Efficacy of 7-day treatment with metronidazole+miconazole (Neo-Penotran) - a triple-active pessary for the treatment of single and mixed vaginal infections.

OBJECTIVE: To evaluate the efficacy of Neo-Penotran pessaries (metronidazole 500 mg + miconazole nitrate 100 mg) in candidal, bacterial and trichomonal vaginitis and in mixed vaginal infections. METHOD: Ninety-seven patients with clinical diagnosis of vaginitis entered this open, non-comparative study. Each patient inserted one pessary twice daily for 7 days. Gynecological and microbiological assessments were carried out before, and 8-10/21-23 days after the start of treatment. RESULTS: Vaginitis symptoms were resolved in 91% of the 74 patients evaluated, and improved in a further 7%. Microbiological cure rates were 97.3% for trichomonal, 86.6% for bacterial and 81% for candidal vaginitis. Recurrence rates were 2.7, 3.8 and 16.1%, respectively. Overall microbiological cure rate for mixed infections was 86%, with 93% for trichomonal+bacterial, and 73% for bacterial+candidal vaginitis. In two out of three cases with trichomonal+bacterial+candidal infection, the microorganisms were eradicated completely. CONCLUSION: Neo-Penotran provides immediate and effective treatment for vaginitis, irrespective of single or multiple infection, even when the diagnosis may be uncertain.

137Cs and relationships with major and trace elements in edible mushrooms from Mexico.

137Cs and 40K specific activity together with major and trace elements were determined in soil samples and in different edible wild mushroom species collected from a seminatural temperate forest ecosystem located in the central part of the Mexican Volcanic Belt. The activity measurements were made using a gamma-ray spectrometer system with a high purity germanium (HpGe) detector. The major and trace elements were determined using emission spectrography and mass spectrometry, respectively. The aggregated transfer factors for 137Cs were estimated in 30 local mushroom species collected from 1993 to 1999. Differences as large as three orders of magnitude were observed. The contribution of mushrooms for the total 137Cs dietary intake by the local population was estimated to be 37%. Mushrooms also showed to be good accumulators for Rb, Cu, Cs and Se.

Response to Duovent of chronic reversible airways obstruction--a controlled trial in general practice.

A controlled trial was carried out in general practice comparing, in two successive 3 week periods, respiratory function in patients with chronic reversible airway obstruction treated, in the first period with their existing metered dose inhaler (MDI) and in the second period with Duovent , an MDI combining anticholinergic and beta2-stimulant agents respectively. One hundred and twenty-two patients were included. Peak expiratory flow rates were increased to a statistically significant degree during the Duovent period and, during this period, there was a significant reduction in day-time cough, night-time wheeze and night wakings . Global ratings by patients and physicians showed that 76% of patients improved with Duovent , and two-thirds of the patients preferred Duovent to their former MDI. Side effects with Duovent were mild, with dry mouth predominating.

Neotran--a new double-active pessary for the treatment of vaginitis.

The common causes of vaginitis have changed in recent years. Infection with Trichomonas vaginalis is much less frequent than it used to be. The most common causes of vaginitis are candidiasis and bacterial vaginosis due to infection with mixed bacterial flora. In general practice, treatment of vaginitis is often begun before microbiological confirmation of the diagnosis is available. A vaginal pessary has been formulated to provide an effective therapy in all three forms of vaginitis mentioned above. This formulation, Neotran, is believed to be the first pessary to contain both metronidazole and miconazole. In vivo and in vitro studies showed Neotran to be effective against Candida albicans and mixed bacterial flora. Neotran was shown, in vitro, to be active against Trichomonas but, because of the current rarity of trichomonal infection in the UK, it was not possible to show its effectiveness against Trichomonas in vivo. In a general-practice clinical trial of the treatment of vaginitis in 80 patients, Neotran pessaries achieved a microbiological resolution rate of 83% in those patients who were bacteriologically evaluable and was well tolerated. Overall clinical evaluation of the patients showed the condition to be resolved in 73% of patients, improved in 21% and unchanged in 6%.
