[The effects of different anaesthetic technics on lactate under the course of aorto-femoral bypass operation (author's transl)].

46 measurements of lactate during aorto-femoral bypass-operation were performed under epidural analgesia, halothane- and neurolept anaesthesia. Independent of the metabolic parameters a significantly higher lactate rate was found under halothane anaesthesia than in the two other groups even though - at the same perfusion volume in all three groups - a significantly lower arterial mean pressure and peripheral resistance was measured under halothane- and epidural anaesthesia than under neurolept anaesthesia. The authors come to the conclusion that under aorto-femoral bypass operations a moderate hypotension can be carried out even in patients, showing arterio-sclerotic changes of the vessels, without a significant influence on metabolism.

[Tachyphylaxis in continuous epidural anaesthesia with bupivacaine 0.125% and 0.25% (author's transl)].

Based on the results in 105 patients under continual thoracic epidural analgesia differences in the dose response relationship of bolusinjections are found. While under bupivacaine 0.25% a decrease of the effectivity of additional bolusinjections can be stated, the dose response relationship does not change under bupivacaine 0.125%. Moreover the effectivity of bupivacaine 0.25% increases after an interruption of the local anaesthetic infusion. The pH-value of bupivacaine 0.25% was 6.0 and that of bupivacaine 0.125% 6.3. Even so the differences of the pH-values between the two solutions are rather small, a possible key to the problem of the dose response relationship, concerning tachyphylaxis, can be suggested. But further investigations have to show, whether the use of local anaesthetic solutions, being stable at pH-values of 7 to 8, makes the continuous epidural analgesia easier to handle for postoperative pain relief.

[Modification of the analgetic effects (buprenorphine, pentazocine, pethidine) on respiration and haemodynamics by epidural, halothane- or neuroleptanaesthesia (author's transl)].

In 38 patients buprenorphine, meperidine and pentazocine were given in a single dose for postoperative pain relief 20 hours after the end of anaesthesia. Measuring the parameters of the high- and low-pressure system as well as the metabolism the authors found that the effects of these analgetic medicaments, intravenously injected were significantly influenced by fentanyl, halothane or diazepam, given under the course of operation. Especially buprenorphine, injected after epidural anaesthesia in combination with diazepam sedation, proved to have a rather negative effect, because it caused a strong depression of respiration and circulation. On the other hand buprenorphine had, given after neuroleptanaesthesia, a neutralizing - and pentazocine and pethidine in combination with neuroleptanaesthesia a stimulating influence on the circulation. After halothane-anaesthesia the effect of the analegtics on the cardiovascular system was, when buprenorphine was given, depressing and when pentazocine was given indifferent. Similar reactions, but more pronounced, could be seen in the epidural group. With certain reservations, caused by the preliminary character of this study, the following conclusions can be drawn for the anaesthetic practice: 1 Choosing analgetic drugs for postoperative pain relief, the anaesthesist has to be aware of the interactions, possibly resulting from the medicaments, given during anaesthesia. 2. The number of medicaments, given during anaesthesia, should be kept small, considering the eventual interactions and the unintentional secondary effects.

[Effects on platelet functions].

Platelet adhesiveness was measured in a total of 589 healthy volunteers and patients. Patients suffered from heart failure, diabetes mellitus, myocardial infarction and deep vein thrombosis have a significant higher platelet adhesiveness as healthy volunteers. The effect of the socalled stressors on platelet adhesiveness was shown in vivo; the same values of platelet adhesiveness were seen as in patients. Therefore it can be concluded that stressors constitute a risk factor in patients with altered vessel walls.

[Cimetidine in the treatment of duodenal ulcer (author's transl)].

In a double-blind trial of patients with duodenal ulcer at three centres the healing rate after a four-week course of treatment (1 g cimetidine daily) was 79% (30 out of 38 patients), compared with 58% (23 out of 40 patients) on placebos. Cimetidine did not effect subjective symptoms or the need for antacids to relieve pain, except for a minor beneficial effect on diurnal ulcer symptoms during the second week of treatment. Results of long-term administration suggest that cimetidine at 400 mg daily prevent s relapses, but the possibility of an increased relapse rate after cimetidine withdrawal can not as yet be excluded.

The response of oscillating glycolysis to perturbations in the NADH/NAD system: a comparison between experiments and a computer model.

The glycolytic pathway is described by a set of coupled non linear differential equations of first order with respect to time. The individual terms of these equations consist of enzyme velocities assuming a steady state hypothesis for the enzymatic forms. These are specified and the system is solved numerically. Oscillations are explained by interaction of PFK with the adenylate system. The conditions for the occurrence of oscillations are tested in a series of computer runs. The phase relations between intermediates of the model agree with those found in yeast cells. As an application of the model the disturbation of oscillations by the addition of acetaldehyde is simulated. The predictions of the model agree with experimental results.

[Fetomaternal macrotransfusion after attempted external version].

BACKGROUND: External cephalic version is an alternative to both vaginal breech delivery and to caesarean section. The objective of this study was to summarize potential complications of external cephalic version. Which means are available for early detection and treatment of these complications? CASE REPORT: A 28-year-old 1/0 with breech presentation was considered for external cephalic version at the 38th week of gestation. As three version attempts failed, the patient was discharged after normal fetal heart tone (FHT) registration and normal ultrasonographic findings on the next day. Caesarean section was planned six days later. On the day of admission, FHT registration revealed a decreased beat-to-beat variability and a sinusoidal baseline. Doppler-flow indices of the A. umbilicalis and A. cerebri media were normal, although middle cerebral artery peak systolic velocity was increased. Rapid caesarean section was performed, and an anemic baby (hemoglobin 3.4 g/dl) was born. After transfusion of 100 ml red blood cells, further development of the newborn was normal. DISCUSSION AND CONCLUSION: Fetomaternal macrotransfusion may be a rare complication of external cephalic version, occurring even several days after the mechanical manipulation. FHT registrations and ultrasonographic doppler flow measurements performed periodically unit birth are necessary to detect such complications early. The Kleihauer-Betke test is the method of choice to diagnose fetomaternal macrotransfusion.

[Endometriosis and malignoma].

Malignant tumors arising from endometriosis are rare. A frequency of about 1% has been reported with in 80% the ovary, and in 20% extragonadal sites being affected. The most common extragonadal manifestations are the rectosigmoid and the rectovaginal septum. For extragonadal malignant tumors arising from endometriosis, complete resection followed by post-operative radiotherapy, possibly plus adjuvant progestin therapy, is the treatment of choice. Endometriosis-associated ovarian carcinomas are likely to present with lower stage disease and predominantly lower grade tumors. While their treatment follows that of common ovarian cancer, a poorer response to chemotherapy must be considered. As unopposed estrogen replacement therapy has been identified as a risk factor for the development of endometriosis-associated cancer, it is not recommended for hormone replacement therapy in women with a history of endometriosis. Loss of heterozygosity and mutations of the PTEN tumor suppressor gene may be early events of tumorigenesis. Endometriosis and its malignant transformation, perhaps, may serve as a suitable model in this regard. According to recent studies, endometriosis is associated with an increased relative risk of non-Hodgkin lymphoma.
