Oral and gastrointestinal manifestations of epidermolysis bullosa.

The mouth, oesophagus, and anus are often involved in dystrophic and junctional epidermolysis bullosa, but the frequency is unknown. Among 246 patients with epidermolysis bullosa, dysphagia developed in 76% of those with recessive dystrophic, in 20% of those with dominant dystrophic, in 15% of those with junctional, and in 2% of those with simplex forms. Lingual adhesions or microstomia occurred in dystrophic epidermolysis bullosa only, but were eight times more common in recessive than in dominant subtypes. These lesions are provoked by the trauma of eating and further reduce food intake, which exacerbates constipation caused by anal blisters and results in malnutrition. Management requires specialised multidisciplinary care.

Interpretation of a normal chest X-ray.

Thousands of chest X-rays are taken every day in hospitals throughout the county, but what indications suggest that a chest X-ray is 'normal'? This article, written by two radiologists, offers a step-by-step guide on how to interpret the normal chest X-ray, and will assist nurses in evaluating the significance of this common diagnostic test.

Intravenous digital subtraction angiography in the management of peripheral vascular disease.

All patients undergoing peripheral injection intravenous digital subtraction angiography (ivDSA) for peripheral vascular disease during 1987 have been reviewed (140 patients). The radiological distribution of disease was aorto-iliac occlusive (AI) 36 (23%), femoropopliteal (FP) 68 (44%), mixed AI/FP 36 (23%). Clinical diagnosis was correct for AI disease in 92%, for FP disease in 65% and for mixed AI/FP disease in 47%. ivDSA was the only investigation required for management in 111 of all 140 patients (79%), and in 33 of the 36 patients with AI disease (92%), but was less accurate in defining femoropopliteal disease, 22 patients (32%) requiring additional arteriography. Conventional arteriography was performed in 200 patients in 1982 compared with 100 in 1987. ivDSA is particularly suitable for the investigation of AI disease, a condition which is accurately detected by clinical examination, with a reduction in morbidity and bed occupancy.

Imaging in paranasal sinus disease.

This article reviews the basic anatomy of the paranasal sinuses and discusses the role of imaging in the spectrum of paranasal sinus disease. A separate section on facial trauma is included.
