Herpesvirus group antibodies in children with Hodgkin's disease.

During the period of three years ((1972-1974), serum samples from 60 patients (children and adolescents) with lympho-hematopoietic system diseases were examined for antibodies to all four human herpesviruses. Among these were 26 active Hodgkin's disease (AHD) patients and 6 HD patients with a minimum five years' remission. Simultaneously matched controls (age, sex) of AHD patients were examined. Antibody levels against the viral capsid antigen of Epstein-Barr virus (EBV/VCA) in AHD patients were significantly higher, with overrepresentation of higher titres (greater than or equal to 1:160), than in matched controls. The lowest EBV/VCA antibody titres were in the leukemia-non-Hodgkin's lymphoma patients. We could not prove any significant relationship between cytomegalovirus or herpes simplex virus type 1 antibody titres and AHD or any other disease of lympho-hematopoietic system. The varicella-zoster virus antibody titres in AHD patients were significantly higher than in matched controls. No significant differences in antibodies against EBV/VCA and the other human herpes viruses between the evolution and remission period of AHD patients could be detected. No differences in EBV/VCA antibody titres were observed between the healthy school-children aged 10 to 15 years who were and who were not in contact with a HD patient.

[Correlation of clinical and anatomical findings in childhood meningitis].

Clinical and anatomical findings of 76 prophylactically not irradiated children with acute leukaemia were compared. In the group of 17 children (22%) with clinically manifest symptoms of CNS involvement in the course of the illness leukaemic infiltrates were identified in the CNS at the autopsy and during histopathological examinations even in 70% in spite of clinical healing and normalization of the cerebrospinal fluid finding. In children who had either no clinical symptoms of CNS leukaemia (42 children) or only terminal symptoms in the last 24 hours of their life (17 children) leukaemic infiltrations were found in 25.4% of the cases. Non-myeloid types showed 44.5% of histopathological changes, myeloid types 22.7%. Clinical main symptoms and anatomical findings were correlated to the corresponding groups of patients and typical illustrations were added.

Fibrinolysis, fibrinogen and trhombin time in newborns.

Comparative experiments were made on blood samples collected with and without addition of a potent fribinolysis-blocking agent in newborn infants aged up to 12 h. Prevention of the in vitro fibrinolysis resulted in a diminished frequency of an abnormal euglobulin clot lysis time, but there was no significant difference in fibrinogen levels and in the frequency and degree of a prolonged thrombin time. These results indicate that the prolongation of thrombin time in newborn infants cannot be prevented by blocking the in vitro fibrinolysis. The significance of these findings is discussed with regard to the possible existence of an abnormal 'fetal fibrinogen'.

Prognostic criteria and staging in ALL in childhood.

On the basis of a retrospective review of 100 children with acute non-myeloid leukaemia the authors examined the effect on the prognosis of some initial features such as age, nodal and organ enlargement, initial white blood cell count, blast morphology and cytochemical characteristics, and platelet count. A remarkable correlation was found between age at diagnosis, initial white blood cell count and platelet count, and total survival and duration of remission. There was no significant correlation between PAS score and prognosis. The beta-glucoronidase reaction did not prove a useful index of prognosis. Adverse prognostic factors were an age below 2 or one above 10 years, a high initial white blood cell count and a low platelet count. The usefulness of attempts at staging in acute lymphoblastic leukaemia in childhood are discussed together with the predictive value of some features present at disgnosis.
