[Extrinsic stenosis of the ureter. Our caseload and review of the literature].

Obstructive uropathy is the common presentation course for various processes with an origin either intraluminar, parietal or extraluminar. A retrospective analysis of 41 cases of extrinsically originated obstructive uropathy seen in our Urology Unit from 1976 to 1991 was carried out. The cases were divided in three groups following an exclusively etiological criterion: 21.9% (9 patients) corresponded to primary retroperitoneal fibrosis; 51.2% (21 patients) to tumoral ureteral obstruction; and 26.8% (11 patients) to non-tumoral processes. For each division established, the features of clinical presentation, performance of the various diagnostic procedures used, and different therapy approaches are described also including a review on the current literature.

[Intravesical echography. II. Staging].

We evaluated 137 patients with bladder cancer submitted to surgery in order to determine the usefulness of intravesical ultrasonography (IVU) in tumor staging. The characteristic scan images were defined for each tumor stage. The application of the ultrasound patterns yielded an accuracy rate of 91% for tumors Ta"1 and 71% for T4. Based on the results of this study, IVU does not appear to be useful as an intraoperative monitoring technique.

Polyorchidism discovered as testicular torsion associated with an undescended atrophic contralateral testis: a surgical solution.

We report a case of polyorchidism presenting as torsion of the supernumerary testis associated with an undescended atrophic contralateral testis. Biopsies of both descended testes revealed a normal histological pattern with active spermatogenesis. After the testicular torsion was corrected, one of the supernumerary testes was placed in the contralateral empty hemiscrotum and the undescended atrophic testis was removed. The aesthetic and functional results were excellent.

Treatment of massive haematuria with aluminous salts.

Fifteen patients with massive vesical haematuria were treated with a 1% aluminium potassium sulphate solution in sterile distilled water, using continuous intravesical lavage with a double channel catheter. The haematuria was caused by vesical tumours in 13 patients, radiation cystitis in one and transurethral resection in one. Immediate side effects were few and none were noted in the long term, as judged by randomised biopsies from vesical mucosa. A complete response was noted in 66% of the patients, partial response in 15% and failure in 20%. This treatment is recommended for intractable bleeding from radiation cystitis and bladder tumours.

Squamous cell carcinoma of the renal pelvis associated with hypercalcemia and the presence of parathyroid hormone-like substances in the tumor.

A case of hypercalcemia associated with documented squamous cell carcinoma of the renal pelvis without bone metastasis is reported. High levels of cross-reacting parathyroid hormone-like material in serum and its presence in tumoral tissue were detected. To our knowledge this is the fourth case described in the literature and the first in which tumoral production of parathyroid hormone-like material is demonstrated.

Testicular malacoplakia.

We report the twentieth case in the world literature of testicular malacoplakia. This lesion occurs mainly in middle-aged men, appearing clinically as epididymo-orchitis or enlargement of the testicle with fibrous consistency and some soft areas. Malacoplakia is now considered a nonspecific inflammatory granulomatous lesion subsequent to an infection by gram-negative bacteria. In our case Escherichia coli was cultured in the urine and testicular tissue. The ultrastructural study shows the histiocytic nature of the cellular infiltrate as well as the morphology of Michaelis-Gutmann bodies. Bacilliform structures were not found. The cellular and humoral immunological studies did not show alterations.

The value of ultrasonic scanning in the differentiation of acute post-transplant renal failure.

In 43 renal transplant patients studied by ultrasonography there were 32 episodes of acute rejection and 17 of acute tubular necrosis. The size of the kidney increased in all cases. However, in 75 per cent of the patients with acute rejection the central echoes of the renal sinus increased conspicuously, while in 77 per cent of those with acute tubular necrosis the echogenicity of the central sinusal complex decreased. These findings may be expressions of different pathological lesions at the renal sinus level in patients with acute rejection and acute tubular necrosis, which would modify the acoustic interphase. Therefore, we have found panniculitis, vasculitis, thrombosis and inflammatory cellular infiltration of the urinary collecting systems to be significant in patients with acute rejection but not in those with acute tubular necrosis. Ultrasonography can be an adjuvant instrument for the sometimes difficult differential diagnosis between acute tubular necrosis and acute rejection immediately after transplantation, and also for the diagnosis of acute rejection whenever it appears.

[Retroperitoneal malignant xanthogranuloma. 1 case and a literature review].

A case is reported, the 33rd to be published in the literature, of malignant retroperitoneal xanthogranuloma. The tumor involved 2 cm of the right ureter at the junction between the middle and upper thirds. The middle third of the ureter was removed and ureteral continuity established by end to end ureteral anastomosis. The patient died fourteen months later probably from metastases, though the presence of these was not confirmed.

[Vesicovaginal fistulas: correction using lyophilized dura mater].

Thirty three cases of surgically corrected vesicovaginal fistulae, from a total of 73 patients with iatrogenic or pathological urinary tract conditions examined and treated in our service, are presented in this paper. Both the etiology and different surgical techniques that can be used are reviewed, contributing the novelty use of freeze dried dura mater as the material to interpose for the correction of the fistulae by means of abdominal surgical techniques. The material was used in 6 patients, with recovery being obtained in all cases and achieving the fistulae closure with a single surgical procedure.
