A new instrument for the assessment of oedema based on the impression method is described. The measurement parameters are defined and the errors corresponding to the electrical and mechanical stability of the instrument are measured. The overall accuracy is calculated theoretically for the translocated volume. The accuracy and reproducibility are evaluated on plastic foam. The clinical procedure to measure oedema with this instrument is described. We show that the accuracy of the translocated volume parameter is very dependent on the overall stability of the instrument and that this instrument has an overall relative error of less than 7.3 per cent for a representative measurement. Experimental measurements on plastic foam show that the measurement parameters could be reproduced with standard deviations of less than 6 per cent and that the standard deviations for translocated volume are within the calculated overall relative error. Measurements on four patients with chronic oedema in an extremity show significantly different results on locations where pitting could be recognised compared to the contralateral nonoedematous extremity.
The impression method for assessment of subcutaneous oedema was evaluated in a rat testis model where the testicular interstitial fluid volume was changed both artificially by infusion of rat plasma and pharmacologically by administration of human chorionic gonadotrophin. Both the integral value and the impression force value, as measured with the impression method, changed with infused volume, and changes as small as 16 microliters (approximately 7% of the total interstitial fluid volume in a testis) could be detected. Rats were treated with a single injection of 100 i.u. human chorionic gonadotrophin in order to induce changes in the volume of interstitial fluid in the testis. Both the integral value and the impression force value seemed to reveal information on testicular interstitial fluid volume in rats treated with human chorionic gonadotrophin that was similar to data revealed by measuring the actual fluid content in the testis. Interstitial fluid volume measured morphometrically in the contralateral testis in human chorionic gonadotrophin-treated rats was significantly correlated to the impression force value (r = 0.75) and the integral value (r = 0.52). This rat testis model proved to be an interesting experimental set-up for evaluation of the impression technique.
In a randomised, double-blind study, the preparation Curbicin, obtained from pumpkin seeds and dwarf palm plants (Cucurbita pepo L. and Sabal serrulata), was compared with a placebo in the treatment of symptoms caused by prostatic hyperplasia; 53 patients took part in the study, which was carried out over a 3-month period. Urinary flow, micturition time, residual urine, frequency of micturition and a subjective assessment of the effect of treatment were all significantly improved in the treatment group. No untoward side effects were noted.
Valerian root contains two substances of special pharmacological interest--valepotriates and sesquiterpenes. The former, which has been used for standardization of the drug, is cytotoxic. The latter has no such effect. Both have sedative effects. A double blind test has been carried out on a preparation (VALERINA NATT) containing primarily sesquiterpenes. When compared with placebo it showed a good and significant effect on poor sleep (p less than 0.001). Forty-four percent reported perfect sleep and 89% reported improved sleep from the preparation. No side effects were observed.

Feeding rabbits with mycelium from Fusarium roseum is shown to produce infertility, the disappearance of aggressivity, and homosexual behaviour in males, while the effect of the females is only infertility. This mycelium contains the mycotoxin zearalenon which has effects similar to those of the female hormone. This fungus willingly grows on common crops and is consumed both by people and animals-although normally in very low doses. It cannot be ruled out that an increased growth of fungus, due to artificial fertilization, which then can be demonstrated to be found in food and consumed, can be a factor which increases the incidence of homosexual behavior in people.
Testicular capillary blood flow was studied in rats using laser Doppler flowmetry, in vivo fluorescence microscopy and videophotometric capillaroscopy. All the methods revealed rhythmical oscillations in testicular microcirculation with a periodicity of 4-10 c.p.m. In arterioles, capillaries and small post-capillary vessels, periods of continuous blood flow alternated with periods of no or very low flow. No visible leakage of dextran-150 was observed from the testicular blood vessels. Four, 8 and 16 h after an s.c. injection of 200 IU hCG the blood flow was continuous and there was leakage of dextran-150 from the microvessels to the interstitial tissue. Twenty-four and 32 h after hCG the blood flow pattern was again rhythmical, and at 32 h there was no leakage of dextran-150. This suggests that hCG induces changes in blood flow and transvascular fluid exchange in the testis, perhaps by altering smooth muscle activity at the arteriolar-level.
In a randomized prospective trial with hens it is shown that artificially fertilized forest blueberries are toxic. In a high dose (1.7 1/hens) the birds died within 72 hours (p less than 0.01). In a lower dose (0.33 1/hens) all survived but a lifelong change in the egg production occurred implying defects of the shells (p less than 0.0000001). It seems that fertilizers change the microflora in the ground, which releases soluble aluminiumnitrate which is absorbed in the berries. Aluminium poisoning has been found to have clinical significance in dialysis patients and probably also in senile dementia. In smaller doses it can be difficult to establish this relation as the symptoms can have a latency period of more than 10 years.
Thirty-five patients who had suffered from bronchial asthma for an average of 12 yr, all receiving long-term medication, 20 including cortisone, were subject to therapy with vegan food for 1 yr. In almost all cases, medication was withdrawn or drastically reduced. There was a significant decrease in asthma symptoms. Twenty-four patients (69%) fulfilled the treatment. Of these, 71% reported improvement at 4 months and 92% at 1 yr. There was a significant improvement in a number of clinical variables; for example, vital capacity, forced expiratory volume at one sec and physical working capacity, as well as a significant change in various biochemical indices as haptoglobin, IgM, IgE, cholesterol, and triglycerides in blood. Selected patients, with a fear of side-effects of medication, who are interested in alternative health care, might get well and replace conventional medication with this regimen.
Twenty-nine patients who had suffered from essential hypertension for an average of 8 years, all receiving long-term medication for hypertension, were subject to therapy with vegan food for 1 year. In almost all cases medication was withdrawn or drastically reduced. There was a significant decrease in systolic and diastolic blood pressure. A number of reported symptoms disappeared. There was a significant improvement in a number of clinical variables as well as a significant change in various biochemical indices such as urea, haptoglobin, cholesterol and triglyceride concentrations in blood. Subjectively all patients reported improvement. Selected patients, with a fear of side-effects of medication, who are interested in alternative health care might replace conventional medication with this dietary regimen.
Since 1969, seven orthopedic dissertations on the following subjects were submitted from the author's hospital. Postoperative use of special elastic bandages and oxyphenbutazone significantly decreased the frequency of thrombosis. Use of a randomized single-blind technique significantly improved chronic cases of lumbago by a new traction treatment called auto-traction. Two control groups were treated by bed rest and analgesics or routine physiotherapy measures. With a new mechanical device for tibial osteotomy, the precision of the angular change was significantly increased. In intracapsular hip fractures treated by Deyerle's technique, only 18% had necrosis of the femoral head after two years. By use of a new radiologic method for determining the stability of tibial fractures it was possible to determine a stability quotient; the quotient established the point at which the fracture could be mobilized without risk of delayed union or pseudarthrosis. In shoulder joint dislocations, three to four weeks of immobilization resulted in a recurrence rate equal to that of ordinary treatment in a sling. In a randomly selected group of people in Sweden, the incidence of shoulder dislocations was 2%. Forty percent of these patients had never seen a doctor. In Colles' fracture, the uptake of radioactive Tc-MDP was significantly increased by treating the fracture with electromagnetic fields. Arthroscopic percutaneous meniscectomies reduced the operation time normally required for an arthrotomy by an average of 30 minutes.
We have earlier reported that local testicular blood flow, recorded by laser Doppler flowmetry, shows large oscillations with a frequency of 5-10 min-1. In the present study it is proposed that the recorded oscillations represent mainly local microvascular blood flow variations rather than variations in total testicular blood flow or tissue movements. The reasons for this are: (a) Blood flow simultaneously measured at two separate sites showed oscillations with different frequencies. (b) A local subcapsular injection of room-tempered saline under one probe site eradicated oscillations under that probe but not under another adjacent probe. (c) When the testicular capsule was split open, recordings of blood flow continued to show oscillations. (d) The amplitude of the oscillations was rather large (peak to peak value about 50% of mean flow value). No movements of the testicular surface were seen. A 20 min continuous infusion of 0.4 microgram/min noradrenaline did induce a decrease in plasma testosterone concentration, but did not change the mean blood flow. However, the oscillations nearly completely disappeared during the infusion period. The present study also shows that laser Doppler flowmetry is a versatile method and the rat testis provides a suitable organ in the study of the origin and functional importance of these oscillations.
Laser Doppler flowmetry was used to continuously measure testicular blood flow in rats. The method was found applicable on surgically exposed testes. Regular oscillations in blood flow, with a periodicity of 8.6 +/- 0.7 cycles per minute (mean +/- SD), were observed in recordings from 22 to 23 rats. Clamping of the testicular artery reduced the blood flow signal to background values. Effects of catecholamines administered into the tail artery on testicular blood flow together with systemic effects on mean arterial blood pressure and heart rate were measured. It was found that noradrenaline as well as adrenaline caused a significant decrease in blood flow when 10 micrograms was injected. Only noradrenaline decreased the blood flow when 1 microgram was given. The large oscillations detected in the blood flow recordings disappeared quickly when 10 or 1 micrograms of both hormones was administered. It was concluded that catecholamines can exert rapid effects on testicular blood flow.
The pain conducting systems in the thorax will be described. The diagnosis angina pectoris is a symptom diagnosis. The common medical assumption is that these symptoms are caused by decreased circulation in the coronary vessels. The truth is that exactly the same symptoms can be the results of pathological changes in for example the oesophagus, the mediastinum and the thoracic spine. Our knowledge of referred pain and the pathways for pain conduction permits us nowadays to understand how pain symptoms can have different origins and still be experienced as identical.
Pain identical with angina pectoris can be released from the disks Th4-5, Th5-6 and Th6-7. Most common, cardiac-like pain is elicited from Th5-6. Physical stress such as lifting, rotation of the thorax and heavy work is especially symptom provoking. It is not unusual that movements in the bed during the night can provoke a pain attack. The pain can subside after a while but more often it continues for a long time in spite of rest. In all patients with symptoms of angina pectoris the thoracic spine should be examined. Pressing on the spinous process of the Th5 usually elicits an intensive and typical pain. The verification of the diagnosis is made by "specific" investigation of the spin and - ex juvantibus - by the momentary relief of pain after manipulation of the actual spinal segment.
