Cineangiographic evaluation of changes in regional left ventricular wall motion after acute myocardial infarction.

To evaluate the natural course of changes in regional left ventricular function after acute myocardial infarction, we compared cineventriculograms taken during the acute and chronic stages of myocardial infarction in ten patients who had sustained coronary occlusion despite intracoronary thrombolysis. Three ventricular silhouettes (end diastole, aortic valve opening, and closure) were superimposed using external reference markers, and 36 radial grids were drawn from the center of gravity of the end-diastolic silhouette to the endocardial margin of each silhouette. Measurement of the length of each radial grid provided quantitative details of segmental centripetal wall motion. In the infarcted area, percentage total systolic shortening (% delta LTOT), iso-volumic shortening (% delta LISO), and ejection phase shortening (% delta LEJ) remained unchanged. In the nonischemic area, % delta LTOT tended to increase from 34% +/- 6% to 41% +/- 6%. Although % delta LISO was unchanged (9% +/- 4% vs. 8% +/- 1%), % delta LEJ increased from 25% +/- 4% to 37% +/- 6% (P less than 0.05). Thus, progressive increases in ejection phase shortening in normal regions may contribute to the functional recovery of the left ventricle after myocardial infarction.

Clinical effect of danazol in patients with IgA nephropathy.

A multi-center study on the clinical effect of danazol in patients with IgA nephropathy is described. Two different doses of oral administration, including 200 mg or 300 mg per day, were employed in the present study. Urinalysis and measurements of complement components, i.e. C3 and C4, were performed before, and in the eighth and 20th week after the administration of danazol. It was demonstrated that 200 or 300 mg of danazol administration was effective in reducing the amount of proteinuria and in increasing the levels of serum complement components in the patients. It is concluded that the administration of danazol may be useful for treatment of patients with IgA nephropathy.

Cardiovascular effects of ICI 118,587, a new beta-adrenoceptor partial agonist in man.

Effects of a new selective beta1 partial agonist, ICI 118,587, on cardiac function were assessed in clinical settings. In 7 patients, responses to multistage treadmill exercise were studied before and after an acute intravenous injection of the drug. The heart rate and blood pressure were not altered by ICI 118,587 at rest but increases in both parameters in response to exercise were significantly reduced. Neither oxygen consumption nor plasma norepinephrine level was modified by the drug both at rest and during exercise. The long term effects of ICI 118,587 were assessed in 6 patients with mild to moderate cardiac failure consequent upon ischemic heart disease. After chronic administration of the drug exercise duration was increased. The symptom-limited maximal oxygen consumption increased by 16%, associated with prolongation of the exercise tolerance. In those patients who also had symptoms of angina pectoris, exercise levels which caused angina during the control study were tolerated without symptoms after ICI 118,587. Twelve patients with nocturnal bradycardia resulting from atrial fibrillation of sick sinus syndrome were treated with ICI 118,587. Monitoring of heart rate by 24-hour Holter ECG showed that ICI 118,587 increased minimal heart rate during sleep. Being a beta1-adrenoceptor partial agonist, it has both agonist and antagonist properties. Thus, ICI 118,587 buffers the heart from an excessively low sympathetic tone which may occur during sleep and from an excessively high tone during exercise. It appears to be of benefit in the treatment of mild to moderate cardiac failure consequent upon ischemic heart disease. It also improves oxygen demand-supply imbalance without inducing further myocardial depression or inappropriate bradycardia at rest. ICI 118,587 may therefore be described as a cardiostabilizer.

Immunoelectron microscopy of the outer membrane of rat hepatocyte nuclear envelopes in relation to the rough endoplasmic reticulum.

Prefixed nuclei were isolated from rat liver after perfusion with dilute glutaraldehyde. These nuclei sometimes were associated with the rough endoplasmic reticulum (ER), and occasionally continuity of the outer nuclear membrane with rough ER membrane was found. When these prefixed nuclei were incubated with ferritin antibody conjugates against cytochrome P-450, the cytoplasmic face of the outer nuclear membrane was labeled with ferritin similar to the labeling of the rough ER membrane. In the nuclear pore region, ferritin particles were present on the outer membrane up to the outer annuli of the pore complexes. When unfixed nuclei were incubated at 0-4 degrees C or at 20 degrees C, some outer nuclear membranes were detached from the nuclear surface. In this case the nuclear pore complexes remained associated with the inner nuclear membrane, and small inside-out vesicles were formed at these pore regions. We also found some rough vesicles heavily labeled with ferritin particles within the nuclear matrix. They probably were derived from the rough ER or from the outer nuclear envelope which was internalized artificially during incubation.

Clinicopathological features in the Japanese patients with IgA nephropathy.

The actual state of IgA nephropathy in Japan was surveyed throughout the nation by a questionnaire. Five hundred patients were collected from 26 departments of pediatrics and 2,175 from 27 internal medicine. IgA nephropathy accounted for 19.2% of the children and 30.0% of the adults among primary glomerular diseases. Most of the patients, regardless children and adults, were detected by chance proteinuria and/or hematuria. No significant difference of renal glomerular findings were observed morphologically, between the children and the adults. IgA nephropathy was characterized by relatively benign clinical course, namely, 87.2% of the children and 74.6% of the adults had a favorable outcome. Nephrotic syndrome developed in 4.6% of the children and 3.7% of the adults, and hypertension in 4.6% and 10.6% of the children and the adults, respectively. While, the fact should not be neglected that 1.8% of the children and 8.0% of the adults developed renal failure or died.
