Ultraviolet protection by summer textiles. Ultraviolet transmission measurements verified by determination of the minimal erythema dose with solar-simulated radiation.

BACKGROUND: Apart from sunscreen lotions, clothing provides protection from acute and chronic sun damage. Therefore, it is very important to know the ultraviolet (UV) protection factor (UPF) of textiles, in particular of lightweight summer clothing. Usually, the UPF of a textile is determined by spectrophotometric assessment of the UV transmission (in vitro method). OBJECTIVES: To compare the relationship between in vitro tests and in vivo tests of UPF using solar simulators for determination of the minimal erythema dose (MED), applied to 30 different summer textiles. METHODS: Thirty summer textiles were spectrophotometrically assessed, and UPFs were calculated with respect to the International Commission on Illumination (CIE) erythemal action spectrum.1 Based on the in vitro UPFs 'on skin' and 'off skin', in vivo testing was performed using a solar simulator for the determination of the MEDunprotected and MEDprotected. RESULTS: The UPFs obtained from in vivo 'on skin' testing were significantly (r = 0.95; P < 0.001) lower than the predicted in vitro UPFs. This disparity was also confirmed by chromometric assessment of the MED testing; the erythemal responses measured after textile protection were significantly (P < 0.001) higher than those obtained without protection. However, the in vivo 'off skin' UPFs did not significantly (r = 0.98; P > 0.05) differ from the in vitro UPFs; comparison of the chromometrically assessed erythemal responses was also insignificant (P > 0.05). CONCLUSIONS: The different correlation between in vitro and in vivo measurements of the UPF may be due to the optical-geometrical properties of textiles and the different amount of direct and diffuse radiation passing through the spaces between the yarns. As spectrophotometric measurements of a textile may generally yield lower UPFs than those obtained under average field conditions, the in vitro test method provides 'safe' UPF values representing a 'worst-case scenario'. In contrast to in vitro testing, in vivo methods are much more expensive and time-consuming. Thus, with respect to practicality, spectrophotometric measurements seem to be most suitable for the evaluation of UV protection of textiles.

Calcitriol 3 microg g-1 ointment in combination with ultraviolet B phototherapy for the treatment of plaque psoriasis: results of a comparative study.

BACKGROUND: Combinations of topical treatments and ultraviolet (UV) B phototherapy for plaque psoriasis may be more beneficial than either type of treatment used alone. OBJECTIVES: To determine the efficacy of calcitriol 3 microg g-1 ointment in combination with UVB phototherapy in treating plaque psoriasis. METHODS: Calcitriol ointment with UVB was compared with vehicle plus UVB in a randomized, double-blind study in 104 patients. RESULTS: Mean global improvement scores for both groups increased over the 8-week study period; there was a statistically significant difference (P < 0.05) in favour of the calcitriol/UVB combination from week 1. At end-point, 45% of the calcitriol/UVB group showed considerable improvement or clearing of psoriasis, compared with 21% of the control group. The superiority of calcitriol plus UVB was also reflected in the global severity and Psoriasis Area and Severity Index (PASI) scores; at end-point the mean percentage decrease in PASI score was 65% for the calcitriol/UVB group and 43% for vehicle/UVB (P = 0.0014). The incidence of skin-related adverse events was low (< 12%) and similar in the two treatment groups. No clinically significant changes in blood chemistry, in particular calcium levels, occurred. The greater efficacy of combined calcitriol and phototherapy allowed a 34% decrease in total UVB exposure. CONCLUSIONS: Calcitriol 3 microg g-1 ointment and UVB phototherapy in combination provides a promising therapy for managing chronic plaque psoriasis.

Low-dose ultraviolet-A1 phototherapy for lichen sclerosus et atrophicus.

Lichen sclerosus et atrophicus (LSA) is a chronic inflammatory skin disease characterized by white porcelain-like sclerotic skin lesions. It is most commonly seen in adult females and usually affects the genitoanal area. Extragenital LSA appears in 15-20% of cases. We report a 9-year-old Caucasian girl suffering from extragenital LSA that was resistant to conventional treatment. After 40 treatment sessions with low-dose UVA1 phototherapy, all skin lesions were resolved completely. Moreover, the improvement of skin status has been sustained during 6-months of follow-up. Long-wave UVA irradiation has been shown to induce intensively collagenase activity in human dermal fibroblasts. We suggest that UVA1 irradiation could be an effective treatment in patients suffering from extragenital LSA.

Acne inversa (alias hidradenitis suppurativa).

Acne inversa is a recurrent, suppurative disease manifested by abscesses, fistulas, and scarring. Once considered to be a disease of the apocrine glands, it is actually a defect of follicular epithelium. Thus, the term hidradenitis suppurativa is a misnomer and should be abandoned. In cases of familial acne inversa, the pattern of transmission and number of affected individuals are consistent with autosomal dominant inheritance. Aetiological factors such as hyperandrogenism, obesity, smoking and chemical irritants are not consistently associated with the affection. Bacterial involvement is not a primary event in acne inversa, but is secondary to the disease process. Potential complications include dermal contraction, local or systemic infection due to the spread of microorganisms, systemic amyloidosis, arthropathy, and squamous cell carcinoma. As spontaneous resolution is rare and progressive disability is the rule, early definitive surgical intervention is advisable. The surgical procedure of choice in most cases is wide local excision and healing by secondary intention. Pharmacotherapeutic drugs, including synthetic retinoids and antiandrogens, do not prevent progression of the disease.

Rosacea fulminans triggered by high-dose vitamins B6 and B12.

Rosacea fulminans is a rare variant of rosacea conglobata that occurs almost exclusively in women well past adolescence. The aetiology is unknown, although immunological, hormonal, and vascular factors have been suggested. We report the case of a 17-year-old girl with rosacea fulminans that was temporally associated with daily ingestion of high-dose vitamin B supplements. The onset was sudden and cosmetically disabling. The eruption improved when the vitamin supplement was discontinued and a therapeutic regimen including isotretinoin and methylprednisolone was introduced. It seems appropriate to consider the possibility of such a vitamin B-triggered condition in cases of subjects presenting new or exacerbating facial eruptions.

Generalized essential telangiectasia successfully treated with high-energy, long-pulse, frequency-doubled Nd:YAG laser.

BACKGROUND: Generalized essential telangiectasia is a rare cutaneous disorder with limited therapeutic options. OBJECTIVE: To evaluate the treatment with high-energy, high-frequency, long-pulse Nd:YAG laser in a patient with generalized essential telangiectasia. METHODS: A 62-year-old woman presented with a 20-year history of generalized essential telangiectasia. She had extensive telangiectasias primarily on the forearms and lower legs. The patient was treated with an Nd:YAG laser using a fluence of 20 J/cm2 and a pulse width of 20 msec. A sapphire water-cooled chill tip was used to cool the skin during laser therapy. The treatment was performed every 4 weeks over a period of 6 months. RESULTS: After six treatment sessions an almost complete clearance of telangiectatic lesions was observed. Side effects such as purpura, scarring, and pain did not occur. After a follow-up of 6 months, small telangiectatic lesions relapsed. CONCLUSION: Our results suggest that the Nd:YAG laser appears to be an effective and safe treatment option for generalized essential telangiectasia.

[Generalized pustulous psoriasis: A novel extraintestinal manifestation of Crohn's disease?].

A 66-year-old female patient suffering for 10 years from Crohn's disease firstly presented with a parallel outbreak of generalized pustulous psoriasis and Crohn's disease. A second synchronous exacerbation of both disorders occurred after discontinuation of treatment with prednisolone, methotrexate, and mesalazine. As to their pathogenetic concepts, both disease entities reveal similar immunologic alterations, i. e. comparable patterns of cytokines, chemokines, and inflammatory cells (T cells and neutrophils). Generalized pustulous psoriasis, therefore, might develop as hitherto undescribed, more rare extraintestinal manifestation of Crohn's disease.

Extensive partial unilateral lentiginosis.

Partial unilateral lentiginosis is a rare pigmentary disorder characterized by multiple lentigines on otherwise normal skin affecting one side of the body. The histology is that of a lentigo. We report two young men with an extensive form of partial unilateral lentiginosis affecting the skin. No systemic abnormalities could be identified.

Vitamin B(12) cream containing avocado oil in the therapy of plaque psoriasis.

BACKGROUND: There are already many effective topical therapies available for use in the treatment of chronic plaque psoriasis. Unfortunately, these treatments are often associated with a rather significant risk of undesirable effects. OBJECTIVE AND METHODS: In this randomized, prospective clinical trial, the effects of the vitamin D(3) analog calcipotriol were evaluated against those of a recently developed vitamin B(12) cream containing avocado oil in an intraindividual right/left-side comparison. The trial population consisted of 13 patients, 10 men and 3 women, with chronic plaque psoriasis. The observation period was 12 weeks; the effects of therapy were assessed on the basis of a PASI score adapted to the right/left-side comparison technique, the subjective evaluations of the investigator and patients and the results of 20-MHz sonography. RESULTS: There was a more rapid development of beneficial effects with the use of calcipotriol in the initial 8 weeks, although differences in effects were significant only at the time point of therapy week 8 (p < 0.05). After 12 weeks, neither the PASI score nor 20-MHz sonography showed significant differences between the two treatments. While the efficacy of the calcipotriol preparation reached a maximum in the first 4 weeks and then began to subside, the effects of the vitamin B(12) cream containing avocado oil remained at a constant level over the whole observation period. This would indicate that the vitamin B(12) preparation containing avocado oil may be suitable for use in long-term therapy, a hypothesis further supported by the fact that the investigator and the patients assessed the tolerability of the vitamin B(12) cream containing avocado oil as significantly better in comparison with that of calcipotriol. CONCLUSION: The results of this clinical trial provide evidence that the recently developed vitamin B(12) cream containing avocado oil has considerable potential as a well-tolerated, long-term topical therapy of psoriasis.

Interpapillary lines--the variable part of the human fingerprint.

The dermatoglyphic pattern of human palms and soles is individually unique and unchanging. Their prints show the course of the papillary ridges as papillary lines. Case reports and a few older studies of repeatedly taken fingerprints could, however, show that so-called interpapillary lines can develop between the papillary lines. The questions of this study were: How often do interpapillary lines occur? Can the differences between papillary and interpapillary ridges be quantified? Five-hundred and two ink prints of the palms and fingers from the archive of the Bochum Police Department were examined retrospectively. In 121 volunteers, the appearance of interpapillary lines was examined prospectively. From the later collective, the fingerprints of 13 people with interpapillary lines and nine people without were examined further by taking two silicon prints and measuring them with laser profilometry In 215 of the 502 ink prints (42.8%) interpapillary lines could be demonstrated. In those subjects younger than 20 years they were less frequently observed (34.1%) than in those above the age of 20 (51.8%). In all cases using laser profilometry the interpapillary lines could be related to a corresponding interpapillary ridge. The interpapillary ridge heights were 24.9 +/- 10.0 microm, significantly lower than the papillary ridges, which measured 59.0 +/- 19.2 microm. Interpapillary ridge widths were with 194.8 +/- 65.1 microm significantly narrower as compared to 435.5 +/- 57.4 microm in the papillary ridge. Those papillary ridges, between which interpapillary ridges were found, were significantly further apart from each other (610.5 +/- 78.9 microm) than those without interpapillary ridges (484.9 +/- 70.6 microm). During the course of a lifetime new ridges between the regular papillary ridges can develop or manifest. The fact that interpapillary lines are more frequently found on the right hands in men and those with increasing age is consistent with the theory that they correspond to degenerative changes and with sensitivity of touch.

Defined UV protection by apparel textiles.

This article was written to update information on test methods and standards for determining the UV protection of apparel textiles and on factors affecting UV protective properties of fabrics, from dermatological and textile technological viewpoints. Articles from dermatological and textile technological journals published from 1990 to 2001 were identified from MEDLINE, Excerpta Medica/EMBASE, World Textiles, and Textile Technology Digest. Peer-reviewed dermatological articles, textile technological research articles, and normative publications were selected. Independent data extraction was performed by several observers. Spectrophotometry is the preferred method for determining UV protection factor of textile materials. Various textile qualities affect the UV protection factor of a finished garment; important elements are the fabric porosity, type, color, weight, and thickness. The application of UV absorbers in the yarns significantly improves the UV protection factor of a garment. With wear and use, several factors can alter the UV protective properties of a textile, including stretch, wetness, and degradation due to laundering. Standards in the field exist in Australia and Great Britain, and organizations such as the European Standardization Commission in Europe and the American Association of Textile Chemists and Colorists and the American Society for Testing and Materials in the United States are also establishing standards for the determination and labeling of sun protective clothing. Various textile qualities and conditions of wear and use affect UV protective properties of apparel textiles. The use of UV blocking fabrics can provide excellent protection against the hazards of sunlight; this is especially true for garments manufactured as UV protective clothing.
