[Waiting lists and reality. Revision of the waiting list for reduction of breast hyperplasia in health region 1].
The Red Cross Hospital is the only public hospital providing plastic surgical services for the Oslo population. The waiting list for reduction mammaplasty had become unacceptably long. Some women had been waiting for as long as 4.5 years. We therefore decided to reexamine all the 309 patients in question. Only 22% were cleared for operation. 19% were removed temporarily from the waiting list due to obesity (awaiting weight reduction). The remainder was found to be either mainly motivated or to be motivated for mainly oesthetic reasons and therefore not eligible for public health funding.
[Burns among the elderly. Occurrence, causes, local treatment].
Burns in geriatric patients carry a grave prognosis. Reduced thickness of the skin and reduced mobility make older persons susceptible to deep burns. First-aid treatment is insufficient in most instances and generally, there is a delay in seeking medical treatment. Pre-existing degenerative diseases may account for numerous complications and a prolonged course. Accidents at home are most common. It is assumed that most of these could be prevented if proper precautions are taken. Our experiences from treatment of 85 geriatric burn patients at the Red Cross Clinic, Oslo, prompted the present study. We discuss various aspects of problems to geriatric patients.
A long-term psychosocial follow-up study of burned adults.
A clinical psychiatric follow-up study of 70 burned adults was performed 3-13 years after the burn injury. The majority of patients had minor injuries. Twenty-three percent suffered from definite psychosocial problems at follow up. Patients with severe injuries more often had problems (44%) than patients with minor injuries (16%). Outcome was not related to premorbid psychopathology except for patients with more severe injuries. A combination of variables describing length of hospital stay, the presence of scars, premorbid psychopathology, experiencing death threat when injured and deviant behaviour during the hospital stay was found to be the best predictor of negative psychosocial outcome (positive predicting power 83%; sensitivity 94%).
Increased fibrinolytic activity after surgery induced by low dose heparin.
Forty women undergoing surgery under general anaesthesia for hyperplasia mammae were randomized to treatment with low dose heparin (5000 IU twice daily) or not. Preoperatively, and repeated on the 3. postoperative day, assays of euglobulin clot lysis time (ELT) after venous stasis, tissue plasminogen activator (t-PA) and plasminogen activator inhibitor (PAI) were performed. Compared to the control group heparin was found to give a significant rise in t-PA antigen before (24.0 vs. 11.2 ng/ml, p = 0.02), and especially after venous stasis (104.8 vs. 47.3 ng/ml, P = 0.007). t-PA activity was also significantly more increased after venous stasis in the heparin group than among the controls (4.2 vs. 1.4 U/ml, p = 0.04). This was also reflected in the ELT after venous stasis which was significantly shorter in the heparin group (p = 0.01). No differences in PAI were found between the groups. The present results point to a heparin-induced increase of t-PA synthesis in the endothelium, also giving rise to an increased level of circulating t-PA as measured immunologically. This effect of small dose heparin may play an important role in the prophylaxis against thrombo-emboli, in addition to the anticoagulant effect.
