Enchondroma in the hand retrospective study--recurrence cases.

This is a retrospective analysis of 21 patients with documented enchondroma of the hand. They were operated on between 1980 to 2000 with conventional curettage and bone grafting. The objective of this study is to analyse the chance of recurrence and evaluate the effectiveness of surgical treatment. One patient had recurrence after six years. Longer follow-up period is recommended as there is a possibility of recurrence even after five years. This study revealed that enchondroma of the hand is more common on the ulnar site.

Radius lengthening for the treatment of Bayne and Klug type II and type III radial longitudinal deficiency.

PURPOSE: We performed radius lengthening to treat radial deviation of the wrist in patients with Bayne and Klug type II and type III radial longitudinal deficiencies. The purpose of this investigation was to review our results of radius lengthening for radial longitudinal deficiency. METHODS: Beginning in 1991 radius lengthening was performed to treat 4 patients with radial longitudinal deficiency whose mean age at the initial lengthening was 16 months. The corrections for radial deviation of the wrists were performed simultaneously by soft-tissue distraction of the wrist. According to Bayne and Klug's classification 2 patients had type II and 2 patients had type III deficiencies. All but 1 patient had lengthening several times to correct the recurring discrepancy between the radius and the ulna. RESULTS: The corrections were achieved just after the lengthening but deformities recurred because of growth discrepancies between the radius and the ulna as the children grew. The radius was lengthened by a mean of 28 mm, with a mean length gain of 79%. The mean period in the fixator was 136 days. Two patients had both functionally and cosmetically acceptable correction after several lengthening procedures. In the other patient we abandoned this treatment method because of severe bone absorption at the distal end of the lengthened radius. One patient died of cardiac disease after one lengthening. CONCLUSIONS: Radius lengthening for Bayne and Klug type II and type III deficiencies may be accomplished successfully with the primary benefit of maintaining wrist and forearm motion. It is likely that at least 3 lengthenings may be required and this may need to be accompanied by a soft-tissue distraction at the ulnar carpal joint as well. TYPE OF STUDY/LEVEL OF EVIDENCE: Therapeutic, Level IV.

Three-dimensional CT imaging of flexor tendon ruptures in the hand and wrist.

OBJECTIVE: The purpose of this study is to determine the applicability of 3D CT imaging in the evaluation of flexor tendon rupture in the hand and wrist. DESIGN: Twenty-four digits in 22 patients (18 adults, 4 children) with a spectrum of finger flexion disturbances were investigated by a multidetector-row CT scanner, and diagnosed by 3D CT image with the volume rendering technique. The accuracy of the image diagnosis was confirmed operatively in 20 digits. RESULTS: 3D CT imaging gave a precise analysis in all adult cases. It clearly depicted the location of rupture and tendon stump. All of the 12 digits diagnosed from the images as tendon rupture were operated on, and the operational finding correlated well with the diagnosis based on the image. In the children, the image analysis was equivocal. CONCLUSIONS: 3D CT imaging can be useful when the diagnosis of flexor tendon rupture cannot be made based on trauma history and clinical examinations, and can be helpful in surgical planning.

Arteriovenous malformation of the ring finger. Pre- and postoperative evaluation using three-dimensional computed tomography angiography.

We experienced a case of arteriovenous malformation of the ring finger. The lesion was examined by three-dimensional computed tomography angiography with volume rendering and maximum intensity projection techniques pre- and postoperatively. The images demonstrated the location and extent of the lesion and were quite useful for surgical planning and the evaluation of operative results. Three-dimensional computed tomography angiography may be a valuable diagnostic tool even in the evaluation of peripheral vascular lesions.

Relationship between osteochondritis dissecans of the lateral femoral condyle and lateral menisci types.

The purpose of this study was to examine the relationship between osteochondritis dissecans (OCD) of the lateral femoral condyle and lateral menisci. From 1993 to 2002, 38 knees (28 patients) were diagnosed with OCD of the lateral femoral condyle. OCD locations were graded by the Cahill and Berg classification. The types of lateral menisci were classified by Watanabe's classification. The relationship between OCD of the lateral femoral condyle and lateral menisci was examined. On the anterior-posterior view, 25 OCDs were located in zone 4 and 13 in zone 5. The types of lateral menisci were complete discoid in 19 knees, incomplete discoid in 15, and normal in 4. Ten of the 19 complete discoid menisci were damaged. Complete discoid menisci without tears were found in OCDs located in zone 4; incomplete discoid menisci were found in OCDs located in zone 5. The authors found a relationship between the type of OCD and the state of the lateral meniscus.

Three-dimensional computed tomography imaging: its applicability in the evaluation of extensor tendons in the hand and wrist.

OBJECTIVE: The purpose of this study was to determine whether 3-dimensional (3D) computed tomography (CT) imaging with volume rendering can accurately evaluate extensor tendons in the hand and wrist. METHODS: Three-dimensional CT imaging was used to evaluate 14 cases. The location where the rupture was suspected was the wrist level in all cases. The accuracy of the image diagnosis was confirmed during surgery in 13 cases. RESULTS: Three-dimensional CT could identify all extensor tendons proximal to the metacarpophalangeal joint. In the cases with rupture, the location of the distal stump could be identified, whereas that of the proximal stump was uncertain. The tendon stump location confirmed during surgery correlated well with the images. CONCLUSIONS: This study revealed the usefulness of 3D CT imaging in defining extensor tendons proximal to the metacarpophalangeal joint. This method increased the accuracy and ease of diagnosis and was always useful in surgical planning and patient education.

Immediate implant-supported oral rehabilitation using a photocurable plastic skull model and laser welding. A technical note on the screw-retained type: Part 1.

PURPOSE: This article describes a new procedure for immediate implant-supported oral rehabilitation using a photocurable resin skull model and a laser-welding apparatus. MATERIALS AND METHODS: Preoperatively, the framework was fabricated on a photocurable resin skull model produced from a CT scan and individually designed guide template. The implants were immediately placed using the guide template; laser welding connected the components of framework. RESULTS: Despite the custom-made prosthesis, the total treatment from implant placement to superstructure placement can be completed within only 1 day. CONCLUSION: This procedure for immediate implant-supported oral rehabilitation using a photocurable resin skull model and a laser-welding apparatus may be useful for any implant system and patient.
