[Type I atrial flutter radiofrequency ablation. Importance of bidirectional block of isthmus between the inferior vena cava and of tricuspid valve ring].

PURPOSE: To determine the clinical importance of a bi-directional line of block demonstration in the inferior vena cava-tricuspid annulus isthmus as an end-point for radiofrequency (RF) atrial flutter (FL) ablation. METHODS: Forty consecutive patients (51 +/- 11 years) with type I FL were divided in 2 groups: GI (30 patients) anatomic, non-electrophysiologic isthmus ablation technique (interruption and non-induction FL criteria); and GII (10 patients) anatomic with electrophysiologic evaluation of bi-directional isthmus conduction. The isthmus activation was analyzed before and after anatomic RF ablation with a cateter exploring each side of the line of block, depending on the conduction evaluation (anterograde or retrograde). RESULTS: FL was interrupted and not reinduced in 26/ 30 (86.6%) GI patients and in 10 (100%) GII patients (p = 0.5558). During follow-up FL recurred in 30% of the patients in both groups. In GII, 6 patients with bi-directional block remained assymptomatic, whereas 3 patients with unidirectional block presented recurrence (p = 0.012). CONCLUSION: Electrophysiologic demonstration of bidirectional line of block in the isthmus is related to long-term success and should be the criterion for interruption of type I atrial FL RF ablation.

[Analysis of neonatal tetanus mortality in Brazil]

This study reports mortality from neonatal tetanus in Brazil (1979-1987), based on an analysis of death certificates. The distribution of causes of death in time and space was used as an indicator of proportional neonatal mortality from tetanus. Underreporting remains a serious problem, particularly in northern and northeastern Brazil. In most municipalities, only one death was notified during the period under consideration, thus highlighting the importance of considering the theoretical approach of "sentinel events" in the control of this disease. A proportional decrease in mortality from neonatal tetanus was observed in the states of Rondônia, Pará, Sergipe, São Paulo, Paraná, and Rio Grande do Sul. It was not possible to detect an association between immunization coverage with two doses of tetanus toxoid and neonatal tetanus mortality. The reduction in the number of deliveries taking place in the home during the period studied appears to have played an important role in the reduction of neonatal deaths attributable to this cause.

[Routine analysis of public health surveillance data: which statistical procedures to use?].

In recent years an increasing interest in epidemiological surveillance (that we prefer to label public health surveillance) has emerged. The viewpoint that the ease of access to computers and statistical software may permit the use of more sophisticated statistical methodologies in the analysis of surveillance data has been profounded in many studies. It is a cause of concern that this attitude, when used indiscriminated by, may lead to analysis lacking in theoretical support. Thus, first, a viewpoint about surveillance in public health is presented and the basic propositions of the theory of statistical hypothesis tests and interval estimation are described briefly and without technicalities. The nature of surveillance data, their nonsampling character and non-random selection are also commented on. Some descriptive procedures that may be used without loss of quality in analysis are described than some procedures that are proposed in the relevant international literature but that need fuller investigation before being introduced in routine data analysis are given.

[Difference in the action of prostaglandin E2 (PGE2) on longitudinal circular muscle contractions of the colon. Experimental study in dogs].

The effect of prostaglandin E2 (PgE2) on the circular and longitudinal contractions of the canine colon was studied in chronic conditions. A mechanical transducer developed in our laboratory and implanted on the canine colon collected simultaneously and separately the variations of length in two perpendicular directions. The recordings started at the 6 postoperative day. The colonic motility was recorded two hours before (as control period) and two hours after the intravenous injection of PgE2 at three different doses: 0.1-1.0 and 10 micrograms/kg. PgE2, at the three doses, induced, progressively over time, a significant decrease in duration and amplitude of the circular contractions, while the amplitude of longitudinal contractions remained unchanged. 10 micrograms/kg of PgE2 immediately induced a significant increase in the duration of longitudinal contractions followed by a decrease in this value during the 2nd hour after PgE2 administration. Each dose of PgE2 shortened the period of recurrence of longitudinal contractions, inducing rapid or immediate bursts of contractions. This effect was moderate or absent for the circular contractions. It is concluded, that PgE2 has different actions on circular and longitudinal contractions of the colon. According to the dose used, it induces hypokinesia (relaxation) in the circular muscle and premature strong contractions in the longitudinal motor activity.

[Basic principles of statistical methods applied to public health surveillance data analysis: a review].

Statistical methods in public health surveillance data analysis are important for detecting temporal and/or spatial disease clusters, which can indicate an outbreak or an epidemic. In this paper we present a review and literature survey introducing the public health surveillance process and focusing on some statistical methods which are indicated to the data analysis in routine procedure. Comparative studies of these methods are also analyzed.
