Regret among 547 Danish sterilized women.

To help identify potential regretters of female sterilization, the women sterilized at Frederiksborg County Hospital, Hørsholm, Denmark, from 1978 through 1982 were contacted by questionnaires, and their medical records were reviewed. Ninety-two per cent (547/594) responded. The median observation time was 50 months. Five per cent of the women (n = 28) regretted the sterilization. The risk of regretting the sterilization was significantly increased among women who at sterilization experienced marital disharmony (12.5% regretted), had a child less than one year old (14.7% regretted), had the sterilization performed in connection with another surgical procedure (16.1% regretted), were outside the social groupings (17.0% regretted), or had no paid work (10.0% regretted). No association between number of children at the time of sterilization, prior number of contraceptives used, abortion at sterilization, and later regret was found. Twenty-five per cent of the regretters had had psychiatric problems prior to the sterilization. The regretters were sterilized shortly after their request (median 1 month) compared to 3 months among non-regretters. The main reason for regret of sterilization was the desire for more children, independent of a change in marital status. Sequelae to the sterilization was a common complaint. Refertilization was requested by 1% of the sterilized women. The study suggests that the psychosocial situation should be carefully evaluated in women requesting sterilization. A time lag from request to the sterilization should be mandatory and a thorough pre-sterilization counseling including information about possible sequelae and alternative contraceptives should be given.

Thoracic injuries. A review of 93 cases.

A report is presented of 93 patients referred for specialist management of thoracic injuries in the 5-year period 1973-1978. Penetrating trauma to the thorax had been sustained by 18 patients and blunt trauma by 75. Falls and traffic accidents were the most common causes (32 and 35 cases). Rib fractures were the most frequent lesions, and 10 of these 67 patients had flail chest. Hemothorax and/or pneumothorax were found in four-fifths of the patients. The diaphragm was torn in five cases. Three had lesions of the heart and/or pericardium and one patient had a tracheal lesion. Closed pleural drainage was the most commonly used treatment. Thoracotomy was performed in 10 of the 93 patients. The mortality rate in the series (10%) agreed with findings from other, similar studies.

Direct vision internal urethrotomy.

During a five-year period, direct vision internal urethrotomy was used for the treatment of urethral strictures in 34 men. After the primary operation the patients were followed for an average period of 29 months (range 3-73 months). During this period 53% of the patients were found to have one or more recurrences. In most cases the new stricture was diagnosed during the first 12 months of observation. In a few cases, re-stricture was found as late as 36 months after the first treatment. Because of this tendency, a longer period of postoperative observation, including flow rate measurements at regular intervals is recommended. The operation is preferred for the treatment of strictures in the male urethra because it is easy to master and because of the low rate of complications.

Sexual and pituitary-testicular function in torture victims.

Seventeen men who previously had been subjected to torture were studied. Sexual dysfunction (reduced libido and erectile dysfunction) occurred in 29%. There was no relationship between presence of sexual disturbances and previous cranial and/or genital traumas, severity of torture, or length of imprisonment. Plasma concentrations of pituitary gonadotrophins, prolactin, and testosterone were normal in all subjects studied. The cause of sexual dysfunction in torture victims remains to be established.
