[Intracavitary application of silics for the treatment of inflammatory-purulent processes].

The results of clinical application of Silics--new sorbent of polydirected action, based on highly disperse amorphous silica, were presented. Silics owes unique properties, which are manifested while its intracavitary application for treatment of inflammatory-purulent processes. Osmotic activity of sorbent is more than 500%, waterabsorbability--up to 1000%, it sorbates up to 800 mg/g of proteins, microorganisms--up to 10(10) of the colony forming units on 1 g. Silics promotes lysis of necrotized tissues. In clinical application of Silics in 146 patients with retroperitoneal space phlegmon in an acute destructive pancreatitis, in 23--with abdominal cavity abscess, in 34--with inflammatory-purulent pleuropulmonary diseases it was established, that this preparation promotes the intoxication severity lowering, the reduction of microbal sowing of purulent cavities and resistance of the wound microflora to antibotics. The securing of fragmentation of necrotized tissues and the fibrin deposits, lowering of the exudate viscosity rises efficacy of the inflammatory focus drainage. Application of Silics for the treatment of inflammatory-purulent processes permits to improve essentially its results on the account of lowering of mortality and the treatment cost, and the shortage of the patients stationary treatment.

[Effect of spore-free anaerobic microorganisms of pancreatic tissue in experimental acute pancreatitis].

The simulation model of an acute pancreatitis (AP), using translocation of microorganisms from intestine to ductal system and pancreatic tissue, was created in experiment on dogs. Interrelationships between pancreatic tissue and anaerobic microorganism in an AP were studied, using electron microscopy. Possibility of the microorganisms migration to pancreas from ductal system in early stage of AP due to enhanced reproduction of anaerobic microorganisms in duodenum was established. It is impossible to exclude the possibility of their invasion via the lymph and the blood flow into destructively changed pancreatic tissue in late terms of the disease.

[The effect of polysorb on the course of the wound process].

The peculiarities of a contact of the sorbent particles of a polysorb for medical purposes with the somatic cells have been studied. The destructive action of the preparation its capability to block the functionally active sites of a cellular membrane were revealed. The recommendations on clinical use of a polysorb have been developed.

[A twin drain].

A design for drainage is suggested consisting of connected tubes with openings. For continuous and long-term drainage the openings are made one opposite to the other on the contiguous sides of the tubes.

[Method for retroperitoneal drainage in destructive pancreatitis].

The authors developed a method for drainage of the retroperitoneal space in acute destructive pancreatitis, which includes retroperitoneal advancement of tubes to the pancreatic bed. To limit the spreading of the pyonecrotic process by creating an effective withdrawal of the exudate, the drains are introduced on both sides through the space between Toldt's fascia and the colon, and additional drains are also introduced into the paracolonic space limited by Toldt's and prerenal fascias.

[Antibiotic therapy of purulent-inflammatory diseases].

The analysis of 161 case record of the patients with purulent-inflammatory diseases was carried out. The dependence of duration of treatment on the age of a patient, character of pathology, body temperature, leukocytic count, presence of pathologic changes in the urine was established. The expediency to limit antibiotic therapy in purulent-inflammatory diseases is substantiated.
