Total condylar knee arthroplasty. A report of 2-year follow-up on 247 cases.

Two hundred and forty-seven consecutive total knee replacements using the Insall-Burstein standard total condylar knee system with 2 years' follow-up were studied. Of the 238 available to follow-up, two were reoperated during the period, owing to infection and traumatic loosening of a tibia component respectively. Seventy-nine patients were operated for rheumatoid arthritis and 94 for osteoarthritis. In assessment of the total therapeutic result, registration of pain, walking ability, range of motion, muscle strength, flexion deformity, valgus-varus deformities, instability, and use of walking aids were included, according to a rating system. Excellent or good results were found in 91.5%; 6.5% were fair, and 2% were poor. There were two cases of infection; one resulted in removal of the prosthesis and an arthrodesis was done. There were four suspected aseptic loosenings, three cases of transient peroneal nerve palsies, and three patients who developed reflex dystrophy.

Presenting symptoms and treatment delay in osteosarcoma and Ewing's sarcoma.

Presenting symptoms and treatment delay were investigated in a series of 84 osteosarcomas and 40 Ewing's sarcomas. The treatment delay averaged 6.4 months for osteosarcomas and 9.6 months for Ewing's sarcomas. In both types the delay was relatively short in patients with constant pain and swelling. The prognosis was about the same whether the delay was short or long. This may be explained by the more severe nature of the presenting symptoms, in particular the constant pain and swelling, in aggressive tumours, a factor that also tends to mask other relevant prognostic parameters. It is concluded that the presenting symptoms and the treatment delay constitute important pathophysiologic factors which should be given more attention when analysing material from bone sarcomas.

Histopathological grading in soft-tissue tumours. Relation to survival in 261 surgically treated patients.

A system for histopathological grading of malignancy in soft-tissue sarcoma is described in detail, and the importance of the grade of tumour in predicting survival is demonstrated in a consecutive series of 261 surgically treated patients with sarcoma of the somatic soft tissue. Mitosis index is the main discriminating criterion. Delay in fixation, for instance in large specimens, may cause an artificially low mitosis index calling for the need of other criteria, too, reflecting grade of malignancy: cellularity, anaplasia, number of pycnotic and/or fragmented nuclei. There is a significant difference in survival between the three grades, 10 years survival with surgical treatment alone is 97%, 57% and 29% for grades 1, 2 and 3 respectively.

The subchondral bone of the proximal tibial epiphysis in osteoarthritis of the knee.

The trabecular bone of the proximal end of the tibia was assessed as an endoprosthesis-bearing structure. The mass and mineral content as well as the activity of subchondral trabecular bone were determined in osteoarthritis knees with varus or valgus deformity. Bone specimens were taken from the lateral condyle, the medial condyle, and centrally from the intercondylar area of seven varus and four valgus knees. The percentage volume of trabecular bone was determined by histomorphometry. On an additional nine knees, five with varus and four with valgus deformity, as well as ten knees from a normal autopsy material, photon absorptiometric determination of the mineral content of the same areas was performed. On average, the loaded condyle had twice the percentage volume of trabecular bone, and accordingly twice the mineral content, of the unloaded condyle. It was remarkable that the mineral content of the latter was of the same order as the condyles of the normal material.

Mechanical testing of trabecular bone in knee replacement.

To measure the strength of the juxta-articular trabecular bone in knee replacement, an osteopenetrometer was developed. The principle of its function is tha a needle penetrates the bony tissue and during this penetration the pressure on and the displacement of the needle is registered. The instrument was tested on cadaveric specimens of the proximal tibia. Good agreement was found between the osteopenetrometer findings in 18 cases and compression tests on the same bony structure. Forty-five measurements at different sites of the tibial resection surface revealed that the force needed for the penetration varied according to a pattern known from the determination of the elasticity modulus and ultimate compressive strength of bone in the same area. Histological examination of the penetration canals in the bone disclosed only sparse upsetting of bone. Curves from corresponding bone proved to reflect, in height as well as shape, the morphology of the skeletal structure. The osteopenetrometer appears to be applicable fo objectively assessing the solidity of trabecular bone during the operation. Thereby it is possible, in each individual case, to form a basis for more varied planning of the postoperative treatment programme.

The relationship between increasing intraarticular pressures and intraosseous pressures in the juxtaarticular bones. An experimental investigation in dogs.

The present experiment investigation of the relationship between intraarticular and intraosseous pressures was carried out in mongrel dogs. Under general anaesthesia simultaneous pressure measurements were performed in the femoral artery, the knee joint and the tibia and femur adjacent to the knee joint. The intraarticular pressure was increased by infusion of saline. Increase in the knee joint pressure caused an increase in the femoral intraosseous pressure, while the tibial intraosseous pressure remained unchanged. The pressure increase in the femur was most pronounced in dogs with intact epiphyseal plates. It is suggested that the rise in the femoral pressure was caused by compression of the venous drainage from the bone. Further evidence to support this suggestion was obtained from anatomical studies, intraosseous phlebography and experiments with selective venous compression.

Synovectomy as a prophylactic measure in recurrent haemophilic haemarthrosis. Follow-up of 23 cases.

Because of frequently recurring haemarthrosis which could not be controlled by conservative management 19 haemophiliacs were subjected to synovectomy on a total of 23 joints--17 knees, 5 elbows, and 1 hip. The patients were followed for an average of 23 months. Primary postoperative complications occurred, in the form of recurrent bleeding, in 8 joints. Rehabilitation was often difficult and long-lasting, and the range of joint motion was essentially restricted in 4 cases. After a follow-up period exceeding 6 months the findings in the remaining, mobile 19 synovectomized joints were: 12 had been relieved of haemorrhage, 5 had rare and two frequent haemorrhages. The reduction in the number of haemorrhages was significant (p is less than 0.01). In the light of the complicated postoperative course it is concluded that synovectomy should be used only on strict indications, viz. only in otherwise intractable cases of progressing haemophilic arthropathy.

Hemipelvectomy. Postoperative rehabilitation assessed on the basis of 41 cases.

Rehabilitation was evaluated on the basis of 41 consecutive hemipelvectomies for malignant tumours. Owing to early metastasization and death, 11 patients were not supplied with prostheses, while prosthetic fitting was attempted in the remaining 30. Of this number 27 completed prosthetic training, with the result that 15 used their prosthesis, while 12 discarded it after some time, six because of poor general health owing to recurrence of the tumours and six because they felt that the prosthesis was too heavy and difficult to wear. Twenty-three returned to work. After elimination of the most severely tumour-affected patients, there were 19 one-year survivors without recurrence. Thirteen of them were using their prosthesis every day, and thirteen had gone back to work. Serious mental sequelae were found in five patients, including four with long-lasting exogenous depressions and one with anxiety neurosis.
