Immunofluorescent antibodies in patients with bird-fancier's lung.

To assess its value as a screen for avian antibodies, indirect immunofluorescence against avian intestinal tissue has been to test sera from thirty-nine patients with documented bird-fancier's lung disease, thirty-eight asymptomatic bird-fanciers and 257 controls without known avian contact. Immunofluorescent antibodies occurred more frequently than precipitins among patients with BFL and asymptomatic bird-fanicers. Globular fluorescence within the mucus occurred only in patients with avian contact, although other fluorescent antibodies were seen with control patients. No particular pattern was confined to patients with the lung disease. When included in an autoantibody profile, indirect immunofluorescence provides a sensitive and convenient alternative to precipitin methods in screening for avian antibodies.

IgA and IgG reticulin antibodies in coeliac and non-coeliac patients.

Using indirect immunofluorescence, comparisons were made of patterns of staining of five tissue substrates with 28 RI reticulin antibody-positive coeliac sera, and 23 RI reticulin antibody-positive non-coeliac sera. IgA and IgG fluorescein conjugates were used separately. IgA antibodies were seen in 22 coeliac patients (78%) compared with three non-coeliacs (13%). Concomitant sinusoidal fluorescence (RS pattern) was seen more frequently with the non-coeliac (60.9%) than with the coeliac sera (10.7%). Such differences may help to distinguish those patients who should have a jejunal biopsy when using the reticulin antibody as a screening test for coeliac disease.

Ratio of urinary 6beta-hydroxycortisol to 17-hydroxycorticosteroids in patients with liver disease.

The ratio of 6beta-hydroxycortisol to 17-hydroxycorticosteroids in the urine of 73 patients with liver disease, and 53 controls has been measured. The mean ratio was significantly greater in the liver disease group (p less than 0.001), this elevation being most marked among patients with liver metastases and patients with acute hepatitis. We feel that the use of the ratio as a test of early liver cell dysfunction requires further evaluation, in particular in the early recognition of metastatic liver disease.

Smooth-muscle antibodies in infertility.

Sera from 77 infertile women and 77 post-partum controls were examined for autoantibodies by indirect immunogluorescence. 27 (35%) of the infertile group had smooth-muscle antibody compared with 2 (3%) controls. Significantly more infertile patients had anti-nuclear factor, and reticulin antibodies were also more commonly found in this group. The differences for organ-specific autoantibodies between the two groups were not significant. There was no correlation between the occurrence of smooth-muscle antibodies and immunofluorescent anti-sperm antibodies. The high frequency of smooth-muscle antibodies in infertile women requires further study to determine whether an underlying viral infection is involved.

Bird-fancier's lung and jejunal villous atrophy.

Sixteen patients with bird-fancier's lung were screened for evidence of coeliac disease by assessing their clinical features, red-bloodcell or serum folate levels, and serum for reticulin antibodies. Five of nine patients selected for jejunal biopsy showed villous atrophy, and in some this seemed to be a true gluten-sensitive enteropathy.

Controlled prospective study of the effect on liver function of multiple exposures to halothane.

Patients who had received halothane within a periof of one year and who required another anaesthetic were allocated at random to be given halothane or a control anaesthetic, the control being fiben using halothane-free apparatus. There were 76 patients entries in each group. Serum-glutamic-oxaloacetic-transaminase (S.G.O.T.) levels were measured before the anaesthetics and serially postoperatively for two to three weeks. The S.G.O.T. levels in the halothane group were significantly higher than in the controls. High levels were confined to patients who had had less than four previous halothane anaesthetics, increases above normal in the remainder and in the controls being rate. 1 patient in the halothane group had an S.G.O.T. of 440 I.U. per litre and hepatocellular necrosis on liver biopsy. 2 patients in the halothane group whose S.G.O.T.s rose to more than twice normal showed a similar reaction to re-exposure to halothane, although they had not shown a reaction to an intervening control anaesthetic.

Peripheral neuropathy and indomethacin.

A patient with seronegative inflammatory polyarthritis developed a predominantly motorperipheral neuropathy associated with the use of indomethacin. Three other cases of peripheral neuropathy associated with indomethacin treatment have been reported to the Committee on Safety of Medicines. In all cases the neuropathy regressed when indomethacinwas stopped. Peripheral neuropathy should be recognized as a rare complication of indomethacin therapy and considered in the differential diagnosis of a neuropathy accompanyingrheumatoid arthritis.
