Hamartoma of the tongue: two case reports.

Hamartoma designates an excessive focal overgrowth of mature normal cells and tissues in an organ or tissue composed of identical cellular elements. Two cases of hamartoma at the back of the tongue are reported. Histologically they comprised fibrous tissue, fat, salivary gland lobules, and smooth muscle. The literature is reviewed and discussed.

Argon laser therapy of port-wine stains: effects and limitations.

One hundred and one patients (112 parts) with port-wine stains were treated by argon laser therapy. We strictly evaluated all patients according to color shift, scar formation, and pigmentation. We observed good to excellent results in 44 percent of patients, and less satisfactory results in 56 percent. The relationship between the effects of treatment and several factors, such as patients' age, anatomic site, and histologic type of the lesions, were investigated. More satisfactory results tended to be obtained in patients with neck lesions and with histologic type III lesions (dilated type). To date we have not discerned any definite criteria for precisely predicting the response of port-wine stains to argon laser therapy. Consequently, the performance of a test seems to us to be the best way of discovering the response and course of healing of an individual port-wine stain after laser therapy. Thirty-six consecutive patients were studied to determine whether compression and cooling would improve the outcome of argon laser therapy. The use of this method did not produce any noticeable effects.

Massive edema of the ovary associated with hydrothorax and ascites.

A 29-year-old woman with massive edema of the ovary which was grossly mistaken for neoplasia because it was associated with large volumes of both pleural effusion and ascites is described. The ovarian mass measured 10 X 10 X 8 cm and weighed 850 g. Total hysterectomy and bilateral salpingo-oophorectomy were performed. Microscopically, the ovary consisted of diffusely edematous stroma enveloping nests of hyperplastic theca cells or showing a loose myxomatous contexture. However, no endocrine activity of the ovary was demonstrated on the basis of laboratory endocrine values obtained by radioimmunoassay techniques.

[Clinical studies of sulbactam/cefoperazone in the field of obstetrics and gynecology].

Sulbactam (SBT), a new beta-lactamase inhibitor, in combination with cefoperazone (CPZ) was studied for the clinical evaluation in the field of obstetrics and gynecology. SBT/CPZ was given to 6 cases with the following infections; 4 of pyometra, 1 of Douglas-abscess and 1 of pelvic cellulitis. Five cases were old patients accompanied by cancers, and 2 cases were refractory to other antibiotics therapy. The clinical efficacy was excellent in 1 case, good in 3 cases, poor in 2 cases. No side effects nor abnormal laboratory findings associate with the agent were observed.

[Endocrine profile and electron microscopic study of ovarian androblastoma].

The endocrine profile of a 23-year-old woman with an androblastoma of the right ovary and the results of electron microscopic observation of the tumor are presented. The calculated ratio of testosterone in testosterone intraoperative peripheral vein blood, right ovarian vein blood, and right ovarian tumor fluid was 1:1.4:4.0. The peripheral levels of hormones before right salpingo-oophorectomy were testosterone, 6.26 ng/ml; dehydroepiandrosterone, 17.80 ng/ml; androstenedione, 1.61 ng/ml; and cortisol, 16.5 micrograms/ml, and the corresponding levels at 14 days after surgery were 0.50 ng/ml; 13.80 ng/ml; 1.27 ng/ml; and 15.2 micrograms/ml, respectively. The tumor was responsive to hCG, resulting in a marked increase in the serum concentrations of testosterone, androstenedione, and dehydroepiandrosterone (3.7 times, 5.3 times, and 2.4 times, respectively). Preoperatively, an elevated basal level of luteinizing hormone (LH) and a normal basal level of follicle-stimulating hormone (FSH) (high LH:FSH ratio, 6.6) were found. The electron microscopic findings for the tumor revealed Leydig cells, Sertoli cells, transitional cells with features of both Sertoli cells and Leydig cells, and dark cells. The dark cells had features similar to dark cells of normal ovarian stroma.

[Fundamental study on tissue distribution of ceftriaxone in the field of obstetrics and gynecology].

Fundamental study on tissue distribution of ceftriaxone (Ro 13-9904, CTRX), a new cephalosporin parenteral antibiotic, was studied and the following results were obtained. CTRX had been administered by intravenous drip infusion with 1 g to 9 cases who received simple total hysterectomy. The level of CTRX in the cubital venous serum and uterine arterial serum was determined as well as the tissue concentration in the oviduct, ovary, endometrium, myometrium, cervix uteri and portio vaginalis. The level in the oviduct and portio vaginalis was generally high, although it was observed only in a few cases. The variation of the level in the myometrium was large. CTRX is expected to be clinically useful considering the fact that its half-life time is 8.5 hours, which is longer than that of existing cephalosporin antibiotics.

Digital subtraction angiography for the evaluation of persistent uterine trophoblastic disease.

In three patients with persistent uterine trophoblastic disease, follow-up after primary chemotherapy was undertaken by digital subtraction angiography. It was found that the angiographic appearance of the intramural uterine lesions was comparable with that obtained by conventional pelvic angiography. Digital subtraction angiography may be applicable as an easy, safe, and valuable diagnostic method for gestational trophoblastic disease in place of conventional pelvic angiography because it is a relatively noninvasive procedure that can be performed on outpatients.

[Electron microscopic study on the angioarchitecture of glomeruli in experimental toxemia of pregnant rabbits].

The present paper reports studies on the change in the angioarchitecture of the glomeruli in induced hypertensive pregnancy in rabbits. Experimental toxemia of pregnancy in late pregnancy in rabbits was induced by constriction of the aorta below the renal arteries, followed by estradiol administration. The results are as follows: 1) Constriction of the aorta in the pregnant rabbits on the 21st day of gestation significantly elevated the blood pressure and resulted in intrauterine growth retardation in 88.9% of the living fetuses. 2) When the pregnant rabbits underwent constriction of the aorta and estradiol treatment, all the fetuses were in a state of intra-uterine death. 3) The transmission electron microscopic findings of the glomeruli revealed swelling of endothelial cells with vacuolization and vacuolated mesangial cells. 4) The angioarchitecture of the glomeruli was observed by scanning electron microscopy. The enlargement of the endothelial cells was found to reduce the size of cavities of vessels.

[Immunochemotherapy of recurrent and advanced uterine and ovarian cancer using cisplatin].

A combination immunochemotherapy regimen containing cisplatin (20 mg, days 1-5), peplomycin (20 mg, days 2, 9, 16), +/- vinblastine (5 mg, days 1, 2), and picibanil (3-5 KE/week) was performed in twelve patients with advanced or recurrent uterine and ovarian cancers under intravenous hyperalimentation (IVH), except one patient receiving peplomycin by a continuous infusion method using IVH bag (10 mg/day for 5 days). This regimen was repeated every three weeks. Five (71.4%) of seven evaluable patients showed partial response. No patients yielded the complete disappearance of disease. No severe and lethal pulmonary or renal dysfunction occurred and all was well tolerated. In a regimen without vinblastine, myelosuppression, especially thrombocytopenia, occurred later compared to the regimen including vinblastine.

[Fundamental and clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Fundamental and clinical studies on T-1982 (cefbuperazone), a new cephamycin antibiotic, were carried out in the field of obstetrics and gynecology, and the following results were obtained. In a fundamental study, T-1982 was given to 7 patients with postpanhysterectomy at a dose of 1 g by intravenous drip infusion, and T-1982 concentrations in uterine artery and elbow vein serum and each uterine tissue (fallopian tube, ovary, endometrium, corpus uteri, cervix uteri and portio) were serially determined. The concentration in the endometrium (though the range of values was wide) was found higher than that in the other tissues. In a clinical study, T-1982 was given to 5 patients; 2 patients with parametritis and each one patient with right salpingitis, pyometra and metroendometritis. The efficacy rate was 80%. No side effects nor remarkable abnormalities in hematology, urinalysis and renal and hepatic function were observed during the therapy.

[Combined administration of futraful and esquinone for the treatment of advanced ovarian cancer].

Thirty-seven patients with advanced ovarian cancer were treated with futraful (FT-207) and esquinone (CQ). These patients had infiltrated tumors to the pelvic and peritoneal cavity and also distant metastasis, so they were not operated perfectly. There were evaluable 12 patients out of 37. FT-207 + CQ is an effective drug combination against advanced ovarian cancer with an overall response rete of 40.9%, a complete response rate of 4.5% (1/22), partial response rate of 36.4% (8/22), no change rate of 27.3% (6/22) and progressive rate of 31.8% (7/22). Patients with adenocarcinoma showed a response rate of 66.7% (8/12). According to the stage, the patients in stage III showed response rate of 50.0% (6/12), in stage IV showed a response rate of 37.5% (3/8). The total dose of FT-207 used in the study was 38.3g in the progressive disease group and 177.6 in the good response group. The group of long term administration of FT-207 and CQ included many patients with good response.

Evaluation of Stahl's ear, third crus of antihelix.

In our unit, 38 patients with Stahl's ear were seen in thirteen years. We found no characteristic distribution, in terms of either sex or affected side. We classified the condition into four types, and corrected it by excision of a narrow strip of cartilage and mattress sutures or in some cases by turning over the elliptical cartilage.

Attempts to increase the surviving length in skin flaps by a moist environment.

A method of raising a random patterns flap on the back of a rat was used to study the relationship between the surviving length and distance of dye penetration. It was found that the intraoperative dye distance was an accurate predictor of survival length in the Control Group. The effects of dressing were evaluated against the controls. An apparently significant increase in the survival length (beyond the dye distance) was seen with the "tunnel procedure" we used. It is suggested that a moist environment will favor survival of that portion of a flap where necrosis is usually seen when no dressing is used.
