Studies on quantitative morphology. XIV. Morphometry of connective tissue in normal and cirrhotic rat livers between accuracy and working expense.

The percentages of connective tissue in sections of normal and cirrhotic rat livers up to values of about 40% are measured by point counting with net point distances between 160 microns and 5,120 microns. For a test area of about 1 cm2 a suitable distance of 640 microns led to a sufficient number of data for an always acceptable accuracy, therefore the measuring time per organ was not more than 10 min for approximately 250-400 hits. In the case presented the coefficient of variation for 10 repeated measurements decreases from 26.9% for normal livers down to 3.5% for livers with about 40% connective tissue. An enlargement of the net point distance for this test area to 1,280 microns and about 100 or less hits results in higher coefficients of variation and inacceptable differences between the x min and the x max data during a series, which becomes more clear for lower percentages. A further enlargement of the net point distance is accompanied by a further inaccuracy, but an enlargement of the test area allows larger net point distances.

The nominal section thickness--importance of their correction for morphometry.

The real finite section thickness of histologic slices is one of the dominant artefacts in morphometrical studies. Investigations without correction of the section thickness error are not reproducible or comparable. On this basis we measured the real finite section thickness of histologic slices with a nominal section thickness of 2, 4 and 6 microns with a microinterferometrical method. We also estimated the size of the error of a fictive volume density and a fictive Sholpo-correction. On the nominal section thickness of 2 and 6 microns we determined significant differences between the nominal section thickness and the real finite section thickness. We found analogue differences in the estimation of the fictive volume densities and in the fictive Sholpo-corrections. We did not determine a significant difference between the real finite and the nominal section thickness for the nominal section of 4 microns. We also could not determine significant differences between the real and nominal fictive volume densities and Sholpo-corrections. The measurement errors are discussed, and a test of reproducibility was performed.

Autopsy

[Granuloma in Crohn's disease of the colon and rectum--a study of resection specimens].

The aim of the present study was to investigate frequency and distribution of granulomas in resected specimens of patients with well known Crohn's disease. Granulomas were observed in 57% (8/14) of all cases. Their distribution varied remarkably between different sections of the same resectional sample as well as between the patients. 11.6% of all granulomas were found within the lamina propria, 42.9% within the submucosa, and 45.5% within the muscularis propria. The mean value per cm muscularis mucosae was 0.85 +/- 0.82 granulomas. Caused by the heterogeneity in frequency and distribution there is no guarantee for a successful identification of this helpful parameter. During the last years the diagnosis Crohn's disease became more and more certain even in absence of granulomas. Therefore they are an useful but dispensable feature.

[Morphometry of the heterogenic distribution of tissue components].

If the structure elements are distributed unhomogeneously, it is possible that morphometric measurements of single sections lead to data which are in less agreement with the true relations in an organ. In such cases, systematic sampling should be better than random sampling and often a very few sections with approximately equal distances are sufficient for measurements with an acceptable accuracy. This was demonstrated in examples of thyroid and mammary glands, cirrhotic livers and heart biopsies.

[Clinically unrecognized colorectal cancers? An autopsy study].

The aim of our study was to discover pathomorphological features of clinically undetected colorectal carcinomas in autopsy material. Out of 400 colorectal carcinomas from the periods of 1960 to 1966 and 1981 to 1983 126 (32%) of all carcinomas remained clinically undetected during lifetime of the patients. In 1/3 of these cases the clinician did not suggest a malign neoplasm, in the other 2/3 a malignant neoplasm of unknown or false origin was assigned. 44% of the right-sided colon carcinomas, 36% of the left-sided colon carcinomas and 22% of the rectal carcinomas remained clinically undetected. At the time of autopsy in 10 cases (13%) a second malignant neoplasm far from the colorectal region could be demonstrated. In 104 cases (82%) the colorectal carcinoma was the cause of death. 18% of the colorectal carcinomas were an additional finding at autopsy, independent of the cause of death.

Studies on quantitative morphology. XIII. Heterogeneity in the mammary gland of rats.

In the rat mammary gland, the distribution of tissues forming this organ is not homogeneous. In order to get representative information about the percentual distribution of lumina, epithelial, adipous and connective tissues it is necessary to take serial sections from the whole organ. This is a very strenuous procedure and is defensible at most in small organs. We demonstrated that, in general, 2-4 histological sections are satisfying all demands when they are taken at distances of about 600-1,000 micron. The percentages of glandular constituents are appropriately ascertained by using a net point system with distances of about 600 micron between single points, provided there is sufficient number of hits. For special purposes, e.g. the recording of drug effects, it is advisable to reduce the distances of step sections to 400-500 micron with net point distances of about 200 micron. Here we found a good compromise complying with both accuracy of results and expenditure of measuring time.

Morphometric investigations on endocrine glands. VI. Changes in rat pituitary glands after ovarectomy and a single injection of estradiol benzoate once per cycle period.

The application of 0.7 microgram estradiol benzoate to ovarectomized rats twice a week (about once per cycle period) causes a return of pituitary weight to values comparable with untreated animals. In contrast, such dosages are ineffective to suppress the enlargement of gonadotropic cells after ovarectomy. After gonadectomy of rats an increase is observed both in number and in volume of the so-called gonadotropic (PAS-positive, alcian blue-negative) cells in their pituitary glands. Natural or synthetic steroidal compounds with estrogenic, androgenic or gestagenic effects may prevent or reduce such changes (review Kramer et al. 1968). Analogous results are seen after administration of the non-steroidal methallibure (Paget et al. 1961). A daily unit dose of 0.5 microgram estradiol seems to be sufficient (Hohlweg and Dohrn 1932; review Romeis 1940). The estrogen level is a function of cycle stage with maximum and minimum, therefore it should be tested, whether or not a single injection of an equal dose will inhibit the development of castration cells.

Morphometric investigations on endocrine glands. VII. The influence of some estrogens on the formation of castration cells in the anterior pituitary of rats.

After ovarectomy there are observed changes in body weight increase, uterus weight and area size of the gonadotropic cells in the adenohypophysis. A replacement by estrogens may diminish such variations, but their extent is different for each of the single parameters; therefore no high correlation coefficients resulted in calculations on the basis of all data of a test. The administration of estradiol benzoate, estriol and estrone twice a week (about once per cyclus period) caused moderate changes in body weight increase and marked differences in uterus weights, but in the tested doses no alteration in the cell size of the gonadotropes, compared to ovarectomized animals without further treatment. Daily application of different doses of estradiol benzoate or estrone signify a higher efficacy of the first steroid. Different treatment conditions - starting shortly after excision of the ovaries or one week later - yielded only slight divergencies. Progesterone injected together with estrone caused no further significant deviations. Mestranol had only a moderate effect on the gonadotropes, but the uterus weight may increase under such a replacement up to values similar to those in untreated animals. On the other hand, there was observed a marked shrinking of the gonadotropes following higher doses of estriol, but in this case the uterus weight was less affected. In relation to equal doses of estrone, the uterus weights are similar following estrone cyanate, but in contrast to the parent compound, the effect on the gonadotropes was only low.

Studies on quantitative morphology. XI. Morphometrical determination of connective tissue in rat livers.

Even under similar experimental conditions, the distribution of connective tissue in cirrhotic rat livers may be more or less heterogeneous. Although there seem to be no extreme differences between the lobes, it is advisable to measure several step sections with equal or approximately equal distances instead of 1 section only. Presupposing a sufficient number of hits, all net point distances up to 0.6 mm do not influence the accuracy to a greater extent.

Studies on quantitative morphology. XII. Distribution of thyroid gland compartments in an activated rabbit thyroid gland.

A series of longitudinally cut step sections with a distance of 50 microns of a rabbit thyroid gland was measured by point counting for the relative tissue elements colloid, epithelium, and interstitium. Furthermore, the ratio epithelium:colloid was computed, which was somewhat high, indicating an activated stage. Higher colloid data were found in the outer regions and insignificantly lower epithelium values. Neither the volume data nor the distribution of the structure elements show any difference between both of the lobes. The degree of heterogeneity was comparably low, but only about 30% of all data for the relative colloid percentages or for the ratios are enclosed in a range of +/- 5% in relation to the most accurate value from all hits all over the gland, and about 20% of the ratio data are crossing the +/- 20% borderlines. It was calculated that measuring of not more than 5 sections with approximately equal distances with a very few 10(3) hits yields values of an acceptable accuracy for all parameters and this with a reasonable time expense.

Studies on quantitative morphology. IX. Component distribution in the thyroid gland of a hamster.

Morphometric investigations were performed on equidistant stage sections of a hamster thyroid gland, which led to the following conclusions: In sections of the outer regions there are observed higher relative colloid percentages and lower data for the epithelium in relation to the values of sections of the middle part. Therefore the ratio epithelium: colloid is higher in the central region. There are high negative correlation coefficients for the comparison of these main constituents, but low values are calculated between the relative interstitium percentages and the epithelium: colloid ratio. Both of the lobes of the thyroid gland showed different volumes, but nearly the same composition. Up to a section distance of approximately 500 micron most of the relative colloid percentages of bodies, constructed by all requisite section areas and the corresponding section distances, are laying in a range of +/- 5% in relation to the value, which was calculated from all sections with a distance of always 20 micron. For the epithelium percentages such a borderline was found near 400 micron. A sharper criterion is the ratio epithelium: colloid. All data beneath a section distance of 200 micron are lying in a +/- 5% range, since +/- 10% borderlines are not exceeded up to 500 micron. This corresponds to about 5 sections per lobe in this example, which were measured in less than half an hour.

Studies on quantitative morphology. VIII. Model-type calculations for thyroid gland morphometry.

A sphere with a nucleus and a shell each with different tissue component distributions was used as an approximative model for the thyroid gland. Noticeable divergencies are often seen between the composition of the total body and single parallel sections through such a sphere, but there is a changing region around the middle of the sphere radius, in which coincidence is found between the distributions both in the model-type gland and in such a section. In practice the location of this places is often not known precisely enough and therefore much more accurate results are obtained in an acceptable processing time by measurements of a few slides with approximately equal distances.

Morphometric investigations on endocrine glands. VI. Point counting in th thyroid gland.

The component distributions in a dog thyroid section are found to be inhomogeneous in a different degree. Therefore special conditions are needed in morphometrical research of such tissues to get accurate results, e.g. a sufficiently large area. The net point distance may reach 400 microns for test series, and 1,000 or more hits are enough for an acceptable representative value. The often observed zonal heterogeneity for the colloid and epithelium is diminished and blurred if the relative colloid percentage is high. The interstitium values show an anisotropic distribution in the gland. The epithelium: colloid ratio seems to be higher around larger areas of interstitium in relation to the data of the total section, but no high correlation between these both parameters was observed.

Studies on quantitative morphology. VII. Distribution of interstitium in the thyroid gland.

The variability of interstitium percentage values in dog thyroid glands is high, especially in comparison with data from smaller fields. The distribution is generally of the isotrope type, sometimes superimposed by a slight zonal heterogeneity. The low extended interstitium is characterized by a comparatively homogenous arrangement. Whether a net point distance of more than 100 micrograms may cause little deviation from lower values than the true percentage is in most cases not of practical consequence. Often areas larger than 5 mm2 must be measured, or even total sections. Under consideration of the time spent for measuring this can be done only with large net point distances. Low correlation coefficients are observed in these examples for the comparison epithelium: interstitium; and the ratio colloid: epithelium seems to be not always influenced by the interstitium content.

Studies on quantitative morphology. VI. Morphometry of colloid and epithelium in the thyroid gland.

The colloid and epithelium percentages in the thyroid gland are characterized by a noticeable variability and a zonal heterogeneity; in the border region an enriching of the colloid component is observed and in the central part a higher epithelium concentration. Studies with different net point distances and measuring areas led to the following conclusions: provided that a sufficient number of points are attached, a distance of 250 micrometers may be used for common tests; the prolongation of the measuring time for lower distances than 100 micrometers is often not accompanied by an essentially better accuracy. Measuring areas should not be too small, even between fields of 20 mm2 differences of the few per cents may be observed in the same slide. The polynomial regression curve for the comparison of epithelium and colloid data seems to be influenced by the interstitium values: up to about 30% colloid an increase of the epithelium values is seen, since a further growing of the colloid percentages is connected with a decline of the epithelium percentages.

[Testicular biopsies in male diabetics].

Histological exazerbations in 17 diabetics who underwent a biopsy of the testicles and 6 control persons with healthy metabolism could increasedly prove findings deviating from the normal in male diabetics. Disturbances of the germinal epithelium of slight to middle degree are in the foreground, which must not be overvaluated in their effect on fertility. In the relatively frequent childlessness in the marriages of diabetic males disturbances of the potentia coeundi apparently play an important role.
