Is caffeine involved in the pathogenesis of combat-stress reaction?

The role of caffeine in the pathogenesis of combat-stress reaction is discussed in light of the expanding literature on caffeine's influence on stress reactions in animals and in humans, and in regard to the two clinical entities concerning caffeine: caffeinism and caffeine withdrawal. It is proposed that caffeine is a contributing factor to the development of combat-stress reaction and that the use of decaffeinated coffee in military settings might reduce the prevalence of the various anxiety reactions, including combat-stress reaction.

An outbreak of penicillin-resistant Neisseria gonorrhoea in southern Israel.

Since the early 1980s the incidence of gonorrhea has been declining worldwide, as well as in Israel. This decline has been attributed to the fear of AIDS. We report an outbreak of penicillin-resistant Neisseria gonorrhoea that occurred in the last quarter of 1988 and throughout 1989 in southern Israel. During 1987 and the first three-quarters of 1988 only 13 new cases were diagnosed, all penicillin sensitive. During the epidemic, 94 new cases were diagnosed, 41% of them penicillin resistant. In March 1989, ceftriaxone was substituted for penicillin for the treatment of gonococcal urethritis. The number of cases of penicillin-resistant gonorrhea dropped dramatically. It is concluded that despite the AIDS panic, outbreaks of gonorrhea can still be encountered, as demonstrated in this study.

Tuberculosis in Israel: a 10-year survey of an immigrant society.

Over a 10-year period (1978-87), 279 tuberculosis (TB) patients were diagnosed at the Soroka Medical Center. Of these, 48% were Ethiopian Jews, 28% Bedouin Arabs, and 24% were Jews of other origins. The Bedouin and Ethiopian patients were younger and had fewer concomitant diseases. The Ethiopian patients were hospitalized longer than other patients. Ethiopian and Bedouin patients were less compliant with therapy. There were 187 cases of pulmonary TB (67%), and 92 cases of extrapulmonary TB (33%). The majority of the Bedouin and Ethiopian patients had pulmonary tuberculosis, while extrapulmonary TB was more frequent among the rest of the patients. It is concluded that TB in the Negev reflects the diversity of its population, with characteristics of both a developing and a developed country.

Extrapulmonary tuberculosis in an immigrant society: clinical and demographic aspects of 92 cases.

Extrapulmonary tuberculosis accounted for 33% of all new cases of tuberculosis identified at the Soroka Medical Center in Beer Sheva, Israel, during a 10-year period. The most common types of extrapulmonary infection diagnosed were genitourinary tuberculosis (54% of patients), lymphadenitis (13%), pleural tuberculosis (9%), and tuberculosis of bones and joints (8%). Of 92 patients, 51% were Jews of Ethiopian origin, 29% were Jews of non-Ethiopian origin, and 20% were Bedouins. Thus, extrapulmonary tuberculosis remains a significant problem for Israel's heterogeneous population.

Traumatic war experiences affect outcome in a case of Tourette's syndrome.

BACKGROUND: Stress has been associated with the course of Tourette's syndrome (TS). METHODS: The case report presented aims to emphasize the effect a severe traumatic experience has on the expression of TS. RESULTS: Following an intense traumatic experience, combat-related, a relatively mild case of TS became intractable. CONCLUSION: The combination of both of these severe, sometimes refractory, disorders is rare, and poses a therapeutic challenge.

Tuberculosis cases in the Negev 1978-1987: ethnicity, sex, and age.

BACKGROUND: The incidence of tuberculosis (TB) in Sub-Sahara Africa is considered to be one of the highest in the world. During the past decade thousands of Jewish refugees from Ethiopia were settled in the Negev and might constitute a potential reservoir of infection for the indigenous populations. This study provides some baseline information about TB in the Negev just prior to and after an Ethiopian immigration peak. METHODS: The files of every case of TB diagnosed during the decade 1978-1987 at Soroka Medical Center were reviewed and each diagnosis was validated by rigorous clinical and microbiological criteria. The age, gender, and ethnic background of each case were recorded, and approximate population denominators were estimated from Ministry of Health and Census data. Annual and decade incidence rates were then calculated for the different demographic categories. RESULTS: 279 cases of TB were verified. The main 10-year incidence rate per 10,000 Israeli Jews was 0.28; for the Negev Beduins it was 1.52; for the Ethiopian Jews, 91.9. In the Jewish population, cases among males (59) far exceeded those among females (7), but the reverse was observed, both among the Beduins (47 female and 31 male cases) and the Ethiopian immigrants (79 female and 56 male cases). In all three groups TB incidence increased with age, ranging from 0.03 per 10,000 for young non-Ethiopian Jews to a remarkable 623.8 per 10,000 for elderly Ethiopian Jews. CONCLUSIONS: The results of this study indicate the existence of a potentially large TB reservoir in the Negev. Health workers must be alerted to the importance of continued case finding, effective case management, and the control of infection transmission. The unique integration of the Negev Health Delivery System should help monitor intervention strategies.

Clozapine for the treatment of agitated-depressed patients with cognitive impairment: a report of three cases.

Clozapine, an atypical antipsychotic, is mainly approved for the treatment of resistant schizophrenia. However, a substantial body of evidence suggests that it might be useful in other psychiatric indications, such as treatment-resistant depression, Parkinson's disease, and dementia. In this report we present the cases of three patients hospitalized at the psychiatric division of the Sheba Medical Center, diagnosed with major depressive disorder with cognitive impairment, whose presenting symptom was agitation. These patients were nonresponders to various treatment modalities. However, treatment with clozapine brought about a favorable response.
