hypopharyngeal caustic stenosis].

Esophagopharyngoplasty was employed to treat 7 cases of severe stenosis from scarring due to caustic burns. Emphasis is placed on the advantages of retrosternally applied right ileocolic transplants using the supraphyoidal anterior pharyngeal approach. Results were considered as satisfactory in 4 cases, exclusively oral alimentation being possible, though bougierage, sometimes repeated, was necessary. The 3 unsuccessful cases were due to loss of oropharyngeal muscle peristaltism in one case, progression of scar retraction in the second patient, and the psychological behavior and lack of essential active cooperation in the third case.

[Surgical treatment of primary hyperparathyroidism in patients over 70 years of age].

Hyperparathyroidism is a frequent, often fortuitous finding in elderly people. Our series includes 103 patients over 70 years of age, 90 of whom were operated upon. Elderly patients rarely are without any symptom, but it is difficult, prior to surgery, to associate their clinical disorders with hyperparathyroidism. Our results show that surgical treatment is virtually without danger and that most patients were distinctly improved. The innocuousness and effectiveness of surgery in these patients, as well as the absence of medical treatment are as many reasons to widen the indications of the surgical approach.
