Fatty tissue within the maxillary sinus: a rare finding.

BACKGROUND: We report a rare case of fatty tissue within the maxillary sinus in a 21-year-old woman, with a history of several previous punctures of the maxillary sinus. CASE PRESENTATION: Clinical data of the patient was analysed retrospectively. The patient presented with symptoms of left-sided chronic maxillary sinusitis and had undergone several punctures of the left maxillary sinus 18 months earlier. Subsequent to one of the procedures an acute pain in the left orbit lasting a couple of days was noted. Left endoscopic transnasal antrotomy was performed. The maxillary sinus was filled with polypous, chronically inflamed mucous membrane. Upon its removal, the maxillary roof was identified as drawn downwards and covered with normal mucous membrane. Upon dissection of the membrane, adipose tissue filling the zygomatic recess of the sinus was identified and subsequently removed. The maxillary roof was unchanged. Histopathologic examination confirmed the material to be adipose tissue. No short or long term sequelae occurred. CONCLUSION: Adipose tissue can be found in the maxillary sinus most commonly when penetrating from surrounding locations. It is our hypothesis that in the reported patient it penetrated from the orbit to the maxillary sinus following puncture. It seems that a hole in the maxillary sinus roof, about 1 mm in diameter, caused by the needle, may have been a portal of entry for the adipose tissue into the maxillary sinus. The discussed case suggests particular care be taken in performing puncture of the maxillary sinus.

Functional evaluation of the vestibular organ in infants with risk factors for hearing loss occurring in the perinatal period.

BACKGROUND: Asphyxia at birth, low birth weight and low birth age constitute risk factors for inner ear damage in the perinatal period. Caloric stimulation is one of the most reliable diagnostic tools for vestibular testing in infants. Recording of vestibular-evoked myogenic potentials (VEMPs) evaluates the otolith organ and its pathways. Potentials reflecting the integrity of sacculo-collic reflex pathways are recorded on the surface of the neck muscles. The purpose of our study was to evaluate vestibular function in the discussed group of infants. MATERIAL/METHODS: 72 infants aged 3 months were examined: 40 healthy controls and 32 infants (17 girls, 15 boys) with risk factors for hearing loss occurring in the perinatal period. RESULTS: No reaction to caloric stimulation was elicited from 6 examined ears, no VEMPs were recorded from 6 ears, profound sensorineural hearing loss was diagnosed in 4 ears. The VEMP measurements did not differ between the groups. Profound sensorineural hearing loss occurred most frequently in infants with very low Apgar scores, responses to caloric stimulation were weakened in subjects with low Apgar scores. The response of the horizontal semicircular canals was more frequently abnormal than that of the otolith organs. CONCLUSIONS: The vestibular organ functions were frequently impaired in this group of infants, but the damage was rarely complete. Infants' vestibular organs should be routinely diagnosed.

[Sinonasal haemangiopericytoma of the nasal septum removed under endoscopic guidance].

Haemangiopericytomas originate in extravascular cells (pericytes). They are well-differentiated tumours with low potential for local recurrence or metastasis. The most common presenting symptoms are nasal obstruction and recurrent epistaxis. Differential diagnosis requires immunohistochemical staining. Treatment consists in wide surgical excision. Case report a 44-years old male turned up with symptoms of right-sided nasal obstruction and epistaxis. A reddish, tough tumour 10 mm in diameter was diagnosed, covered with unchanged mucous membrane with small ulceration 3 mm in diameter, growing in the nasal septum. The tumour was dissected from unchanged septal cartilage and removed under endoscopic guidance. Histopathological examination disclosed prolonged, multiangular cells, with no mitotic activity. Focal faint expression of actin was confirmed, while the tumour cells were negative for CD34, CD31, GFAP, EMA, CK and S-100. Endothelial cells were positive for CD31 and CD34. Epithelial cells were positive for EMA and CK. The results of differential diagnosis were consistent with the diagnosis of sinonasal haemongiopericytoma. The patient has been asymptomatic for 6 months. Conclusion. Transnasal endoscopic approach to the discussed sinonasal haemangiopericytoma located on the nasal septum allowed its optimal visualisation and removal with macroscopically sufficient margins.

[Olfactory neuroblastoma removed under endoscopic guidance].

INTRODUCTION: Olfactory neuroblastoma is a rare tumour associated with high rates of recurrence and mortality. Treatment includes surgery, radiotherapy, chemotherapy, or a combination. The ideal treatment modality has yet to be determined. MATERIAL AND METHODS: 46-years old male underwent bilateral endoscopic ethmoidectomy due to chronic sinusitis. During surgery, a 5 mm in diameter tumour was identified in the left superior nasal meatus. On histopathological examination the diagnosis of esthesioneuroblastoma was established and confirmed by immunohistochemistry. The tumour was classified according to Kadish as stage A, according to Biller as T1. MR scans performed two months later disclosed pathological tissues in the left ethmoid region. RESULTS: Multiple biopsies taken on subsequent frontosphenoethmoidectomy did not contain tumour cells. The patient declined radiotherapy. He has been asymptomatic over the 12 monthsí follow-up period. CONCLUSIONS: The exclusively surgical treatment proved effective due to early stage of the tumour and no evidence of infiltration of the frontal skull base. This strategy, however, requires frequent follow-up visits.

[Diffuse hyperplastic oncocytosis of the parotid gland].

An intracellular metabolic disturbance associated with mitochondriopathy, due to an age dependent metabolic defect, is suggested to cause metaplasia of salivary gland cells into oncocytes. The WHO classification of salivary gland diseases considers 3 oncocytic lesions: oncocytosis, oncocytoma and oncocytic carcinoma. In case of diffuse hyperplastic oncocytosis of the parotid gland, extensive metaplasia of acinic and ductal cells leads to transformation of nearly all cells of the gland into oncocytes, which is clinically characterised by swelling of the parotid gland, and is an extremely rare condition. A 82-year-old man (the eldest of all reported) presented with a 2-month history of increasing diffuse swelling of the left parotid gland, 6 cm in diameter. The mass was soft, painless to palpation and grew in the superficial lobe of the parotid gland. The patient underwent surgical removal of the lesion with wide margins of healthy tissue. Histopathological examination of the operative specimen disclosed oncocytosis of the salivary gland. The patient has been followed-up for 6 months. He is doing well without recurrence. Surgical removal of the diffuse hyperplastic oncocytosis of the parotid gland, with indications and extent of the resection like in benign tumors, was effective in the presented patient.

[Tinnitus in elderly patients].

Tinnitus is the perceived sensation of sound in the absence of acoustic stimulation. Spontaneous idiopathic tinnitus is a significant interdisciplinary therapeutic problem. In elderly patients it most frequently coexists with sensorineural hearing loss. The chief idea of tinnitus retraining therapy (TRT) in treatment of chronic tinnitus consists of following strategies: low level and broad band noise surroundings, diversion of the attention to other things and psychological counseling and therapy. The purpose of this study was to verify the benefits and ramifications of tinnitus retraining therapy in elderly patients suffering from chronic tinnitus with sensorineural hearing loss. Methods 30 subjects aged 65-90 years suffering from chronic tinnitus and sensorineural hearing loss were questioned about features of the tinnitus using a set of standardised questions. All of them were fitted with modern digital hearing aids and questioned about subjective hearing results after a month of follow-up. Main result 24 of the patients declared to have had considerable improvement in tolerance of the tinnitus. Main conclusion Fitting with hearing aids is an effective way of treatment in the majority of elderly patients with chronic tinnitus. The effectiveness of supplying elderly patients with hearing aids for tinnitus management depended in our group of patients on whether the patient had good speech understanding prior to fitting with hearing aids (speech discrimination score below and above 80%).

[ABR short latency negative responses in neonates].

Short latency negative responses (SLNRs) are waves recorded in auditory brainstem responses after stimulation with high intensity acoustic stimuli. They appear between waves I and III. Recordings of the waves evoked in high-risk of deafness infants with broad-band 2000-4000 Hz clicks were compared to those recorded after stimulation with 500 Hz tone bursts, considered to be most appropriate for acoustic stimulation of the saccule. Mean amplitude value of SLNRs recorded after click stimulation was 701.8 nV and after tone burst stimulation - 590.3 nV. SLNR latencies evoked with click and tone burst were 3.21 ms and 5.3 ms. Amplitudes obtained in infants with normal hearing threshold values did not differ from those obtained in subjects with sensorineural hearing loss. SLNRs were not obtained in case of 3 ears with sensorineural hearing loss which suggests serious inner ear damage. Click evoked SLNRs are easier to evaluate.

[Caloric stimulation in infants].

Caloric stimulation is one of few clinically proven tests assessing the function of each vestibule separately in neonates. Its results represent the continuity of vestibulo-ocular reflex, beginning in the lateral vestibular canal. Vestibular disorders are diagnosed in 20 to even 70% neonates with sensorineural hearing loss with the prevalence of individuals with profound and acquired deafness. 58 high risk of hearing defect infants were included in the study. Their age ranged from 3 to 6 months. A group of 27 healthy controls with negative history concerning sensorineural hearing loss risk factors was also examined. Caloric stimulation was performed according to Veits. External ear canal was irrigated with 20 ml of water at the temperature of 20 degrees C and eye movements were watched indirectly. In about 34% infants the nystagmic reaction to cold water was weaker than in normal controls. The reaction was most frequently impaired in infants with perinatal pathology, multiple congenital defects and aminoglycoside administration. Caloric stimulation in infants should be performed with cold water and the syringe used in the test should be fitted with a soft hose enabling irrigation of the interior part of the external ear canal.

[Bilateral progressive sensorineural hearing loss of unknown aetiology].

UNLABELLED: Bilateral progressive sensorineural hearing loss of unknown aetiology is diagnosed very rarely. Prognosis of hearing recovery is poor and results of treatment unsatisfactory. The disease occurs in teenagers and in elderly patients. CASE REPORT: Total hearing loss developed in 81-years old female patient over the period of 2 months, despite intensive pharmacological treatment. No reason for the condition was established and diagnosis of bilateral progressive sensorineural hearing loss of unknown aetiology was established. RESULT: Click evoked otoacoustic emissions and distortion products of otoacoustic emissions disclosed cochlear pathology. Click evoked auditory brainstem responses were absent. CONCLUSION: Audiological results obtained in the discussed patient and also relatively isolated onset of deafness as well as the severity and persistence of the hearing loss suggest that bilateral progressive sensorineural hearing loss of unknown aetiology is most probably related to prominent hypoperfusion of the internal auditory artery.

[Self-assessment of the effect of dysphagia on the quality of life in patients after partial laryngectomy for cancer initially located in the supraglottic area].

Dysphagia is frequent in patients after partial laryngectomy for cancer initially located in the supraglottic area. To ensure the best quality of life, establishing how the patient feels after treatment is necessary. Therefore, a self-administered questionnaire was designed to evaluate the dysphagia. 95 patients (75 male and 20 women) who were operated in ENT Department CMUJ between 1998-2004 participated in this study. The MDADI questionnaire in the Polish version was used. The subjective evaluation by the patient of dysphagia is directly related to the size of the tumor and the extent of the partial laryngectomy treatment. Patients, whose tumor was located in supraglottic-glottic area, had subtotal laryngectomy m. Miodoński. Their quality of life was significantly worse due to dysphagia than those with tumors located only in the supraglottic area. Swallowing dysfunctionality was significantly greater after the resection of the hyoid bone. Reconstruction of the base of a tongue by a graft of angiopedunculated submandibular gland diminished the difficulties in swallowing. Moreover, the longer the period after treatment, the higher the quality of life becomes because of the improvement in the swallowing functionality.

[Vestibular tests in infants after treatment with aminoglycosides].

UNLABELLED: Thorough evaluation of hearing and balance in infants after treatment with aminoglycosides is essential due to serious danger of the inner ear damage. Caloric stimulation is one of the most reliable diagnostic methods of vestibular testing in infants. Recording of vestibular-evoked myogenic potentials (VEMPs) makes evaluation of the otolith organ and its pathways possible. Potentials resulting from sacculo-collic reflexes are recorded on the surface of the neck muscles. AIM OF THE STUDY: The purpose of the study was to evaluate vestibular organ function in the discussed group of infants. MATERIAL AND METHODS: 70 infants aged 3 months, 40 healthy controls and 30 after treatment with aminoglycosides were examined. 24 of them had been treated with amikacin, 2 with gentamycin, 2 with amikacin and tobramycin, and 2 with amikacin and gentamycin. RESULTS: No reaction to caloric stimulation was elicited from 12 examined ears, no VEMPs were recorded from 6 ears, profound sensorineural hearing loss was diagnosed in case of 4 ears. Values of VEMP measures did not differ between the groups. Correlation was found between results of VEMPs and caloric test. CONCLUSIONS: The vestibular organ was frequently impaired in the discussed group of infants. The obtained results correlate with one another due to simultaneous impact of the risk factors on the maculae of the ampullar and otolith systems causing damage to both of them.

[Vestibular tests in infants with TORCH and after CNS infections].

Thorough evaluation of hearing and balance in infants with congenital and perinatal TORCH (ang. Toxoplasmosis, Other, Rubella, Cytomegalovirus, Herpes) infections and after infections of the central nervous system (CNS) is essential due to serious danger of inner ear damage. Caloric stimulation is one of the most reliable diagnostic methods of vestibular testing in infants. Recording of vestibular-evoked myogenic potentials (VEMPs) makes evaluation of the otolith organ and its pathways possible. Potentials resulting from sacculo-collic reflexes are recorded on the surface of the neck muscles. 76 infants aged 3 months, 40 healthy controls and 36 with risk factors of the inner ear damage: 25 with CMV infections, 7 after CNS system infections, 3 after congenital rubella and 1 with congenital Toxoplasmosis were included in the study. Values of VEMP measures did not differ between the groups. Complete lack of response to the stimuli was observed in 6 examined ears. Correlation was found between results of VEMPs, hearing threshold and caloric test. The vestibular organ was frequently impaired in the discussed group of infants. The obtained results correlate with one another due to simultaneous impact of the risk factors on the maculae of the ampullar and otolith systems causing damage to both of them.

[Functional assessment of posterior cordotomy in patients with bilateral vocal fold paralysis].

UNLABELLED: Surgical treatment of bilateral vocal fold paralysis must be undertaken if such a condition lasts 6-12 months or longer and causes dyspnoea. The purpose of the procedures is to assure proper airflow through the glottis and to preserve good voice and unimpaired swallowing. Modern endoscopic surgery of the glottis is performed with CO2 laser. The following paper presents results of laser posterior cordotomy performed in 17 patients aged 19-73 years suffering from bilateral vocal fold paralysis. RESULTS AND CONCLUSIONS: In 7 patients the range of vocal fold resection had to be expanded. 3 individuals developed post-operative granulation in subglottal region. Spirometry and electroglottography were performed before and after the operation to assess the patients' laryngeal functions: respiration and voice quality. The results indicate that laser cordectomy was effective in the discussed group of patients.

[Electroglottography in patients operated for thyroid gland malignancies].

Electroglottography (EGG) was performed in 9 patients with vocal fold paresis (VFP) arisen after thyroid gland surgery performed for malignant tumours. Clinical symptoms, EGG waveforms and % Irregularity were analysed and correlation between dysphonia and % Irregularity computed. % Irregularity proved to be an effective measure of vocal fold dysfunction, significantly correlated with dysphonia.

[Auditory brainstem responses to logon and tone burst 1000 Hz frequency stimuli in infants--own experience].

The use of frequency-specific stimuli in recording auditory brainstem responses (ABR) allows for hearing objectivity concerning the given frequency. Non-linear intensity growth (logon) and linear intensity growth (tone burst), 1000 Hz frequency short tone stimuli are most commonly used. Differences between hearing threshold values and other ABR measures in recordings obtained after stimulation with broad-band click and logon were analysed in a group of high-risk infants. 71 infants aged 1 to 9 months (36 girls and 35 boys) were included in the group. Broadband envelope shaped 2000-4000 Hz repeated 5 times per second clicks, acquisition and analyse time 12.5 ms and Gauss shaped 1000 Hz logon and tone bursts repeated 4 times per second, acquisition and analyse time 20.0 ms were used as stimuli. Click induced mean threshold values were the same after stimulation with logon, tone bursts and clicks. Wave 1, III and V latencies decreased with stimulus frequency. Mean amplitudes of waves I, III and V obtained after logon stimulation were slightly higher than after tone burst stimulation.

[Our experience in treatment of patients with inverted papilloma of lateral nasal wall].

Inverted papilloma is a benign neoplasm of uncertain etiology arising from the lateral nasal wall and middle meatus. The tumour is considered locally aggressive. In selected cases inverted papilloma can be associated with squamous cell carcinoma. Computed tomography and magnetic resonance imaging are widely used for diagnosis and preoperative workup to evaluate the degree of sinus involvement. Its tendency to invade adjacent paranasal structures and high recurrence rate causes controversy as to the optimal method of surgical removal. During the past decade, endoscopic surgical approaches have become accepted as standard of care in case of inverted papilloma. Continually increasing skill with the use of the endoscopes and a better understanding of the anatomy of the region have allowed surgeons to extend the applications of sinus endoscopic surgery. The following paper reports clinical outcome of treatment of 20 patients with inverted papilloma of the lateral nasal wall treated in ENT University Department in Kraków, provides review of current literature concerning the subject and discusses indications and contraindications of this method of treatment in comparison with traditional extranasal approaches in order to search for the optimal philosophy of sinus surgery for inverted papilloma.

[Language tests in electroglottography].

An Electroglottograph (Fourcin Laryngograph Processor) was used to determine, whether there was a significant difference in degree of Irregularity in vocal fold vibration (% Irregularity) or Contact Quotient (Qx) in the vocal fold vibratory cycle (Lx) between different phonatory tasks and between men and women. The most recent software package, 'Speech Studio', was used to analyse the data. The subjects were 24 healthy subjects, 16 females and 8 males, aged from 22 to 71. The tasks consisted of sustained vowels: a, o, u, short words: Ala, Ola, Ula and a short phrase in Polish, spoken at comfortable pitch and volume. The average age of females and males did not differ significantly on t-testing. The results showed there were no statistically significant differences between mean values of Qx between phonatory tasks on ANOVA testing. However, the differences between mean values of % Irregularity were significantly different between tasks. The % Irregularity obtained for the phrase, was significantly different from the values obtained for the sustained vowels and single words. The values for the latter two did not differ significantly. The difference between male and female Qx and % Irregularity values were not significant on t-testing except for % Irregularity for the word 'Ola', which was greater in men. Our findings show that for calculation of Qx either sustained vowel phonation, single words or phrases may be used. % Irregularity of vocal fold vibration is however task dependent.
