Dual ectopic thyroid gland with Graves' disease and unilateral ophthalmopathy: a case report and review of the literature.

Ectopic thyroid gland (ETG) is a rare entity and can be seen anywhere in the route of descending gland. It is much rarer when encountered away from the midline. Dual ETG is such a rare entity that only a few have been reported in the literature. ETGs not only cause thyroid dysfunction, most commonly hypothyroidism and rarely hyperthyroidism, but also frequent local symptoms. Therapeutic options change according to patients' co-morbid diseases, age, size of goiter and presence of local symptoms. Graves' disease is very rarely detected in a patient with ectopic thyroid. There is no report in literature concerning ectopic thyroid presenting with Graves' disease and unilateral ophthalmopathy. We describe a case with dual ETG and Graves' disease and unilateral ophthalmopathy: it is the first reported in literature.

Subacute thyroiditis in a single lobe.

A 33-year-old woman with no history of thyroid disease reported pain in her neck and a sore throat. On physical examination, the thyroid gland was palpable. Serum T3 and T4 levels were increased, and the thyroid-stimulating hormone level was decreased. Thyroid scintigraphy with Tc-99m pertechnetate revealed nonvisualization of the left lobe of the thyroid. Ultrasonographic examination confirmed the presence of the left thyroid lobe. Fine-needle aspiration biopsy revealed thyroiditis of the left lobe of the thyroid. The patient was started on an anti-inflammatory drug. The follow-up thyroid scan showed a normal thyroid gland.

Thyroid hemiagenesis.

Thyroid hemiagenesis is a rare congenital anomaly in which one thyroid lobe fails to develop. Tc-99m pertechnetate scintigraphy and ultrasonography can be used to visualize this anomaly. The authors evaluated four cases with Tc-99m pertechnetate scintigraphy and ultrasonography and confirmed thyroid hemiagenesis. In one case, a Tc-99m MIBI scan excluded the suppresssion of the contralateral thyroid lobe. Tc-99m MIBI can be used to confirm the diagnosis of thyroid hemiagenesis.

Dobutamine combined with low-level exercise for myocardial perfusion scintigraphy.

Increased abdominal background activity is one of the limitations of dobutamine myocardial perfusion scintigraphy which may interfere with interpretation of the images. In this study, we evaluated the value of low-level exercise supplementation to dobutamine infusion in improving image quality. The control group (n = 32, Group A) received dobutamine alone, while the study group received dobutamine plus low-level exercise for the last 2 min of dobutamine administration (n = 26, Group B). One hundred and eleven MBq of 201Tl was injected 1 min before cessation of peak dose. The ratios of cardiac/hepatic (C/HEP), cardiac/subdiaphragmatic (C/INF) and cardiac/pulmonary (C/PUL) were calculated from anterior planar images taken immediately following the test. Cardiac/non-cardiac activity was also visually graded on a three-point scale. Visual evaluation showed improved cardiac/non-cardiac ratios as confirmed by significantly higher C/HEP and C/INF ratios for Group B (1.5+/-0.3 and 1.7+/-0.2, respectively) compared to corresponding values in Group A (1.2+/-0.4 and 1.4+/-0.4, respectively) (P<0.05). The cardiac/pulmonary (C/PUL) ratio was also higher for Group B (2.3+/-0.5) as compared to corresponding value in Group A (2.0+/-0.6, respectively), but this difference did not reach statistical significance (P > 0.05). No difference in the frequency of side effects and ECG changes were detected between the two groups (P > 0.05). Low-level exercise supplementation to dobutamine infusion for myocardial perfusion scintigraphy is a safe method and leads to a significant decrease in uptake by the areas adjacent to the heart, improving image quality.

Effect of silicone breast implants on myocardial imaging.

The authors investigated the effect of silicone breast implants (SBIs) on thallium-201 myocardial perfusion scintigraphy. Twenty-nine patients with SBIs and 14 control subjects were included in the study. All of the participants underwent a resting thallium-201 myocardial perfusion study. Comparison of the thallium images between patients with SBI and control subjects was done both visually and semiquantitatively. On visual analysis, neither image distortion nor image artifact attributable to the SBIs was noted. Semiquantitative analysis revealed that in patients with SBIs, the uptake values of the anteroseptal, anterolateral, lateroanterior, and lateroinferior myocardial walls was slightly, but significantly, lower than those of control subjects: anteroseptal, 87.7+/-6.0% vs. 92.8+/-5.1%; anterolateral, 88.0+/-5% vs. 92.6+/-4.5%; lateroanterior, 87.4+/-5% vs. 91.4+/-4.7%; and lateroinferior, 86.0+/-6.0% vs. 91.7+/-7.0%. The uptake value of the remaining walls in patients with SBIs were not significantly different than the control subjects. In conclusion, SBIs did cause a significant decrease in uptake values of the myocardial walls, which should be considered during interpretation of the images.

Devastating effects of chemotherapy: deafness and acute renal failure in a patient with epithelial ovarian cancer.

Paclitaxel and platinum combination is the standard chemotherapy regimen for patients with advanced epithelial ovarian cancer. The dose-limiting toxicity effects of this combination are myelosuppression and neuropathy. Herein, we report a case of a 71-year-old female with advanced epithelial ovarian cancer who developed bilateral total loss of hearing and acute renal failure related with paclitaxel- and carboplatin-based chemotherapy. Acute renal failure accompanied by complete loss of hearing in patients treated with carboplatin and paclitaxel combination has not been previously reported. This uncommon adverse effect of carboplatin and paclitaxel combination was discussed, and all the literature in English related with the toxicity of paclitaxel and carboplatin were reviewed.

Combined use of chemotherapy and 131I-metaiodobenzylguanidine in the treatment of advanced-stage neuroblastoma.

Despite intensive chemotherapy, surgery and/or radiation, prognosis continues to be very poor in disseminated neroblastoma. Owing to neuroblastoma sensitivity progress might be achieved with high-dose radiation. Metaiodobenzyl guanidine (MIBG) coupled with 131I provides the opportunity for highly selective radiation treatment of neuroblastoma, which could potentially deliver five to 10 times the dose of conventional external-beam treatment without specific tissue toxicity. To improve the long-term results in advanced-stage neuroblastoma, we integrated this new treatment modality with conventional chemotherapy. Eight neuroblastoma patients refractory to conventional treatment were treated with 131I-MIBG-chemotherapy (vincristine, VP16, iphosphamide, carboplatin, epirubicin, cyclophosphamide) combination. Five of the eight patients responded to 131I-MIBG treatment (2 complete and 3 partial responses). There were also three patients with stable disease. Median survival was 48 months (range 1 to 84 months), and five patients relapsed in their follow-up and died of progressive disease. We concluded that a combined 131I-MIBG and chemotherapy approach is useful in advanced-stage neuroblastoma patients, with considerably less side effects. Although survival is improved when compared to conventional treatment, the overall prognosis is still poor. More lethal radionuclide conjugation to MIBG which will deliver higher tumor and lower critical organ doses may offer the best solution for targeted radionuclide therapy of neuroblastoma.
