[Sympathetic ophthalmia. Apropos of 5 cases].

PURPOSE: The objective of this study was to show the severity of sympathetic ophthalmia and the need for immunosuppressive treatment. METHODS: The study included five cases of sympathetic ophthalmia following ocular injury in three cases and evisceration in two cases. The ocular lesions included total uveitis in all cases with papillary oedema in two cases. The treatment was based on corticosteroid therapy, with immunodepressants in four cases. RESULTS: Functional prognosis was good in four cases, with final visual acuity between 3/10 and 10/10. CONCLUSION: Immunosuppressive treatment must be used on all cases of sympathetic ophthalmia with the goal of improving visual prognosis.

[Pleural fibrous mesothelioma. Apropos of a case].

A 27-year-old man with an uneventful history was hospitalized after fortuitous discovery of an opacity in the right lung field. The clinical examination revealed a multi-nodular euthyroidian goiter. Laboratory tests, including blood glucose, were normal. Computed tomography of the chest showed a pleural formation and associated pleural effusion. Complete surgical exeresis led to the diagnosis of benign fibrous pleural mesothelioma. Fibromas of the pleura are rare and usually discovered due to extra thoracic signs. The clinical course is long and often silent. Surgical removal is indicated and usually is complete.

[Antibioprophylaxis and itinerant ambulatory surgery in the milieu of endemic trachoma].

Between 1987 to 1992, 3,309 eyes had been operated on during the seventeen missions realized in Morrocco, by the Morroccan Itinerant Ophthalmologist Association. To prevent postoperative infectious endophthalmitis, asepsis was rigorous and local antibiotics prophylaxis were administered preoperatively, during surgery and postoperatively. Ophthalmic solutions (gentamicin, norfloxacin, or ofloxacin) were used. Six (6) eyes in our study developed infectious endophthalmitis (1.8). The fluoroquinolones are a new class of antibacterial agents with considerable potential in the treatment of ophthalmic infections. A systematic antibioprophylaxis with fluoroquinolones, is licit in itinerant surgery.

A logistic regression analysis of multiple noninvasive tests for the prediction of the presence and extent of coronary artery disease in men.

The incremental diagnostic yield of clinical data, exercise ECG, stress thallium scintigraphy, and cardiac fluoroscopy to predict coronary and multivessel disease was assessed in 171 symptomatic men by means of multiple logistic regression analyses. When clinical variables alone were analyzed, chest pain type and age were predictive of coronary disease, whereas chest pain type, age, a family history of premature coronary disease before age 55 years, and abnormal ST-T wave changes on the rest ECG were predictive of multivessel disease. The percentage of patients correctly classified by cardiac fluoroscopy (presence or absence of coronary artery calcification), exercise ECG, and thallium scintigraphy was 9%, 25%, and 50%, respectively, greater than for clinical variables, when the presence or absence of coronary disease was the outcome, and 13%, 25%, and 29%, respectively, when multivessel disease was studied; 5% of patients were misclassified. When the 37 clinical and noninvasive test variables were analyzed jointly, the most significant variable predictive of coronary disease was an abnormal thallium scan and for multivessel disease, the amount of exercise performed. The data from this study provide a quantitative model and confirm previous reports that optimal diagnostic efficacy is obtained when noninvasive tests are ordered sequentially. In symptomatic men, cardiac fluoroscopy is a relatively ineffective test when compared to exercise ECG and thallium scintigraphy.
