Combined tricuspid and pulmonic stenosis. Clinical, echocardiographic, hemodynamic, surgical, and pathological features.

A 30-year-old man with a history of a murmur since childhood had progressive ascites for 2 years. The patient was found to have severe stenoses of the pulmonic and tricuspid valves with a well-developed right ventricle. Impressive clinical improvement occurred after pulmonic valvotomy, infundibulectomy, and replacement of the stenotic tricuspid valve with a porcine xenograft. The clinical, echocardiographic, hemodynamic, surgical, and pathological features are presented.

Qualitative changes with age of proteoglycans of human lumbar discs.

A detailed study of the biochemistry of each of the lower lumbar intervertebral discs from 3 spines aged 8, 16, and 44 years has shown progressive changes down the spine in a number of biochemical parameters. These were most apparent in the 44-year-old spine. The chemical composition of proteoglycans of the nucleus pulposus and of its constituent proteoglycans differed from those of the corresponding annulus fibrosus of all three spines. The interaction of proteoglycans with collagen, as assessed by extractability, changed markedly with advancing age, while the molecular size of the proteoglycans from both regions decreased and their keratan sulphate content increased. These changes would be expected to affect the mechanical properties of the disc.

Nature of the stimulating cell in the syngeneic and the allogeneic mixed lymphocyte reaction in mice.

Thymus cells from CBA and BALB/c mice are stimulated by syngeneic peripheral lymphoid cells in a "one-way" mixed lymphocyte reaction. The stimulating cell appears to be a mature B cell. Spleen cells from neonatal mice and thymus cells or bone marrow cells from adult mice are not able to induce DNA synthesis in syngeneic thymus cells, although they stimulate significantly allogeneic thymocytes. The ability of peripheral B cells to serve as stimulating cell in a syngeneic reaction develops with the age of the animal. The marginal stimulation of syngeneic thymus cells when 90% pure peripheral T cells were used as stimulating cells indicated that T cells alone were ineffective in stimulating in syngeneic mixed lymphocyte reaction. However they stimulated effective allogeneic thymocytes. On a cell-to-cell basis, light density splenic lymphocytes stimulated both syngeneic and allogeneic thymocytes better than did more dense lymphocytes. The data obtained suggest that stimuli other than those responsible for allogeneic stimulation induce proliferation of syngeneic thymus cells under identical culture conditions.

Idiopathic hypercalciuria and hyperparathyroidism.

With provocative tests of parathyroid activity based on phosphate deprivation and the administration of chlorothiazide 8 out of 19 patients with idiopathic hypercalciuria developed hypercalcaemia. Five of the eight underwent exploration of the neck and four of them had parathyroid adenomas; the fifth patient had "normal" glands but responded satisfactorily to subtotal parathyroidectomy. One patient who failed to develop hypercalcaemia had primary chief cell hyperplasia of the parathyroids.
