[The study of mismatch negativity].

We studied age related correlations of mismatch negativity (MMN) auditory event-related potentials (ERPs) using odd ball paradigm in 121 normal subjects (from 6 months to 43 years) and 33 mentally retarded subjects (from 2 to 18 years). The subject was instructed to read a book in order to ignore the stimulus during the experiment, or no specific task was imposed on young child. The MMN wave form was clearly obtained in normal 6 month old subjects. As the age advanced from 6-months, the latencies of MMN progressively shortened till about 7 years, when the latency reached adult level. One-third of mentally retarded patients had prolonged MMN than that normal age-matched subjects. Reliable recording of P300 requires subject's cooperation and attentiveness, which are difficult to achieve in young children or mentally retarded patients. MMN may be utilized in children to evaluate the development of the cognitive function.

[Immunoglobulin class and light chain type of oligoclonal bands in CSF in children with CNS infectious diseases].

Agarose gel electrophoresis and immunofixation of CSF and serum from 16 patients aged from 1 day to 13 years with CNS infectious diseases (5 with purulent meningitis, 5 with aseptic meningitis, 4 with acute encephalitis, 1 with subdural empyema and 1 with SSPE) revealed oligoclonal IgG bands in 10 cases. In 4 cases, a high CSF/serum albumin ratio was recognized without an increase of IgG-index, probably representing a systemic immune response. Neither IgA nor IgM bands were recognized. IgG light chain was examined in 7 cases and IgG kappa bands predominated in 6 cases. IgG lambda bands predominance presented in only one case. Free IgG kappa or lambda chain was not found. These results suggested that the measurement of not only IgG-index but oligoclonal bands may have a diagnostic usefulness to know about local production of immunoglobulins in CSF.

[Visual evoked potential].

Under the pathological conditions, the VEP may show changes in amplitude, latency, or waveform in one or more of its components. The major advantage of the pattern reversal VEP over the flash VEP lies in smaller variability in the waveform and latency of its components in the healthy population. The flash VEP is, however, particularly useful to infants and newborns. The variation in the waveform of the flash VEP was evaluated, and Type V was abnormal waveform. Subsequently, patients with various CNS diseases were examined. Acute hemiplegic patients showed high amplitude or increased latency patterns. In 45 percent of the West syndrome cases, VEP showed abnormal responses. The patients with asphyxia, respiratory distress syndrome and mental retardation showed increased latency. Latency was decreased in hypoglycemia and hypocalcemia of newborns, and it was increased or decreased in autism and epilepsy. In our study, it was concluded that the rhythmic after-discharge is a true response. This assumption is supported by the following observations: (1) the after-discharge of VEP appeared before the occurrence of the alpha-wave in EEG; (2) the frequency of the after-discharge was generally higher than that in EEG; and (3) the frequency of the after-discharge did not change between the conditions of the resting state and hyperventilation. There was no after-discharge in VEP in patients with a history of encephalitis even when their IQs were normal, and the latency was increased in pattern reversal VEP.

Immunoglobulin therapy in the West syndrome.

An examination was made of high dose non-treated immunoglobulin (NTIG) therapy at an early stage of the West syndrome (WS). Six patients with cryptogenic WS who suffered attacks ranging from 15 days to 6 months (mean 70 days) and 5 patients with symptomatic WS who suffered attacks ranging from 14 days to 4 months (mean 32 days), were administered NTIG 6-10 times intravenously at 100-200 mg/kg of body weight at intervals of 2 or 3 weeks. All patients with cryptogenic WS showed complete remission in accordance with normalized electroencephalogram (EEG) without following anticonvulsants medication. Of the 5 patients with symptomatic WS one patient showed cessation of clinical seizures in agreement with EEG improvement and 2 other patients revealed transient cessation of clinical seizures with recurrence. In cases of complete remission, the energy percentage of the power spectrum for each frequency band displayed a tendency toward gradual decrease of delta wave band in correspondence with the increase of theta wave band percentage after NTIG therapy, suggesting that high dose NTIG may be useful for early treatment of cryptogenic WS and for inhibiting brain deterioration owing to epileptic encephalopathy.

A case of carcinoma of the rectum after radiotherapy for carcinoma of the cervix.

Carcinomas are known to develop after radiation therapy for benign or malignant lesions. However, the development of primary carcinoma of the rectum following radiotherapy is relatively rare. This paper presents a case of adenocarcinoma of the rectum in which the history strongly suggested a radiation-induced carcinoma 12 years after initial radiotherapy for carcinoma of the cervix. Furthermore, it was interesting that metastatic adenocarcinoma of the skin was localized only over the pelvic and perineal area that had mild brown pigmentation from the previous irradiation. The relation between the effect of irradiation and a second primary cancer in the irradiated skin is discussed.
