Investigation of serum vitamin A levels of children who had a history of recurrent diarrhoea and acute respiratory infections in Ankara.

Vitamin A deficiency is considered a widespread public health problem among preschool children in the developing countries. A limited number of studies reveal an association between xerophthalmia and significant systemic diseases including protein-energy malnutrition, diarrhoea, and acute respiratory infections. The present study was carried out to assess the vitamin A status of preschool children who have a history of recurrent acute respiratory infections and diarrhoea. We have already shown that serum vitamin A levels of all the study groups were lower than the control group (P less than 0.001) and the detailed statistical analyses made clear that malnutrition is the major contributing factor (P less than 0.001) although infections also have a negative effect on serum vitamin A levels (P less than 0.01). We have also shown that subclinical vitamin A deficiency is a problem of public health importance in this region, since about 64 per cent of the children examined were found to have low levels of serum vitamin A (less than 20 micrograms/dl).

Acute Disease

[Importance of vaccination and bacteriological studies in diphtheria epidemics].

The Ouchterlony - Elek gel diffusion test was performed in twenty-nine patients having a positive culture result for diphtheria bacilli on Loeffler and tellurite agars in January and February, 1984 and toxin production was detected in 20 of them. Except one adult patient, the cases ranged in age 2 and 12 years. In 14 patients who have received at least one immunization with diphtheria vaccine had no complication, besides three patients without immunization in whom two received antibiotics plus antitoxin therapy had carditis and soft palate paralysis. Five patients in the non-immunized group and 6 patients in the group received at least one immunization had diphtheritic membrane. In spite of three patients with cardiac and neural complications, with early diagnosis and beginning therapy with antibiotics and antitoxin before the result of in vitro toxigenicity test, O% mortality rate was obtained.

Antibody Formation

The relationship between blood glucose and serum electrolyte levels in children with acute diarrhea.

The relationship between simultaneous serum electrolyte and blood glucose levels was studied in 119 moderately or severely dehydrated children with acute infectious gastroenteritis. Their ages ranged between two months to fifteen years. A positive correlation was found between blood glucose and serum sodium levels in the children with serum bicarbonate values below 15 mmol/L. This correlation is more prominent in cases of severe metabolic acidosis with serum bicarbonate values less than 10 mmol/L. No correlation was found between blood glucose, serum potassium and chloride levels. Our results show that a positive correlation between blood glucose and serum sodium levels is present only in cases with metabolic acidosis, which is contrary to classical knowledge.

Acute Disease

Sodium and potassium levels in colostrum.

The sodium and potassium concentrations of serum, urine and breast milk from 40 healthy, lactating mothers aged 18 to 35 years were measured. Nineteen women were primiparous and 26 were delivered prematurely for various reasons. No correlation was found between colostrum and urine sodium and potassium levels, and colostrum and serum potassium levels, however, there was a correlation found between colostrum and serum sodium levels (r: 0.311, p less than 0.05).
