[Acoustic and vestibular analysers in patients with mine explosion trauma].

The aim of the study was examination of the acoustic and vestibular functions in patients with mine explosion trauma. Acoustic and vestibular functions were studied in 27 patients (16 females and 11 males aged 19-66 years, mean age 37.2 +/- 4.12) admitted to the ENT clinic of Moscow city hospital N 1. All the patients had suffered from explosion in closed space. General clinical and complex otoneurological examinations were made including gustatory and olfactory investigations, studies of the facial skin sensitivity, nasal mucosa, corneal reflex, soft palate motility, audiometry and vestibulometry. The findings of the tests suggested the following conclusions: mine explosion damages hearing by impairment of both sound conduction (85.7%) and sound perception (61.2%), the latter suffering primarily at the cochlear level; damage to the cranial nerves V and XIII occurs at the level of the brain stem; mixed affection of the vestibular analyzer is seen primarily at the level of stem nuclei.

[Current radiotherapy for extrapulmonary tuberculosis].

Radiation studies are prominent in the diagnosis of extrapulmonary tuberculosis. X-ray study preserves its basic value. Its-based concepts of the pathology of osteoarticular, urogenital, lymphabdominal tuberculosis largely allow for analyzing the images obtained by new medical visualization techniques, such as X-ray tomography, computed tomography, magnetic resonance imaging, ultrasonography, and radionuclide studies. Each method has its merits and demerits and is used by a certain algorithm at different sites of extrapulmonary tuberculosis.

[Radiologic diagnosis of osteoarticular tuberculosis and borderline skeletal diseases].

A total of 3305 ill children and adults treated for skeletal diseases and lesions underwent primary and follow-up radiation studies. Patients with spinal diseases amounted to 72%, of them 58% were found to have neurological diseases, 28% had articular diseases. The examination of patients involved X-ray tomography, contrast myelotomography, computed tomography, and magnetic resonance imaging. Surgical and laboratory verifications of the diagnosis were made in 53% of adults and 78.5% of children. It is shown that there are difficulties in diagnosing and differentiating osteoarticular tuberculosis at present and X-ray tomography and contrast myelography are essential in diagnosing spinal diseases. Indications for computed tomography and magnetic resonance imaging, osteoscintigraphy and the sequence of their use in the diagnosis of diseases were defined. The paper emphasizes that an osteoarticulogist's assessment of complex data of radiation diagnosis is very important.

[Magnetic resonance tomography in the diagnosis of tuberculous spondylitis in children].

The experience of using magnetic++-resonance tomography to diagnose tuberculous spondylitis in 15 children admitted to a hospital for surgical treatment is described. The method proved to be highly effective for evaluating the condition of the spine and spinal medulla and for establishment of early diagnosis of the attendant neurotrophic changes in healthy vertebrae. The examination findings serve to determine the volume of surgical intervention, character of postoperative restorative management and allow the prognosis of treatment outcome. In this respect, magnetic++-resonance tomography is a valuable supplementary diagnostic method.

[The effect of reconstructive bone surgery in tubercular spondylitis on the development of the spinal column in young children].

Long-term results of reconstructive osteoplastic operations on the vertebral column performed for tuberculous spondylitis in 116 children, aged from 1 year and 4 months to 6 years, are presented. All of them were practically cured, thus having a complete recovery of their vertebral column supporting function; in 65.5% of these patients, there was a reduction of a cyphotic deformity with age; 18.7% of the operated persons, including merely 13% of the disabled in their childhood, had resumed their work and 23% of the patients were taken off the dispensary record upon their recovery. Osteoplastic reconstructions for tuberculous spondylitis performed in early childhood failed to affect a future growth of the vertebral column. Besides, they ensure a complete recovery of a child and should have the lead in the whole system of the up-to-date management of spondylitis in children.
