Intestinal permeability in patients with psoriasis.

A possible relationship between intestinal structure and function in the pathogenesis of psoriasis has recently brought about considerable interest. The purpose of this study was to evaluate the intestinal permeability in psoriatic patients by comparing it with healthy controls. 15 psoriatic patients and 15 healthy volunteers entered the study. Intestinal permeability was evaluated using the 51Cr-labeled EDTA absorption test. The 24-h urine excretion of 51Cr-EDTA from psoriatic patients was 2.46 +/- 0.81%. These results differed significantly from controls (1.95 +/- 0.36%; P less than 0.05). The difference in intestinal permeability between psoriatic patients and controls could be due to alterations in the small intestinal epithelium of psoriatics.

Citropten and bergapten suction blister fluid concentrations after solar product application in man.

Citropten (5,7-dimethoxycoumarin) and bergapten (5-methoxypsoralen) are present in bergamot oil which is used as a cosmetic tanning product. The aim of this study was to quantify, using HPLC, the amount of citropten and bergapten in the skin after the application of suntan products (emulsion and oil formulations). A suction blister technique, performed on the volar aspect of the forearm, permitted the collection of these two molecules. The blister fluid concentrations were 37 and 51 ng/ml (emulsion) and 26 and 23 ng/ml (oil), respectively, for citropten and bergapten. Our results show that the suction blister technique could be used for comparing transepidermal penetration of several compounds in vivo in man.

Use of anti-collagenase properties of doxycycline in treatment of alpha 1-antitrypsin deficiency panniculitis.

Three patients (a 71-year-old man and 2 women, 73 and 50 years, respectively) with recurrent panniculitis associated with alpha 1-antitrypsin deficiency are presented. Because the concept of chronic and exaggerated inflammatory response in the patients with alpha 1-antitrypsin deficiency is based on the theory of protease-antiprotease imbalance, we suggest that tetracyclines will alleviate this condition. Indeed, tetracyclines were found to inhibit collagenase activity. The total remission of the condition in these 3 patients underlines for the first time the effects of doxycycline on a condition characterized by deficiency of the antiprotease system. A review of the 23 reported cases of panniculitis associated with alpha 1-antitrypsin deficiency is presented in table form.

Cutaneous response to ultraviolet radiation-induced erythema in patients with type A dysplastic nevi.

The effects of an exposure to 6 times the minimal erythemal dose of ultraviolet radiation (UV) were studied using a visual scale and a tri-stimulus colorimeter in 12 sun-tolerant (skin types III and IV) patients with type A dysplastic nevus syndrome. In such individuals, the melanocytic system would be susceptible to neoplastic transformation induced by sunlight. The cutaneous response to a marked UVB erythema has been shown to identify the high-risk patients prone to light-induced skin cancer. This study indicates that the UVB cutaneous susceptibility of patients with type A dysplastic nevi is not significantly different from control subjects, in terms of erythemal and tanning response.

Localized scleroderma--response to 1,25-dihydroxyvitamin D3.

1,25-Dihydroxyvitamin D3 [1,25(OH)2 D3] may be an immunomodulatory drug which could have a role in controlling collagen deposition, and inducing reversal of fibrosis in some tissues. These observations prompted a study of the possible use of this hormone for the treatment of scleroderma. A 35-year-old woman, who had been suffering from localized scleroderma for 2 years, was given oral 1,25(OH)2 D3 for 6 months. The effects of the treatment were evaluated using clinical and physical measurements (skin thickness, extensibility properties of the skin). The evolution of the patient's condition during the 6-month therapy suggests that 1,25(OH)2 D3 is beneficial in localized scleroderma. The mechanisms of action are discussed in relation to the literature, which suggests both immunoregulatory and inhibitory effects on fibroblast growth.

[Malherbe's calcified epithelioma. Review of 80 cases].

The authors review a series of 80 Benign Calcifying Epitheliomas (BCE), and describe their main clinical, histologic and prognostic features, in comparison with those reported in the literature. BCE or pilomatrixoma arises from the hair matrix, and represents approximately 1/1,000 of all skin tumors. Statistically, it is found more frequently among children under the age of 10 years and anatomic locations are mostly located on the face and upper limb. These benign tumors tend to develop slowly in subcutaneous tissue, and their treatment consists of simple surgical excision, except in some instances, where signs of more aggressive behavior can be found.

[Solid facial edema associated with acne. Therapeutic efficacy of isotretinoin].

Persistent solid facial edema is considered a complication of acne vulgaris and has never been reported in the French literature. The authors report the cases of 2 patients, a 15-year old girl and a 20-year old man, who suffered from the condition for 1 and 2 years respectively. Clinical features and course are described. Treatment with isotretinoin in doses of 1 mg/kg/day administered during 6 and 8 months resulted in an 80 p. 100 and 50 p. 100 reduction of the edema respectively.

[Bazin's hydroa vacciniforme. General review of 2 cases].

Hydroa vacciniform is a rare recurrent photosensitive skin condition in which deep-seated vesicles appear on sun-exposed areas and heal leaving a depressed scar. Two cases are reported, with particular emphasis on phototesting investigations. Results of blood and urine porphyrin studies were normal and no systemic abnormalities were noted. Phototesting may be viewed as an important diagnostic aid, since the induction of lesions, clinically identical to hydroa vacciniform can occur following sequential exposure to UV-A, as was the case in one of our patients.

Nevoid basal cell carcinoma syndrome and acrokeratosis verruciformis. Occurrence of two rare inherited autosomal dominant conditions in the same patient.

An exceptional case of nevoid basal cell carcinoma (NBCC) syndrome occurring with acrokeratosis verruciformis (AKV) in the same patient is presented. The occurrence of two rare autosomal dominant diseases in a patient suggests that AKV might be one of the different dyskeratoses of the NBCC syndrome. The clinical aspect of the flesh-colored papules of AKV, indistinguishable from flat warts, might nevertheless prompt performance of a biopsy.

An original procedure for quantitation of cutaneous resorption of sebum.

This paper describes an original in vivo device to investigate and quantify cutaneous resorption of sebum. Such a phenomenon was explored using a photometric determination of residual skin surface lipids at different times following initial deposits of known amounts of artificial sebum on demarcated areas. The technique was carried out on seven healthy subjects. The chosen area was the anterior aspect of the forearm, owing to the very low sebum production of this site, less than the sensitivity of the measuring instrument. The kinetics of absorption of applied sebum turned out to be hyperbolic-like and well correlated with experimental results (r = 0.992), allowing a mathematical determination of the initial velocity of penetration of sebum into the skin, reaching 20 micrograms/cm2.min for a standard program including a mean initial deposit of 116 micrograms/cm2. This kinetics actually does not fit with a true percutaneous absorption of lipids but is compatible with that of absorption into the stratum corneum. It conveys the faster absorption of the latter for sebum, a well-known but not so far quantified property of stratum corneum. The model here described is not at once transposable to skin areas with high sebum production since, quite obviously in such zones, the stratum corneum is permanently saturated with skin surface lipids. Therefore, the conclusions of this work cannot be correlated with the regreasing parameters studied so far on the forehead, but they provide interesting data about the evolution of sebum following its output onto the skin surface and therefore a better understanding of sebaceous physiology.

Assessment of aging of the human skin by in vivo ultrasonic imaging.

The ultrasonic imaging technique that we have developed provides cross-sectional images of human skin in vivo with a resolution of about 80 microns axially (i.e., deep into the skin) and 250 microns lateral (parallel to the surface). In order to study aging skin, we obtained ultrasonic images from the mid-forearm (volar and dorsal sides) of 142 women. Ultrasonically, on the images, the dermis appears composed of two bands: a dark superficial one where the ultrasonic waves are propagated in a relatively homogeneous or non-echogenic medium, and a deeper one, which is lighter in color, suggesting a heterogeneous medium. Our results show that skin is thicker on the dorsal than on the volar forearm. In contrast to previously published results, skin thickness remains constant until the seventh decade of life, diminishing thereafter. The relative thickness of the two bands show marked variations with age: a progressive thickening of the dark band, from zero in infants to approximately 75% of total skin thickness in aged subjects, while the light band shows the inverse trend. Comparing the amplitude of the bands on the volar and dorsal forearm, the relative thickness of the dark band is larger on the dorsal (exposed) side and increases with age. These findings and the analysis of variously stained biopsies taken in some of our patients lead us to assign this dark band to a zone in the upper dermis where the collagen network is delicate, dense, and well organized. This is supported by some data in the literature. The thickness of this subepidermal non-echo-genic band appears to be a far more sensitive marker of skin aging at the dermal level than is the measurement of skin thickness.

Absence of minocycline in sebum?

The authors studied the bioavailability of minocycline in sebum and serum. Blood and sebum samples were collected weekly for 6 weeks from ten healthy volunteers taking 200 mg of minocycline every day for 4 weeks. Sebum was collected by direct extraction with petroleum ether from the forehead. After evaporation, sebum was weighed on a scale accurate to 10 micrograms. Determination of minocycline in serum and sebum was performed using a high performance liquid chromatography technique (HPLC), with a better detection limit at 352 nm than at 400 nm (20 ng/ml and 0.324 microgram/ml respectively). Our results contrast with other studies since no minocycline was detected in the sebum samples of treated subjects and microbiological assays of minocycline in sebum were also negative. In our opinion, the current hypothesis claiming that the effectiveness of minocycline in the treatment of acne vulgaris is based on sebaceous secretion should be reconsidered.

[Verneuil's disease. The diffuse inguino-perineo-gluteal and bilateral axillary form].

The authors report a case of suppurative hidradenitis of unusual severity affecting the axillary, inguinal, perianal, genital and sacral areas. The surgical treatment consisted initially of large excision of the axillary areas. A musculo-cutaneous latissimus dorsi island flap was then used to cover the defect. 45 days later, a complete excision, in only one operative session, of the inguinal, perianal, genital and sacral area was performed, associate durth colostomy. A mesh graft was performed 15 days after the excision. This procedure allowed an early scar.
