A sensitive fluorometric assay for serum monoamine oxidase with kynuramine as substrate.

The detailed procedure of a new sensitive fluorometric assay for human serum monoamine oxidase (MAO) with kynuramine as substrate is described. The data on its reproducibility, stability, correlation with another method, and serum MAO levels for 150 healthy subjects and 205 patients with various diseases, are presented to support the usefulness of this method. Since our method is much simpler and more sensitive than other methods conventionally used, we can recommend it for routine clinical investigation.

Detection of shunt flow in total anomalous pulmonary venous connection using 2D-Doppler echocardiography.

Two-D-Doppler echocardiography was used to diagnose two cases of total anomalous pulmonary venous connection (TAPVC). The first case presented the supracardiac type, the second the infracardiac type. Blood flow imagings produced by 2D-Doppler echocardiography revealed shunt flow in the common pulmonary veins (CPV) and the vertical, innominate, and portal veins. The shunt flow in the vertical vein was laminar, though the shunt flow in the inflow tract of the right atrium was turbulent. Two dimensional Doppler echocardiography is very useful in the detection shunt flow in TAPVC.

[Clinicopathological studies on disseminated carcinomatosis of the bone marrow occurring through metastasis of gastric carcinoma].

Five cases of gastric carcinoma (one woman) were investigated. The average age of the patients was 44 and the clinical manifestation were lumbago, melena and fatigue. Two cases were Borrmann III, and other were Borrmann IV, IIc and IIc advanced, respectively. Three were signet-ring cell carcinoma and the other two were moderately differentiated and well-differentiated adenocarcinoma, respectively. All patients showed leukoerythroblastosis. Bone marrow specimens revealed myelofibrosis in three cases, while adenocarcinoma cells were also found in three. MHA or DIC was found in three patients, all of whom died of bleeding tendency. Mean survival time was 2.6 months. Thus, bone marrow carcinomatosis proved to be very severe.

[Prediction of coronary lesions by two-dimensional echocardiography in patients with healed myocardial infarction].

Regional left ventricular wall motion abnormalities were assessed by two-dimensional echocardiography (2-DE) in 66 patients with healed myocardial infarction (MI) and the results were compared with those of coronary angiography which was performed at nearly the same time as 2-DE. The left ventricular wall was divided into 14 segments and asynergy was assessed in each segment. To compare the severity of segmental asynergy with coronary artery lesions, relation between segments and coronary artery perfusion was assumed as follows: the anterior wall, anterior septum and apex corresponded to the anterior descending artery (LAD); the lateral and inferior walls corresponded to the left circumflex artery (LCX); and the posterior septum and inferior wall corresponded to the right coronary artery (RCA). One vessel disease: In all 24 patients with MI induced by LAD lesions, regional asynergy was identified in the anterior wall, anterior septum and apex. All patients had advanced asynergy (severe hypokinesis, akinesis or dyskinesis) except one with subendocardial infarction. Advanced asynergy was identified in the other segments including the lateral wall (one patient) and the posterior septum (11 patients), but such asynergy was not associated with that of the inferior wall. In four patients with MI induced by RCA lesions, regional asynergy appeared in localized segments of the posterior septum and inferior wall, and the asynergy was severe in all patients except one in whom collaterals were well-developed. In four patients with MI induced by LCX lesions, asynergy was observed in both the lateral and inferior walls. In only one patient, mild asynergy was identified in the apex. Multi-vessel disease: In both groups with double vessel (16 patients) and triple vessel (16 patients) disease, asynergy was recognized in the affected areas of the major coronary artery which was considered responsible for the infarction, irrespective of the severity of the coronary stenosis. Also, there was no significant correlation between the severity of coronary artery stenosis and segmental wall motion abnormalities in non-infarcted areas. No significant correlation was observed between the severity of regional wall motion abnormalities in the infarcted areas and the degree of development of collaterals. In patients with MI induced by LAD lesions, more extensive asynergy in the anterior wall and anterior septum at the basal portion was observed in the patient group with proximal stenosis than in the group with distal stenosis.(ABSTRACT TRUNCATED AT 400 WORDS)

[A case of primary adenosquamous carcinoma of the stomach preoperatively diagnosed by endoscopic biopsy].

A 53-year-old complained of upper abdominal pain. A diagnosis of Borrmann III type gastric cancer in the posterior part of the corpus region was made by X-ray and endoscopy, and primary adenosquamous carcinoma was preoperatively diagnosed by endoscopic biopsy. Subtotal gastrectomy was performed. The part of the adenocarcinoma adjacent to squamous-cell carcinoma was poorly differentiated. Adenosquamous carcinoma of the stomach is rare. Furthermore, preoperative diagnosis by endoscopic biopsy is extremely unusual, having been reported in 7 cases in Japan including ours. And in 5 cases out of 6, region of the adenocarcinoma was the poorly differentiated type.

Post-proline endopeptidase in human placenta.

Post-proline endopeptidase (EC 3.4.21.26) was found in human placenta, purified 3390-fold from it and briefly characterized. The post-proline endopeptidase could be completely separated from dipeptidyl peptidase IV (EC 3.4.14.5) by hydrophobic phenyl-Sepharose chromatography. The pH optimum of the enzyme was 6.7. The Km values for 7-(Succinyl-Gly-Pro)-4- methylcoumarinamide was 1.0 mM. The molecular weight of this enzyme was estimated to be 140 000 by gel filtration and 67 000 by dodecyl sulfate gel electrophoresis, indicating its dimeric structure. Human placental post-proline endopeptidase was suggested to be a thiol proteinase by inhibition studies.

3-(p-hydroxyphenyl)propionic acid as a new fluorogenic reagent for amine oxidase assays.

A detailed procedure of a new and extremely sensitive fluorometric assay for amine oxidases is presented. Hydrogen peroxide, produced by the oxidase reaction, reacted with 3-(p-hydroxyphenyl)propionic acid in the presence of peroxidase to yield a fluorescent compound by which enzyme activity could be determined. The enzyme reaction was terminated by NaOH solution, which increased the fluorescence intensity three- to fivefold. The detection limit thus obtained was as little as 0.02 nmol. The alkalinization also contributed to stopping the enzyme reaction and to the clarification of assay mixtures containing turbid enzyme preparations.

Nails as useful materials for detection of methamphetamine or amphetamine abuse.

Methamphetamine and amphetamine could be demonstrated in nail clippings obtained from methamphetamine users by sensitive gas chromatography/chemical ionization mass spectrometry with N-methylbenzylamine as an internal standard. The methamphetamine levels in fingernails were comparable to those in hair. Both stimulants were more concentrated in toenails than in fingernails. The detection of methamphetamine and amphetamine in nails provides an alternative information to that in hair on their past abuse.

[A case of multicentric early carcinoma of the stomach--preoperative diagnosis].

A 59-year-old man was admitted complaining of substernal pain and anorexia. Upper GI series revealed a benign gastric ulcer in the posterior wall, and gastrofiberscopy was done. Besides the benign lesion, two early carcinomas of type IIc were found. Biopsy specimens of both lesions showed malignancy and gastrectomy was performed. Histological investigation revealed poorly differentiated adenocarcinoma in the anterior wall and well differentiated tubular adenocarcinoma in the posterior wall. Both lesions were absolutely independent and isolated. In most instances, multicentric or multiple early gastric carcinomas manifest similar histological features, but in our case, the lesions were of different types.

Detection of methamphetamine and amphetamine in a single human hair by gas chromatography/chemical ionization mass spectrometry.

A detailed procedure of an extremely sensitive method for quantitation of methamphetamine and amphetamine in human hair by gas chromatography (GC)/chemical ionization (CI) mass spectrometry (MS) is presented. N-methylbenzylamine was used as an internal standard. The samples, after extraction with an organic solvent, were derivatized with trifluoroacetic anhydride before the GC/MS analysis. Quantitation was made with quasi-molecular ions of the derivatives by selected ion monitoring in the CI mode. The detection limit was about 10 pg in an injected volume. The high sensitivity enabled us to measure both stimulants in a single human hair in actual cases.

Kinetic analysis of anaerobic metabolism in rats during acute cyanide poisoning.

In order to investigate the changes in energy metabolism during acute anoxia, blood levels of various metabolites were analysed in cyanide-poisoned rats. After intraperitoneal injection of a sublethal dose of potassium cyanide (5 mg/kg), blood samples were obtained by cervical dislocation at intervals of 5 min until 30 min. Lactate and lactate/pyruvate ratio (L/P) in plasma concomitantly changed with cyanide; increased rapidly at 5 min, remained fairly constant until 20 min and then began to decrease at 25 min. In contrast, the products of ATP degradation, oxypurines and inorganic phosphate (Pi), increased gradually until 25 min and then began to decrease at 30 min. Allantoin in plasma scarcely increased throughout the experiments. The results indicate that the rapid activation of anaerobic ATP formation by glycolysis was followed by the increase in ATP degradation in cyanide-poisoned rats, Thus, increase in plasma oxypurines could be regarded as an indicator for severe anoxic states in tissues with massive ATP degradation.

Determination of beta-phenylethylamine as its isothiocyanate derivative in biological samples by gas chromatography mass spectrometry.

A detailed procedure of a gas chromatographic mass spectrometric assay for beta-phenylethylamine in biological samples, after its reaction with carbon disulphide to form the isothiocyanate derivative, is presented. Our method has advantages over the previous methods with the pentafluoropropionic derivative of beta-phenylethylamine in that the isothiocyanate derivative of beta-phenylethylamine is much more stable than the pentafluoropropionic derivative and that the background in selected ion monitoring is very low. Using the present method, the levels of beta-phenylethylamine in human urine, untreated and pargyline-treated rat brain were found to be 15.3 micrograms 24 h-1, 1.4 and 160 ng g-1 wet weight, respectively.

A new reagent for the rapid determination of total hemoglobin as hemiglobincyanide in blood containing carboxyhemoglobin.

Kinetics of conversion of HbCO to HiCN with various K3Fe(CN)6-KCN reagents was examined photometrically. Complete conversion of HbCO to HiCN required about 8 hr, 150 min, and 50 min with Drabkin's reagent, that of van Kampen and Zijlstra, and that of Taylor and Miller, respectively. The absorbance at 540 nm was stabilized practically in 5 min even in CO-saturated blood when a reagent containing a 1600-fold excess of K3Fe(CN)6 to Hb iron was used. A slight increase in the absorbance was observed around 504 nm in the HiCN solution produced with the reagent. The absorbance at 540 nm in the solution, however, was stable until at least 120 min after mixing. From these findings, a reagent containing a 1600-fold excess of K3Fe(CN)6 to Hb iron and an incubation time of 5 min were used for the complete conversion of HbCO to HiCN. The stabilization of the absorbance at 540 nm was often prolonged to 10 min when the reagent was stored over 2 weeks. The reagent was compared with two other reagents, using 20 blood samples containing various amounts of HbCO, and proved to be suitable for the determination of total Hb in blood containing elevated HbCO as well as in ordinary blood.
