Effectiveness of the cytologic examination of pure pancreatic juice in the diagnosis of early neoplasia of the pancreas.

BACKGROUND: The early diagnosis of pancreatic carcinoma through the use of traditional radiographic or ultrasonographic methods is extremely difficult. METHODS: To detect an early and potentially curable cancer of the pancreas, endoscopic retrograde pancreatography (ERP) and aspiration cytology of pure pancreatic juice were performed in 295 consecutive patients who had symptoms or findings that suggested pancreatic disease but in whom there was neither a pancreatic mass nor ductal stenosis. RESULTS: Positive cytologic results were obtained in 12 patients (4%). With the aid of intraoperative cytodiagnosis, all 12 early neoplasms of the pancreas were successfully resected. Of these 12 resected specimens, 4 were adenocarcinoma with minimal invasion, 3 were in situ adenocarcinoma and 5 were marked atypia. All 12 patients were alive with no evidence of recurrence for an average of 32 months after surgery. The 283 patients who had negative ERP-cytology results were observed, but no further cases of pancreatic cancer were found. CONCLUSIONS: Because ERP-cytology is simple to perform, safe, and reliable, it is useful in the diagnosis of patients who have early neoplasm of the pancreas.

Endoscopic ultrasonography in diagnosis and staging of pancreatic cancer.

The accuracy of endoscopic ultrasonography (EUS) for diagnosis of pancreatic cancers was evaluated in consecutive 232 patients with possible pancreatic cancer, and that for assessment of their locoregional spread was evaluated in 28 patients with pancreatic cancer subjected to pancreatectomy, in comparison with the accuracies of transabdominal ultrasonography (US) and computed tomography (CT). EUS was found to be significantly more accurate than US or CT and was especially useful for detecting small pancreatic cancers of less than 2 cm in diameter. With EUS, pancreatic cancers could be detected as a hypoechoic mass with a relatively unclear margin and irregular internal echoes. EUS was also more sensitive than CT and US for detecting venous and gastric invasions: it was more useful for detecting direct invasion of pancreatic cancers when the tumors were less than 3 cm in diameter. These findings indicate that EUS is an accurate method for diagnosis of pancreatic cancer and assessment of their locoregional spread and is particularly useful for detecting small tumors.

Disaccharide analysis of the skin glycosaminoglycans in chronically ultraviolet light-irradiated hairless mice.

The alteration of main disaccharide units in the skin of hairless mice (HOS:Hr 1) after chronic and repeated ultraviolet light (UV) radiation was investigated using high performance liquid chromatography after labeling with 1-phenyl-3-methyl-5-pyrazolone. The total amount of main disaccharide units increased by UVA irradiation at the 24th and 36th weeks in comparison with the control. At the 36th week UVA significantly increased hyaluronic acid-derived delta Di-HA (HA). Dermatan sulfate-derived delta Di 4S (DS) and chondroitin sulfate-derived delta Di-4S (CS) increased at the 36th week, although not statistically significant. The total amount of main disaccharide units was increased significantly by UVB irradiation at the 24th week as compared with the control. Concerning the compositional change in main disaccharide units after a 36-week repeated exposure, the decrease in delta Di-HA(HA) and the increase in delta Di-4S (DS) were found in the order of control, UVA- and UVB-irradiated groups. These results, for the first time, indicate the precise alterations of glycosaminoglycans, both in the total amount and in the composition, confirming the previous histochemical findings. This disaccharide analysis should provide a useful method to examine the biochemical changes of skin glycosaminoglycans in photoaging.

Ulcerative colitis associated with preceding systemic lupus erythematosus.

We report a 50-year-old female patient who developed ulcerative colitis 31 after years being diagnosed with systemic lupus erythematosus. The overall clinical evaluation of her SLE activity differentiated ulcerative colitis from lupus colitis. Since the association of idiopathic systemic lupus erythematosus with ulcerative colitis has rarely been reported, the combination of these two diseases may be coincidental.

Measurement of skin elastic properties with a new suction device (I): Relationship to age, sex and the degree of obesity in normal individuals.

We measured skin elastic properties (distension ability, retraction ability, and the skin elasticity) of 4 different body sites, finger, hand, forearm, and chest, in 191 normal individuals by means of a new suction device and investigated the correlation of skin elastic properties with sex, site, age, and the degree of obesity. Neither sex nor the degree of obesity were correlated with skin elastic properties. The skin elastic property of the chest was significantly higher than those of the other three sites. Decrease in the skin elasticity of the hand, forearm and chest were associated with aging. This simple and non-invasive device can provide an objective method for evaluating skin elastic property.

Association of p53 gene mutations with short survival in pancreatic adenocarcinoma.

Mutations of the p53 gene have been found in a variety of human cancers and are implicated in the biologic functions of cancer. To investigate the clinical implications of p53 mutations in pancreatic adenocarcinoma, we examined the association of mutations of the p53 gene with patients' prognosis. Single-strand conformational polymorphism analysis and direct DNA sequencing were used to detect p53 gene mutations in 37 pancreatic adenocarcinomas. p53 gene mutations were detected in 16 (43%) of the 37 pancreatic adenocarcinomas. Direct sequencing did not reveal preferential clustering at any specific codon. There was no significant association of the presence of p53 gene mutations with histologic types, extent of tumor invasion, the presence of lymph node metastasis, or tumor stage. Univariate analysis showed that survival of patients with p53-gene-mutated tumors was significantly poorer than that of patients with p53-gene-nonmutated tumors (P = 0.02). Cox's multivariate analysis of ten clinicopathologic features including p53 gene mutations revealed that presence of p53 gene mutations (P = 0.026) and curativity of operation (P = 0.014) were independent predictors of survival. Furthermore, the survival of patients with p53-gene-mutated tumor was significantly poorer than that of patients with p53-gene-nonmutated tumors, both in patients who underwent curative operation (P = 0.04) and in patients who underwent non-curative operation (P = 0.01). These results suggested that mutations of the p53 gene might play an important role in cancer aggressiveness and could be a clinically useful predictor of prognosis in patients with pancreatic adenocarcinoma.

Occupational scleroderma. A 17-year follow-up study.

Two patients with a scleroderma-like disorder induced by epoxy resins were reported from the Department of Dermatology, Gunma University School of Medicine, Japan in 1980. Here, we describe the clinical and laboratory characteristics of these patients after 17 years' follow-up from 1976 to 1993. Their systemic manifestations and indurated sclerotic skin changes disappeared within 5 years. No internal organ involvement has developed during the period of follow-up. On routine laboratory tests, no abnormalities have been found in the peripheral blood or in the blood chemistry or serology. Histological examination revealed atrophy of the dermis and restoration of the normal pattern of fine collagen bundles, when compared with the previous skin biopsy specimens. This scleroderma-like disorder induced by epoxy resins is considered to be different from systemic sclerosis: it has an acute onset and a fairly good prognosis, and does not show involvement of the internal organs.

The emergence of Sézary cells during the treatment of cutaneous T-cell lymphoma.

A 52-year-old man with cutaneous T-cell lymphoma presented with Sézary syndrome-like features including exfoliative erythroderma, lymphadenopathy and leukaemic change without the presence of Sézary cells. Southern blot hybridization analysis revealed T-cell receptor beta-chain gene rearrangements, both in the peripheral blood and in the lymph node. Atypical large cells with convoluted nuclei (Sézary-like cells) first appeared during combined chemotherapy. Although Sézary syndrome is characterized by its particular manifestations and the presence of Sézary cells in the peripheral blood, the clinical significance of Sézary cells has been questioned. It still needs to be decided whether or not the Sézary cell is malignant.

The role of pancreatic islets in experimental pancreatic carcinogenicity.

Our previous studies have suggested that the presence of intact islets is essential for the induction of pancreatic exocrine tumors in the Syrian hamster model. To validate this, we investigated the effect of the carcinogen, N-nitrosobis(2-oxo-propyl)amine (BOP) in hamsters, in which homologous isolated intact islets were transplanted into the submandibular gland (SMG). Freshly isolated pure islets from hamster donors were transplanted into the left SMG of 20 female host hamsters. Ten of these hamsters (group 1) received BOP (40 mg/kg) weekly for 3 weeks. Another 10 hamsters (group 2) were kept untreated. In groups 3 and 4 (10 hamsters each) the salt solution or isolated pancreatic ductal cells, respectively, was injected into the gland. In other groups (10 hamsters each) islets were transplanted into the peri-SMG connective tissue (group 5) or into the renal subcapsular space (group 6). Hamsters of group 1 (40 mg/kg, weekly for 3 weeks) as were group 7 hamsters, which served as BOP-treated controls. All BOP-treated hamsters developed pancreatic lesions. Similar hyperplastic and atypical ductal/ductular proliferation and in situ carcinoma were found in the SMG of many group 1 hamsters. No such lesions were found in the SMG, peri-SMG, or renal subcapsular space of the other groups. Islets appear to be involved in carcinogenicity of BOP. The mechanism is obscure.

[Effects of adjuvant surgical therapy for continuous regional therapy for advanced hepatocellular carcinoma].

In order to repeat the effective transarterial embolization (TAE) or regional chemotherapy, adjuvant surgical therapies were performed for 25 patients with advanced hepatocellular carcinomas (HCC) having collateral feeders. The procedures involved the reduction of tumors with collateral feeders, permanent decollateralization using silicone rubber sheeting of the liver (WRAP therapy), and combination therapies. The 1-, 3- and 5-year survival rates of the patients after the adjuvant surgery were 67%, 18% and 5%, respectively. The 3- and 5-year survival rates of the patients after the initial TAE or arterial chemotherapy were 57% and 12%, respectively. On the other hand, the 3- and 5-year survival rates of patients who had TAE alone (n = 443) were 29% and 14%, respectively. Consequently, this adjuvant surgical therapy may be a useful palliative treatment for HCC.

[Preoperative neoadjuvant chemotherapy with a combination of 5-fluorouracil, adriamycin and cisplatin (FAP) for advanced esophageal cancer invading trachea or main bronchus].

Conventional irradiation and systemic chemotherapy is scarcely effective for advanced esophageal cancer invading trachea or main bronchus. Therefore, to reduce the area of invasion and suppress distant metastasis, we have preoperatively treated 4 patients suffering from advanced esophageal cancer invading the trachea or main bronchus by neoadjuvant chemotherapy (FAP) as follows: 2 times every 4 weeks, CDDP 100 mg and ADR 50 mg on day 1 and continuous infusion of 5-FU 1,000 mg/day for 7 days. The response rate (PR) was 75% (3/4). In 2 of 4 patients (50%), no cancer cells except broad fibrosis were detected histologically in the region of the trachea or main bronchus suspected to be invaded. There was no severe complication. This FAP regimen is suspected to be useful chemotherapy for advanced esophageal cancer.

[The efficacy of several kinds of treatment for advanced hepatocellular carcinoma--a report of an over-five-year survivor by regional and systemic chemotherapy and adjuvant surgery].

We encountered a 53-year-old male patient with advanced hepatocellular carcinoma (HCC) uncontrolled by transcatheter arterial embolization (TAE). Numerous tumors with a huge one occupying the lateral segment were shown on abdominal ultrasonogram, computed tomogram and angiogram. The first TAE was ineffective for the lesions because of the development of collateral feeders. Lateral segmentectomy and, "wrapping therapy" for the liver remnant were performed, and catheters were put both into the hepatic artery and into the portal vein for regional chemotherapy. About a year after the procedure, anticancer drugs were administered. When tumor stains were found by following computed tomography or angiography, TAE was performed. The patient has survived for five years and four months. The combination of several kinds of treatment serves to improve the prognosis of patients with advanced HCC if the liver function is preserved.

[A case of occult cancer of pancreas which was appropriately resected by intra operative cytodiagnosis using a balloon catheter].

A new method of intraoperative cytodiagnosis was performed for one patient with occult neoplasms of the pancreas in order to locate the original site of cancer cells, using a balloon catheter. Cancer cells were detected in the pancreatic juice obtained from the pancreatic body only. The pancreatic body was removed, and the tail of the pancreas was anastomosed to the stomach. The post operative course was good without diabetes, and it was confirmed histologically that minimally invasive adenocarcinoma existed in the pancreatic body. Thus this procedure is beneficial because it can preserve as much pancreatic function as possible without losing the chance of cure by surgery.

Separation of nucleic acids by high-performance ion-exchange chromatography.

Separation of various nucleic acids was evaluated by high-performance ion-exchange chromatography on non-porous resin, TSKgel DNA-NPR. A 1 kb ladder DNA was studied as a model DNA on operational variables like flow rate, gradient time, temperature, sample load, etc.. As results, various DNA fragments were well separated within 15 min by 20 min linear gradient at a flow rate between 0.5 and 0.75 ml/min at room temperature while the resolutin was dependent on molecular weight of the sample. The relationship between sample load and its peak area was examined on polymerase chain reaction (PCR) product. The product was found to be quantitatively recovered even with nanogram loads. The detection limit was 3.8 ng at signal to noise level (S/N) of 3. This non-porous ion-exchanger also showed high resolution on separation of ther nucleic acids like transfer RNA, oligonucleotides (single-stranded) DNA.

Assessment of immunoassays for insulin in diagnostic tests for insulinoma.

An immunoassay for insulin using a monoclonal insulin-specific antibody has been marketed in Japan for 2 years. This new assay may yield different plasma insulin (IRI) values from those obtained by the conventional immunoassay because of its high specificity. In this study, we compared IRI values obtained by three different immunoassays, assays P, M1 and M2, the last two of which involved monoclonal antibodies. For normal subjects, similar IRI values were obtained from all three assays. In patients with insulinoma, IRI values from assay M1 were 7-63% of those from assay P. Assays M1 and M2 gave similar values. Proinsulin interfered with assay P but not with assays M1 or M2. Differences between the IRI(P) value and the IRI(M1) or IRI(M2) value were correlated with the concentration of proinsulin (r = 0.98). The IRI(P) value appears therefore to represent the sum of levels of insulin and proinsulin. We conclude, therefore, that an immunoassay with broad specificity (i.e. assay P) is better for screening for hyper-(pro)insulinemia. Immunoassays with narrow specificity (i.e. assays M1 and M2) may have benefits in some tests aimed at tumor localization.

Expression of autocrine motility factor receptor in colorectal cancer as a predictor for disease recurrence.

BACKGROUND: New biologic prognostic factors are needed to guide the treatment of patients with colorectal cancer. The prognostic value of the altered expression of the cell surface glycoprotein gp78, which has been implicated in tumor-cell invasion and metastasis as an autocrine motility factor (AMF) receptor, was evaluated. METHODS: The level of expression of AMF receptor gp78 was analyzed immunohistochemically in tumor specimens from 118 patients who had undergone surgery for colorectal cancer. Multivariate analysis was used to determine whether gp78 expression status was correlated with patient prognosis. RESULTS: Among the 118 patients studied, the 5-year survival rate for the 51 patients who had gp78-positive tumors was significantly poorer that that of the 67 patients who had gp78-negative tumors (49.8% vs. 89%). The incidence of gp78 positivity was correlated with tumor progression as reflected by histologic type, depth of invasion, lymph node metastasis, vessel invasion, and tumor stage. Among the 101 patients who underwent curative resection, the difference in disease free survival between patients with gp78-positive tumors and those with gp78-negative tumors was significant. This significant difference remained among patients who had Stage II tumors. Multivariate analysis with the Cox regression model indicated that gp78 positivity was a good predictor of disease recurrence, ranking with extent of tumor invasion (T classification) and lymph node status (N classification). CONCLUSIONS: Increased expression of gp78 is correlated with a high incidence of recurrence and, consequently, with decreased survival of patients with colorectal cancer. Expression of gp78 might be of prognostic value in these patients.
