Pharmacological management of cancer pain. A guide for the health care professional.

Cancer pain continues to remain a significant problem for oncology patients treated in both the university and the community setting. Nursing knowledge regarding pain management has advanced significantly in recent years. This article provides health care professionals with current factual information on cancer pain management. The information focuses on the emotional aspects of cancer pain, specific principles of analgesic management, opioid equianalgesics, multiple approaches to opioid administration, management of unwanted side effects, and a description of inappropriate drug therapies.

Training general practitioners to teach psychiatric interviewing skills: an evaluation of group training.

Group teaching in problem-based interviewing based on video and audiotape feedback of the doctor's own consultations significantly improved the ability of experienced general practitioners to teach psychiatric skills to their trainees. When the GPs were randomly allocated to one of three further training experiences--video feedback of their tutorial sessions, discussion about how to teach and no further teaching, there were very few differences between the groups. The greatest impact on improving teaching skills was brought about by watching their own consultations in a group feedback setting.

Clinical Competence

Surgical management of severe coarctation of the aorta in the first month of life.

Between March, 1978, and April, 1982, 48 neonates under 4 weeks of age were operated upon for severe coarctation of the aorta in our unit. Of these, 36 babies were in severe congestive cardiac failure and 11 had renal failure. Angiocardiography and echocardiography demonstrated intracardiac lesions in 29 patients. All had a patent ductus arteriosus. Surgical relief of the coarctation was effected within 48 hours of diagnosis. Those in severe cardiac and renal failure were intubated and ventilated in the intensive care unit before the operation. Continuous monitoring of the central venous and arterial pressures and rectal and skin temperature and periodic checks on the acid-base status were instituted, and acidosis was corrected carefully with a 4.2% solution of sodium bicarbonate. A dopamine infusion (4 to 8 micrograms/kg/min) was used to assist the renal and cardiac functions. Five babies were treated with prostaglandin E for 24 to 46 hours before the operation. Forty-five patients had subclavian flap repair of the coarctation and the other three had repair by polytetrafluoroethylene (PTFE) patch angioplasty. The patients were ventilated for 1 to 8 days postoperatively. Dopamine support was given to all the immediate postoperative period. Three patients with associated cardiac lesions died in the first 48 hours after operation. Three other patients died on the sixth, eighth, and eleventh postoperative days of noncardiac causes. Of the 42 survivors, three required revision of the repair because of restenosis 3 to 18 months later. Three patients with ventricular septal defect had closure of the defect within 8 months after birth. We believe that coarctation of the aorta presenting soon after birth should be treated by a combination of active resuscitation and early surgical relief, and that continuous monitoring for early detection and correction of hypoxia and acidosis is essential to achieve satisfactory results.

Corneal Transplantation

Linear dichroism of rhodopsin in air-water interface films.

Air-water interface films of purified cattle rhodopsin and defined phospholipids are formed by the osmotic lysis of reconstituted membrane vesicles. The interface films thus formed consist of a phospholipid monolayer containing vesicle membrane fragments. Rhodopsin molecules at the interface are restricted within the membrane fragments where they are spectrophotometrically intact and capable of undergoing photoregeneration and chemical regeneration. Multilayers of up to 8 layers can be built from these interface films. The visible absorption band of rhodopsin in these multilayers is linearly dichroic. Quantitative analysis of the linear dichroism reveals that the dipole moment of transition of the retinal chromophore in rhodopsin forms an angle of 15 degrees +/- 4 degrees with the plane of the membrane fragments in the interface film. This orientation of the chromophore relative to the plane of the membrane is essentially the same as that observed in the intact retina. Thus, the orientation of rhodopsin in the interface films is similar to that in the intact disc membranes.

Pelvic inflammatory disease in the United States. Incidence and trends in private practice.

The National Ambulatory Care Survey, conducted by the National Center for Health Statistics (Rockville, Md.), provides national estimates for conditions diagnosed by office-based private practitioners in the United States. The Venereal Disease Control Division of the Centers for Disease Control (Atlanta, Ga.) obtained data from the National Ambulatory Care Survey for the years 1973-1976 and analyzed the epidemiology of pelvic inflammatory disease in women who sought care from private physicians. More that 7.5 million office visits for pelvic inflammatory disease were made over the four-year period (average, 1.89 million visits pe year). All races other than white bear a disproportionate burden of pelvic inflammatory disease. Since the advent of the National Gonorrhea Screening Program in 1972, the number of visits to private physicians for pelvic inflammatory disease has apparently declined.
