[An additional short-term use of benzodiazepine anxiolytics at the initial stage of the treatment of panic disorders].

To increase efficacy and tolerability of antidepressants in patients with panic attacks (PA), a benzodiazepine drug has been used at the initial stage of the treatment. In this study, 32 patients with PA (illness duration from 1,5 to 8 years) received paroxetine, an antidepressant, in dosage 20 mg daily during 12 weeks and lorazepam, a benzodiazepine drug, in dosage 2,5 mg daily during the first 6 weeks of the exacerbation period. A general somatic and neurological examination as well as a quantitative assessment of intensity of mental disorders (anxiety etc.) using corresponding scales have been conducted. A statistically significant decrease of panic attack frequency, along with anxiety and depression, supports the advantages of the combined therapy.

[Coaxil treatment of primary headaches comorbid with depression].

Twenty patients with headaches and concomitant mild depression (the Beck scale mean score 13,6), aged 20-62 years (mean 40,3), illness duration 0,5-40 years, (mean 17,7), attack frequency 3-30 days, (mean 13,4), have been studied. Coaxil was prescribed in dosage of 12,5 mg daily during 6 weeks. Neurological examination and clinical evaluation using a number of scales, along with patients self-rating, was conducted. Twenty one patients have completed the treatment course. Coaxil therapy resulted in significant decrease of frequency and severity of headache attacks and improvement of the patient's general state.

[Analysis of group B streptococcal gene scaAB encoding the protein involved in adherence and aggregation].

The portion of group B streptococcal (GBS) sequence encoding the ABC transport system was cloned and sequenced. The genetic region under study included scaAB gene, which was capable of encoding for putative adherence and aggregation factor, and scaR, which was encoding for putative represser. After the insertional inactivation of scaAB gene on the GBS chromosome streptococci increased the tendency for aggregation. The level of GBS adherence to vaginal epithelium was also different after scaAB inactivation. GBS sea operon was compared to similar ABC encoding bacterial operons.

[The peculiarities of panic disorders determined by gender dimorphism].

In contrast to panic disorder (PD) epidemiology, gender-related etiological and clinical PD peculiarities are not studied well so far. Seventy-five PD patients (27 male, 48 female) with and without agoraphobia diagnosed according to ICD-10 criteria have been studied. No differences were found by gender for the key clinical signs: interrelations in parent family, age at debut, panic attacks frequency, combination with agoraphobia, social maladaptation and response to antidepressant therapy. Along with common features, we revealed gender differences in the clinical picture. In women, neuroendocrinal and motivation disorders preceded PD debut, panic attacks comprised more symptoms, with autonomic symptoms in the attack being subjectively considered as the key ones, autonomic function was more pronounced in between-attack period and depressive symptoms predominated in the clinical picture. Men considered psychic anxious symptoms (fear of death, forcing a patient to seek an outside assistance) as more prominent, and PD in men was more often combined with hypochondria and alcohol abuse.

[A role of depression in chronic dorsalgia: approaches to therapeutic correction].

An impact of depression on general clinical picture was studied in 20 patients with chronic dorsalgia, who were treated with coaxil. Along with complaints to prolonged back pains, patients featured by high depression and anxiety, sleep disturbances, autonomic dysfunction and marked decrease of quality of life. Psychometric tests revealed a presence of depressive symptoms in 90% of the cases. All patients were treated with coaxil in dosage 12.5 mg 3 times daily during 6 weeks. The treatment allowed a reduction of depression and anxiety symptoms, along with muscle dysfunction and pain syndrome. Positive dynamics of autonomic dysfunction, sleep disturbances and quality of life was observed already at the initial treatment stage. The results of the study confirm the fact that depression develops in patients with chronic dorsalgia, influencing negatively on the course of pain syndrome. Coaxil was shown to have high efficacy and safety in the treatment of depression caused by chronic back pain and positively influence on the main syndrome.

Determination of methylation site of DNA-methyltransferase NlaX by a hybrid method.

Using a new method based on a combination of bisulfite reaction, the repair enzyme uracil-DNA glycosylase, and synthetic oligodeoxyribonucleotides, the methylation site of DNA-methyltransferase NlaX (M.NlaX) from Neisseria lactamica was established to be the inner cytosine in the double-stranded pentanucleotide recognition sequence 5'-CCNGG-3' (where N = any nucleoside). 5-Methylcytosine (m5C) type modification by M-N1aX was confirmed by the use of oligonucleotide substrates that contain 5-fluoro-2'-deoxycytidine.

Determination of a non-methylated deoxycytidine residue in the recognition site of DNA-methyltransferases.

A method for determination of a non-methylated deoxycytidine (dC) residue in the recognition site of 5-cytosine DNA-methyltransferases is suggested. The method is based on treatment of methylated DNA by sodium bisulfite and successive reaction of the thus modified DNA with a repair enzyme, uracil-DNA glycosylase. This method was successfully applied to identify NlaX methyltransferase specificity.

[Optimizing the therapy of symptomatic partial epilepsy with sustained release carbamazepine].

This article summarizes a three-year experience of treating 34 patients with carbamazepine (CMZ) retard with partial epilepsy who had previously been treated with a standard form of CMZ. The following steps were taken in order to optimize the therapy: the usage of monotherapy with CMZ, an increase of CMZ dose if it was necessary, usage of CMZ retard-form. Increase of the dose (more than 1400 mg) resulted in adverse reactions in 6 patients, which regressed when the patients started to be treated with CMZ retard form. On the whole, optimization of therapy provided the following effects: 18 patients (53%) did not experience any seizures, in 11 patients (32%) a frequency of the seizures was considerably decreased (by 50% and more as compared with the baseline level). Individual analysis of the seizures frequency showed that in patients treated with CMZ retard-form the maximal decrease of this frequency was observed for sleep seizures. A frequency of seizures decreased by 63% at night and by 34% in day hours.

[Comparative evaluation of various materials used for hermetic sealing of the root channel opening after obturation].

The quality of hermetic sealing of the opening of an obturated root channel with various sealing materials (Bezline and Russian material Foscin and Belokor) was studied. Root channels were obturated using Endomethason, Forfenan, and gutta-percha pins with hermetic AN-PLUS. A total of 187 teeth of different groups were filled in patients aged 17-85 years. The quality of hermetic sealing of an obturated root channel was evaluated by x-ray examination and electrometrically directly after filling (in 10 min) and in remote periods (after 6, 12, and 24 months). The best results were attained with Bezline and gutta-percha. A combination of Bezline and Endomethason yielded satisfactory results.

[Adhesive strength between liner and filling in treatment of root canal: in vitro study].

Adhesive strength of combination of several liner and filling materials used for filling dental root channels was studied and the marginal permeability of root channel from the side of the crown was evaluated after filling with several pairs of pad and filling materials. Filling of root channels with gutta percha pins with hermetics did not deteriorate the strength of adhesion of BaseLine pad to the bottom of the cavity, while filling with Forfenane decreased the adhesion of this pad material 3-fold. Use of Belocor polycarboxylate cement for pad essentially deteriorated the adhesive strength of the pad irrespective of the filling material.

[Parietal theta rhythms in carbamazepine therapy of children with epilepsy].

UNLABELLED: Regional bioelectrical activity changes presenting as prolonged or periodic inhibition at the central parietal leads in theta rhythm (PT) are regarded in some studies as congenital predisposition to epilepsy. The study estimates a carbamazepine (CBM) therapy efficacy in children with epilepsy with- or without PT. To study the prognosis in CBM therapy, 119 patients, 63 with PT (group 1) and 56--without PT (group 2) have been retrospectively examined. Patients with the presence of generalized spike-and-wave activity (SWA) before CBM treatment have been excluded from the study. Despite the insignificant between-group differences of clinical and EEG-data before the therapy beginning, the CBM response was much worse in the group 1: a marked effect was found in 14 (22%) patients, in 41 (65%) the effect was mild or absent at all, and in 8 (13%) a disease course worsening was detected. In group 2 a good effect was in 42 (75%) patients, no effect was detected in 14 (25%) patients and none of the patients was worse. Correlations between SWA appearance and inadequate CBM action, as well as between improvement/worsening absence of EEG and favorable prognosis during therapy have been found. CONCLUSIONS: Carbamazepine efficacy in the children with epilepsy is significantly lower in the presence of parietal theta rhythm. PT is likely to be a liability factor for SWA appearance during CBM therapy.

[24-h ambulatory monitoring of cardiovascular indices in patients with panic disorders].

24-h monitoring of arterial pressure (AD) and heart rate (HR) was twice performed in 14 patients with panic attacks (in exacerbation and after a course of fluoxetine for 6 weeks). Panic attacks were followed by objective increase of both AD and HR in only 1/3 of the cases and by objective HR increase in 60% of the patients. Although patients with panic attacks were normothymics, hypertensive reactions can arise in them, during sleep for the most part. Alteration of HR variability in sleep-awakening cycle was the most paradoxical (excess HR variability in awakening state and decreased change during a sleep). These alterations of HR variability persisted after control of panic attacks with fluoxetine. This allows to consider HR variability as a factor which reflects predispositional peculiarities of autonomic system functioning in patients with panic disorders.

[Neurological masks of depression (effectiveness of tianeptine)].

40 patients with panic attacks, chronic algesic syndromes (chronic headaches of tension, cardialgias) entered the study. They also had depression of mild or moderate degree. 65% of the patients denied actively disorders of the mood; 80% were sure that they had a severe disease that was difficult to diagnose; most of them doubted the efficiency of therapy with psychotropic preparations. An analysis of the polymorphism of clinical symptomatology of the masked depression revealed in more than half of the patients some combinations of clinical syndromes, such as autonomic dysfunction, algesic manifestations, motivation and insomniac disorders. A structure of panic attacks was characterized by the prevalence of the autonomic symptoms as compared with the anxious ones. All the patients were treated with tianeptine (37.5 mg/day) during six weeks. The therapy was effective in 67.5% of the patients. The study revealed an equal efficiency of the drug in comorbid depressive, anxious and autonomic disorders as well as a good tolerance.

[Changes in evoked slow cerebral potential (contingent-negative variation) in epilepsy].

Contingent negative variation (CNV) was examined in 14 epileptic patients (5 with generalized and 9 with partial fits) both before and after 24 hours of sleep deprivation. Background CNV indices differed significantly by higher amplitudes of nearly all CNV components from the ones of healthy individuals. Amplitudes of postimperative negative waves (PINW) and general waves differed most of all. After sleep deprivation there was an PINW increase as well as the tendency to the late CNV component amplitude's increase. Elevation of CNV amplitudes after sleep deprivation was more pronounced in patients in which the deprivation provoked development of epileptic fits in comparison with patients which endured sleep deprivation rather favourably. The changes revealed in CNV components of the patients with epilepsy were considered in terms of disorders in regulatory mechanisms at the level of apical dendrites of cortical neurons' excitation.
