Clinical evaluation of effects of prazosin in patients with benign prostatic obstruction. A double-blind, multi-institutional, Paraprost-controlled study.

Prazosin, a selective alpha 1-adrenoceptor blocker, was compared with Paraprost in a double-blind, multi-institutional study in patients with benign prostatic hyperplasia. The trial encompassed 77 evaluable patients in the prazosin group and 79 in the Paraprost group. Both drugs significantly improved obstructive and irritative symptoms. Residual urine (volume or percentage of total urine) was reduced by both prazosin and Paraprost. Average and maximum flow rates increased significantly in the prazosin group while those were unchanged in the Paraprost group. Side effects occurred in 1 patient treated with prazosin (1.1%) and 3 patients with Paraprost (3.3%). Thus, we confirm the clinical value and minimal side effects of prazosin in the treatment of bladder outflow obstruction resulting from benign prostatic hyperplasia.

[A case of medullary sponge disease associated with horseshoe kidney].

A 60-year-old man visited our hospital with complaints of micturition pain and interruption of urinary stream. X-ray examinations were performed, showing no bladder and urethral stone. However, KUB revealed bilateral multiple renal stones. Excretory urography showed a horseshoe kidney and a concentration of the contrast medium in the ectatic tubules at the pyramidal apices. Calculi were also located in these ectatic tubules. Thus, we considered that this was a very rare case of medullary sponge disease associated with horseshoe kidney. So far there has been no case report of medullary sponge disease with horseshoe kidney in our country.

Structures of sugar chains of water-soluble glycoproteins in developing castor bean cotyledons.

The structures of sugar chains from the water-soluble glycoproteins in developing castor beans have been identified. The structural analyses were done by a fluorescence method combined with exoglycosidase digestions and 1H-NMR spectroscopy. The identified structures fell into two categories; one was an oligomannose-type, the other xylomannose-type or xylose-containing type. Among these oligosaccharides, Man3Fuc1Xyl1GlcNAc2 (M3FX; 38%) and Man6GlcNAc2 (M6B; 22%) were the major structures. The higher mannose-content oligosaccharides (Man8-7GlcNAc2) were only 4.1%, and the further-modified structures (GNM3FX, M2FX) than M3FX were 22% of the total.

[The tidal volume, arterial blood gas and functional residual capacity changes during negative extra-thoracic pressure ventilation and positive airway pressure ventilation].

Eight patients, of ASA physical status I or II soon after total knee replacement under general anesthesia, were studied to compare negative extra-thoracic pressure ventilation (NETPV) with positive airway pressure ventilation (PAPV). The measured parameters during the two ventilatory modes were tidal volume, arterial blood gas and functional residual capacity change (delta FRC). Tidal volume obtained during NETPV was 60 to 80% of that during PAPV at the same absolute values of peak pressure. delta FRC obtained during NETPV was 30 to 40% of that during PAPV at the same absolute values of end-expiratory pressure. A decrease in the esophageal pressure was 4 to 11cmH2O at an end-expiratory negative extra-thoracic pressure of -10 to -20 cmH2O. When the patients were ventilated with the same values of minute ventilation on NETPV and PAPV, there was no significant difference in blood gas values. These findings suggest that efficiency of NETPV is less than that of PAPV at the same absolute working pressure but pulmonary gas exchange of NETPV is almost equal to that of PAPV at the same minute ventilation in the normal lung.

[A case of urachal abscess].

A 26-year-old male with urachal abscess was demonstrated. Ultrasonography, CT and MRI revealed that there was a mass at the supravesical region, which was connected to the umbilicus. However, a differential diagnosis as to whether the mass was a tumor or an abscess was difficult from these examinations. Thus we performed an open biopsy. A pathological examination revealed no malignant change in the biopsy specimen. On the other hand, the pathological diagnosis of a follicular tumor located at the roof of the urinary bladder was TCC G2, but we considered that, this change was a reactive hyperplasia with dysplasia resulting from chronic inflammation of urachal abscess. Thus, the complete excision of urachal abscess and partial cystectomy were performed. The serial sections of the bladder wall revealed no malignant changes. We reviewed similar cases reported in our country and discussed various factors causing urachal abscess, especially related to the infectious root.

[A case of metastatic tumor of the spermatic cord from gastric carcinoma].

A case of metastatic tumor of right spermatic cord in a 54-year-old man is reported. He visited our department with the complaint of a painless mass in right inguinal lesion. The initial diagnosis was a right spermatic cord tumor, and then orchiectomy was performed. Since pathological findings showed a poorly differentiated adenocarcinoma, further examinations were done, postoperatively. An upper gastrointestinal series revealed the presence of gastric carcinoma. Thus, these results suggested that the spermatic cord tumor was a metastatic lesion from the gastric carcinoma. Forty eight cases of metastatic tumor of the spermatic cord have been reported in our country. In 26 of these cases, gastric carcinoma metastasized to the spermatic cord.

[Treatment of female acute cystitis with a single dose of cefminox].

Thirty six female patients with acute cystitis were treated with one shot intravenous injection of 1 g cefminox. Clinical efficacy was evaluated based on extent of subjective symptoms, pyuria and bacteriuria. Of 36 patients, an excellent efficacy was shown in 32 cases, moderate in 3 cases and poor in 1 case. This poor case was diagnosed as complicated urinary tract infection because of right vesicoureteral reflux. Therefore, the overall effectiveness rate of this treatment for acute uncomplicated cystitis in women was 100%. Single dose chemotherapy with an adequate antibiotic is considered a useful treatment for female acute cystitis.

[Evaluation of the performance of a negative extra-thoracic pressure ventilator (OKT-86)].

We evaluated the performance of a new negative extra-thoracic pressure ventilator (OKT-86, which was designed by the authors) applying it on ten patients with ASA Physical Status I (mean body weight: 54.4 +/- 12.1 kg) during anesthesia. Uneven distribution of negative extra-thoracic pressure was not observed within the chamber that a patient was wearing. Tidal volumes of more than 10ml.kg-1 were obtained at a respiratory rate of 10 or 16 breath.min-1 and a peak negative extra-thoracic pressure of -18 to -20 cmH2O. The values of delta FRC were 5.4 and 11.9 ml.kg-1 at continuous negative extra-thoracic pressure of -5 and -10 cmH2O, respectively, and these values were three times as those of the conventional cuirass ventilator. Consequently, sufficient tidal volumes and delta FRC were obtained using OKT-86.

[Bronchiectasis treated with negative extra-thoracic pressure ventilation].

A 25-year old man with 5-year history of bronchiectasis was admitted to the ICU complaining of severe shortness of breath. He had a respiratory rate of 40 to 50 breath/min. On 0.5 l/min of oxygen with nasal cannula, arterial blood pH was 7.39, Paco2 52.3 mmHg, Pao2 45.0 mmHg. Then, on 1 l/min of oxygen, Pao2 was unchanged, but Paco2 increased to 58 mmHg. As his consciousness was so clear, we applied to him the negative extra-thoracic pressure ventilator which was designed by the authors. Negative extra-thoracic pressure ventilation (NETPV) was maintained at a IMV rate of 30 breath/min, peak negative extra-thoracic pressure of -20 to -30 cmH2O, and an inspiratory/expiratory ratio of 1:2. During NETPV, his respiratory rate and oxygen consumption were decreased and Pao2 was increased compared with his spontaneous breathing. He made a recovery from dyspnea, especially, he was able to take a deep breath. When NETPV was applied to him, pulmonary artery and arterial catheterizations revealed that central venous pressure was slightly decreased, cardiac index unchanged or slightly decreased, heart rate, systemic blood pressure, and pulmonary arterial pressure unchanged compared with spontaneous breathing. The patient was able to read books and maintained communication in his voice with his family and the medical staff. After 3 days of the treatment with NETPV, a marked improvement was noted and the patient was discharged from the ICU. NETPV has the benefits as follows. First, it is very easy for both a patient and a doctor to assist his breathing because an endotracheal intubation is not necessary.(ABSTRACT TRUNCATED AT 250 WORDS)

Population dynamics of movable genetic elements in Japanese natural populations of Drosophila melanogaster. I. Microevolution of male recombination determinants in experimental populations.

Five natural populations of D. melanogaster in Japan were surveyed for male recombination frequency in terms of the quantities of the mean and variance after crossing with cn bw females. The experimental condition enabled P-M system hybrid dysgenesis to take place. The means were not so different from one another, but there was a vast variation in the variances between them. Using one of these populations, two additional experimental populations were started, and the male recombination frequencies were monitored for 30 generations. Both the means and variances decreased in value at 25 degrees C. The regression line was Y = -0.000030X + 0.001706 on generation for the mean values, and was Y = -0.000098X + 0.005068 for the standard deviations in the cage population. There was no correlation between the means or the standard deviations and the copy numbers of the members of P element family as a whole. The experimental results indicate that the complete P transposable elements still retained in population were decreased in frequency and replaced by its degenerative nonautonomous forms.

[Metastatic tumor of the spermatic cord from renal cell carcinoma].

The patient was a 54-year-old man with a hard nodule (25 x 15 mm) in his left scrotum. His left spermatic vein had been ligated 6 months earlier because of a varicocele. Preoperative diagnosis was left renal tumor with multiple lung metastasis. The intrascrotal nodule was considered to be a metastatic tumor from renal cell carcinoma. Left nephrectomy was performed and the metastatic lesion of spermatic cord was also removed. Pathological examination showed that the left renal tumor was renal cell carcinoma (clear cell type). The spermatic cord nodule was tumor embolus in dilated veins and consisted of the same clear cells as renal cell carcinoma. Judging from these results, in this case renal cell carcinoma retrogradely metastasized through the spermatic vein to the intrascrotal lesion.

Bladder carcinoma with Werner syndrome.

Werner syndrome is a rapid premature aging disease and is considered chromosomal instability syndrome, occasionally associated with malignancy. Urologic malignancy associated with this syndrome is unusual. Herein we report a case of Werner syndrome with urinary bladder carcinoma.

[Neurogenic bladder due to the anterior spinal artery syndrome: 2 case reports].

A 43-year-old woman with anterior spinal artery syndrome was admitted with complaints of tetraplegia and urinary retention. Her bladder had capacity of more than 500 ml and was inactive, with low voltage of external urethral sphincter. She was treated with distigmine bromide and intermittent catheterization. 50 days later, urodynamic study showed normal bladder and incomplete coordination of external sphincter. A 78-year-old man had chief complaints of urinary retention and paraplegia. A catheter was left indwell for 6 months. Then he was treated with intermittent catheterization, but urgency incontinence was still observed. We discussed the therapy of neurogenic bladder due to the anterior spinal artery syndrome.

[Clinical evaluation of cefmenoxime and cefsulodin in chronic complicated urinary tract infections].

To patients with chronic complicated urinary tract infections (UTI) which were in compliance to the UTI standard, cefmenoxime (CMX) was given singly or in combination with cefsulodin (CFS) and efficacies of the respective preparations were examined. Of 53 patients examined, 40 were given CMX alone while 13 were given a combined preparation of CMX and CFS. Results are summarized as follows. With respect to the general clinical effects, 7 patients given CMX alone were markedly improved, 21 improved and 12 unchanged indicating an efficacy rate of 70%. In case of the CMX-CFS combined administration, 2 patients were markedly improved, 7 improved and 4 unchanged giving an efficacy rate of 69%. As for the bacteriological effects, 38 of 54 strains isolated from the patients given CMX alone were eliminated, 4 decreased, 12 unchanged and 15 substituted. Of 16 strains isolated from the patients given the CMX-CFS combined preparation, 4 were eliminated, 2 unchanged and 3 substituted. Thus, incidence of bacterial substitution was low in the combination treatment. The combination of CMX and CFS was more inhibitory to P. aeruginosa than CMX alone. Regarding side-effects, drug eruption was observed in one of the 53 patients and slight elevation of GOT and GPT was recognized in another.
