[The condition of intestinal microflora in patients with hematologic tumors receiving polychemotherapy].

The subjects of the study (105 patients with blood system tumors), were divided into 4 groups. The patients of the first group received only cytostatic therapy. The subjects of the second, third and fourth groups were administered a combination of cytostatic and antibacterial agents. In addition to this, the patients of the third and fourth groups received prebiotic preparations as means of corrective therapy. The results obtained by measurement of short-chain fatty acids (SCFA) in feces by means of liquid-gas chromatography were used as criteria for evaluation of the therapy influence on intestinal microflora. The study shows that the cytostatic and combined treatment worsen the disbalance between aerobe and anaerobe populations, which manifests in SCFA spectrum alteration. Use of prebiotic preparations results in improvement of intestinal microflora condition or prevents the aggravation of its disturbance caused by the treatment of the fundamental illness, which becomes apparent in stabilization or normalization of SCFA profile in feces. Study of SCFA makes it possible to monitor microbiocenosis, adjust corrective therapy individually and evaluate its effectiveness.

[Helicobacter pylori and stomach adenoma in outpatient practice].

AIM: To study the role of Helicobacter pylori (HP) persistence in generation of gastric mucosa adenoma. MATERIAL AND METHODS: Esophagogastroduodenoscopy with biopsy of the mucosa from the antral portion and body of the stomach was made for diagnosis of HP and morphological changes in 28 patients aged 47 to 86 years with gastric adenoma. RESULTS: Chronic gastritis with HP was diagnosed in 57.1% patients. HP contamination was, as a rule, weak (32.1%) or moderate (53.5%). It correlated with activity of inflammatory and dystrophic changes (the first degree--35.7%, the second one--39.3%; atrophy--99.6%, metaplasia--92.8%, respectively). CONCLUSION: The morphological study revealed a strong correlation between gastric adenomas and HP persistence. As adenomas are characterized by a high risk of malignant transformation and must be removed, it is recommended to eradicate HB infection.

[Some comparative aspects of treating the gastroesophageal reflux disease eradication].

At present proton pump inhibitors have been the most efficient drugs for treating the gastroesophageal reflux disease (GERD). Pariet may be selected as a drug for treating all forms of GERD as it requires smaller doses and shorter treatment period to obtain the clinical and endoscopic remission. Upon the clinical and endoscopic remission, patients need a supportive treatment. Different patients need various supportive treatment schemes. Our goal comes to the provision of the maximum efficiency of the schemes when prescribing the minimum doses of the drug.

[Short-chain fatty acids and implications of their measurement in the diagnosis of encephalopathies].

Sixty patients with chronic hepatitis of various etiology at the stage of hepatic cirrhosis with hepatic encephalopathy syndrome (HES), thirty patients with cerebral atherosclerosis with dyscirculatory encephalopathy (DE) and 60 healthy controls have been examined in the study of basic diagnostic criteria of encephalopathies by short-chain fatty acids (SCFA) serum levels. Sensitivity and specificity of this test was also evaluated. Gas-liquid chromatography determined SCFA in the feces and peripheral blood serum from the above examinees. It was found that relative consent of SCFA and for the most part profiles of propionic and fat acids, acids with long chains, SCFA isomers in patients with encephalopathy of different genesis correlate with the results of clinical, laboratory and psychometric methods of examination and should be used together. Basic criteria are devised.

[Interrelations of hyperplasia of gastric mucosa and helicobacter infection (based on data from a polyclinic center].

AIM: To examine a relationship of hyperplastic processes in the gastric mucosa (GM) to Helicobacter infections. MATERIAL AND METHODS: A hundred patients aged 44 to 86 years who had gastric polyps [Group 1, foveolar hyperplasia (n = 21); Group 2, hyperplasiogenic polyps (n = 65), Group 3, adenomas (n = 14)] underwent esophagogastroduodenoscopy with biopsy of the mucosa from the athrum, body, and fundus of the stomach to reveal Helicobacter pylori (Hp) and GM morphological changes. RESULTS: Hp-associated chronic gastritis was detected in 56% of the patients (66.7, 52.3, and 57.1% in Groups 1, 2, and 3, respectively). The rate of Hp seeding correlated with the magnitude of inflammatory and dystrophic changes. As the regulation of epithelial growth and differentiation impairs, the magnitude of GM structural alterations increase. CONCLUSION: The findings indicate that there may be a relationship between the frequency, degree of Hp infection and the pattern of GM morphological changes, which makes it expedient to use eradication therapy in this group of patients.

[Hyperplastic processes of the gastric mucosa in helicobacter infections].

The investigation of possible H.p. role in the development of stomach mucous changes, promoting to arising of hyperplasiogenic polypus is the purpose of this study. Gastro-duodenal endoscopy with biopsy of stomach mucous from the antral part, body and bottom for H.p. diagnosis and morphological changes was made in 85 patients (at the age of 43-82 years) with hyperplasmiogenic polypus. The H.p. associated chronic gastritis was determined in 47.6% of the patients. H.p. intensity (prevailed weak and moderate--69.2%) correlated with the inflammation activity degree (1st and 2nd--80.3%) and with dystrophic changes (initial and moderate mucous atrophy--74.4%). The obtained data indicate the possible relation to H.p. infection frequency, degree and character of stomach mucous morphological changes. It allows to examine the expedience of the eradiction therapy for the patients with hyperplasiogenic polypus, infected by helicobacterial infection.

[Pariet in eradication therapy plans].

In the article the results of the eradication therapy by Pariet in the patients suffering from the duodenal ulcer disease, associated with Helicobacter pylori (HP), are estimated. Four schemas with use 40 and 20 mg of Pariet per day in a combination with two antibacterial preparations were evaluated. The therapy duration was 7 days. The ulcer cicatrisation has occurred in 100% of the patients. The eradication was achieved in 100% of cases at use of a dose 40 mg of Pariet per day. At use of the schemas with a dose 20 mg of Pariet per day the eradication was observed in 67% of patients. Pariet is considered as the high efficient preparation in the eradication and cicatrisation of the ulcer.

[Prognostic criteria in the course of duodenal ulcer].

Examination and long-term follow-up covered 95 patients with duodenal ulcer (DU). Mean age of the patients was 32 +/- 1.67 years. The patients were divided into two groups: with rare DU recurrences (n = 36) and with frequent recurrences (n = 59). Diagnostic criteria for development of rarely recurrent DU were the following: blood group A(II), exacerbations in winter, weak symptoms at DU onset, weak abdominal pains, solitary surface ulcers. For development of frequently recurrent DU, diagnostic criteria include O(I) blood group, spring exacerbations, marked symptoms of the exacerbation, severe abdominal pains, multiple deep ulcers.

[Composition and role of short chain fatty acids in feces and peripheral blood serum of patients with cholelithiasis].

Gas-liquid chromatography was used to estimate content of short-chain fatty acids (SCFA) in 25 patients with cholelithiasis, 32 patients with irritable colon syndrome and constipation (ICS) and 35 healthy subjects. It was found that SCFA absolute and relative concentration in the feces of cholelithiasis patients is abnormal indicating disturbance of microbiocenosis as shown by changes in functional activity of some anaerobes of the intestinal microflora participating in enterohepatic circulation of bile acids. Alterations of SCFA content in the serum of cholelithiasis patients may be related to steroids disbolism. The study of SCFA in the feces and peripheral blood serum from cholelithiasis patients is of diagnostic value.
